Neuropathy is Multi-Factorial: AZ Podiatrist
 
The tingling, burning, and numbness in the extremities, the loss of balance and coordination, and the constant shadow of pain all point to an unidentified condition for many. Neuropathy is a complex term that often leads to a plethora of unanswered questions for many individuals. Despite the complexity of this field, dedicated clinicians like Alan Shih, DPM, a board-certified podiatrist with over 15 years of experience, guide these often confused individuals with pioneering work, transforming the landscape of neuropathy treatment.

Peripheral neuropathy, with its diverse set of symptoms including numbness, burning, pain, and weakness typically starts in the legs and feet. While diabetes is a common cause, Dr. Shih emphasizes the multitude of contributors, including alcohol, cancer treatments, viruses, heavy metals, thyroid abnormalities, and nerve entrapment. The reality is that neuropathy often results from a combination of these sources, making a one-size-fits-all treatment approach destined for failure.

The front desk staff is essential for checking in patients, collecting copays, and verifying insurance. But there’s more to the story, and out of sight doesn’t mean out of mind. Much of the heavy lifting for billing and collections is done in the back office, so it’s even more important to have a deep bench of talent, says Tammie Olson, manager and coding and compliance strategist at Management Resource Group.
 
3. Analyze insurance denials and develop processes to reduce them.
 
Is there misuse of modifiers? Are denials related to medical necessity? Outdated codes? If you make the effort to track the denials and see where you’re making mistakes, the time spent will be rewarded with higher and prompter payments. Similarly, have processes in place for correcting and refiling denied claims in a timely manner.

I recently (2 weeks ago) had bilateral carpal tunnel surgery done at one time. It took about 45 minutes under MAC. No post-op splints. I had minimal discomfort and was back in the office seeing patients (no palliative care) at 10 days post-op (included the weekend). I will be back doing surgery tomorrow at 17 days post-op.
 
My nerve studies were very abnormal and my fingertips constantly numb. Now I have no issues. I highly suggest you see a trained hand surgeon and talk about fixing the problem before it gets worse.

For those of us who trained under Allan Spencer, DPM, we are forever in his debt. He was a biomechanics guru from the 1960s through his retirement. He was always a gentleman and was generous with his knowledge and encouragement. I had the honor of not only being his student, but also his colleague when I taught with him at OCPM from 1980 – 1997. A quiet man, he nevertheless was passionate about podiatry and the role of pathomechanics and biomechanics. My deepest condolences to Scott Spencer, DPM and his entire family.

I was very saddened to hear about the passing of Dr. Allan Spencer. He was a true gentleman, a great teacher, and I remember him fondly from my years at OCPM. I still use one of his lines “if a patient brings you a bag of shoes, you are never going to make them better.” RIP Dr. Spencer.

I was saddened to hear of the passing of Dr. Allan Spencer. He was my biomechanics professor and mentor at OCPM (class of 1980), and with his help and guidance, I was able to go on to a successful full-scope of biomechanics and surgery practice for 43 years. Dr. Spencer was a wonderful teacher and mentor and always had time to help and advise us during both class and in clinic. I will always think of him with admiration and fondness. He gave back to our profession, which is something we all