NC Podiatrist Uses Mountain Top to Reach Distant Ham Radio Operators

So, what do you do when you have some free time? Nap? Golf? Garden? If you’re Dr. Richard Weinbaum, a podiatrist, you “SOTA.” SOTA — short for “Summit On The Air” — is an amateur radio operators program which encourages portable operation in mountainous areas. In other words, they are ham radio operators. With a portable radio and an acceptable mountain top like Mount Shepherd just west of Asheboro, radio enthusiasts can connect with other radio enthusiasts throughout the world, including the Ukraine, South Africa, and Australia.

Weinbaum chose Mount Shepherd for its height above sea level (1,168 feet) and because the location is designated as part of a point system. Get enough points and you are awarded a certificate leading to the prestigious “Mountain Goat” and “Shack Sloth” trophies. An Honor Roll for Activators and Chasers is maintained at the SOTA online database.

Last Day to Enter APMWA  Annual Student Writing Competition

The American Podiatric Medical Writers Association has announced its 27th Annual Student Writing Competition.

All papers MUST be non-technical in nature. Appropriate subjects include practice management, ethics, law, education, or any topic that would be suitable for a lay publication.

There is no word limitation. Papers will be graded for content, style, grammar, neatness, and overall impact.

First prize will be one thousand dollars ($1,000), sponsored by an APMA Educational Foundation endowment from Dr. and Mrs. Steven Berlin, and recognition in the APMA NEWS and the APMWA Newsletter. Honorable Mention Certificates may also be awarded.

This competition is open to ANY enrolled podiatric student.

Entries must be received by 4/15/13 via e-mail at bblock@podiatrym.com

TX Podiatrist Runs for City Council

Incumbent Joe Truman wants another term on Victoria City Council, but two other candidates want a shot at the job. The three candidates, Truman, former councilman Gabriel Soliz and podiatrist Dr. Andrew Young agree economic development is vital to the city, but from there, priorities and expectations begin to differ. District 5 covers the southern half of Victoria. The election is May 11.

Andrew Young, 39, said he wants to lead Victoria in the direction of the 2025 plan and increase transparency on the council. His priorities are three-part and hinge on each other: promote economic growth, lower the tax burden on residents, and catch up on growth and infrastructure projects. "It's the same concept we look at in medicine," Young said. "When we make decisions, we focus on quality of life."

Dr. Jacobs presented 3 hours of lecture on diabetic neuropathy to 400 primary care and internal medicine providers. This was the first time a podiatric physician had ever been invited to address this assembly.

Nixon? Not exactly. “I had a foot doctor (Dr. Martin Hazan) who was also the foot doctor for Joe DiMaggio. He brought us together in the mid-50s," said Kissinger.

RE: EMR Compatibility with Dragon Voice Recognition (John Scheffel, DPM)

Dr. Scheffel is finding out that trying to implement speech recognition into his EMR slows down his productivity. This is a fault of his EMR program. There are EMRs that are made for speech.  I use Textalk Enterprise. Most other EMR companies say that you can use speech, but perform as badly as Dr. Scheffel is experiencing. Most software programmers for EMRs have no clue how to program their EMRs effectively. If you had an issue with Dragon, they could not help you. They will tell you that you can use Dragon, but don't come to them if you run into issues.

First you need to go to your EMR company and ask them to help you construct a template in a logical fashion. A simple idea:  instead of "clicking" , use the tab key to logically go through your template and use speech to fill it in. You can also use speech commands like "click 1" with 1 being in a field, i.e. <1> and using <2>, etc., to fill in your template. Most good EMRs that use speech will have 25 or so commands embedded in their system. Maybe you need to get that list, if they have it.

You should also ask your EMR company for a demonstration of the use of Dragon with their EMR. Hopefully, this was done prior to purchase.

Based upon your post of having trouble with Dragon and your EMR, I strongly recommend contacting your EMR vendor. Most EMR products have been built with methods that provide better tools for using voice dictation than you are currently using. For example, in SamNotes, the Sammy EMR product, the workflow to create a note using voice dictation is just three easy steps. First, you select the new note feature, then you select the Blank note feature which opens up a box that has the patient's name and the date of service, and finally you activate your voice dictation software so that you can start dictating directly into SamNotes.

When you call your current vendor, please tell them what you want to do and have them demonstrate an easier workflow to allow you to easily dictate your note. If your current software does not have a good workflow and you wish to continue to use voice dictation, I recommend that you start shopping for a new EMR product.

I agree that as an APMA member, I am not thrilled with the APMA Pinterest page. But I want to be clear, I do not have any complaints with the APMA having a Pinterest page. Dr. Holmes accurately pointed out that Pinterest is a very popular social media site, and it is being used effectively  by organizations such as the AMA. They have created a very professional site that promotes their members to the public.

I believe the style of the Pinterest site that our organization has does not present a professional image. Just as there needs to be a level of peer review and quality for an item to be published in a professional journal, there needs to be a vetting process on the APMA site to avoid an unprofessional presentation to the Pinterest community that visits the APMA Pinterest page.

I applaud the APMA on the forward looking approach, and utilizing social media to educate the public about our profession, and urge them to reconsider the current content of the site. It should be a site that makes each and every member proud to be a podiatrist and helps to encourage the best and brightest to join our ranks.

It is human nature to "blame the captain" when the ship hits rough waters. After 9/11, many American screamed, "why didn't the president and congress stop Bin Ladin before -----"
 
What the public often fails to realize is that leaders of nations as well as major  organizations such as our own APMA are neither infallible nor have the gift of prophecy. At times, the best that the most gifted leader can offer is to examine the facts of a matter and make sure that the problem does not recur.
 
I find it especially unfair and even troubling when people point an ill finger at the executive director of the APMA. I have known and worked with Glenn for over 30 years and can swear that both he and members of his family and staff commit their every fiber and soul  toward the advancement of podiatry. I don't know how he manages to effectively run day-to-day APMA matters and handle every crisis affecting us nationally while traveling all over the country to train ever changing new leaders at all of the local regions. This is on top of making sure that our national convention, which caters to thousands of doctors, takes place every year and runs without a single hitch.

It is exactly this kind of response from people who may have a story about a "kind smoker" that I am talking about. There is nothing positive about smoking. Clearly this woman could not stop despite all of her cardiac problems. I am not saying that people who smoke are not kind or otherwise. Just because she was kind to you does not mean her second-hand smoke did not cause health problems for other people.

My mother smoked 2 packs a day for 20 years, but had the discipline to quit. I applaud her. I made no reference to someone's character except that...

In July 2004, Dr. Jay Otero a practicing psychiatrist, who resided and practiced in California, filed suit against me, The Foot and Leg Centers of Georgia, PC and the hospital in which the surgery was performed with the United States District Court, Middle District of Georgia, Macon Division. The Foot and Leg Centers of Georgia, PC and I were initially represented by counsel hired by their medical malpractice carrier. The malpractice carrier contested coverage in the matter and ultimately withdrew its defense of me and The Foot and Leg Centers of Georgia, PC., after obtaining a judicial determination that the claims against me and The Foot and Leg Centers of Georgia were not covered.

I have practiced in Florida for 23 years, and have had the opportunity to see Medicare EOBs that patients have shown me from their podiatrists "up North", when they are visiting our community during the winter. I have seen it all. My patients would show me the debridement codes, avulsion codes, etc. and ask me to bill the same way (which, of course, I do not). I am sorry, fraud is fraud... and many of you have been knowingly doing it for years to maximize reimbursement.

I remember during my internships and residency trying to learn what medical and podiatric pearls I could from my Chicago mentors... but I did see many students hover around the front desk and office learning how to bill. Certainly, trying to figure out whether to bill an exostectomy vs. condylectomy vs. resection of the base of a phalanx may cause one to compare reimbursements and RVUs in order to improve reimbursement... but to knowingly bill an avulsion code spraying ethyl chloride on a nail margin has always been fraud.

I agree, that what we get paid is criminal. Look at the most recent poll on how many of us were unhappy with podiatry. Perhaps the really happy are performing more avulsions.

RE: 104 Applicants Not Matched for Residency Positions

I have been reading with interest and some sadness about the plight of the 104 graduates who were unmatched for residency positions. I agree that this is a tragedy and hope some solution emerges to mitigate the enormous debt incurred and/or a pathway to licensure. While I believe questions should be asked of the ongoing admissions processes at the colleges or even the wisdom of opening new schools, I do not agree with the harsh criticism being leveled at APMA and its collective efforts to elevate the stature of the profession on whole.

For many a time, we as a profession, have sought to move our specialty to one seated at the same table with allopathic medicine. While some of the newer training seems excessive, it is what society demands. Ultimately, familiarity with this professional evolution must be appreciated by all those intent on matriculating into podiatry school. When one matriculates to a podiatry school, one is making a business decision, not an academic one.

Ideally, one should know where one will be heading after all one's education and training is completed. The classic example being the old model of offspring entering a family practice. Being realistically aware of what lies down the road for those thinking of pursuing a career in this field will help to avoid a similar crisis in the future.

I reviewed Dr. Eckles comments and beg to disagree with his conclusions and comments.

Firstly, I do believe there are too many colleges of podiatric medicine matriculating too many students. In my opinion, there is no shortage of podiatrists, at least not in the NYC area. In the NYC area, I can count no more than 20 orthopedic foot and ankle surgeons. For example, the spinal surgeons are all thriving. They are thriving because there aren't many in the market to provide their services and they have a strong MD and hospital referral base.
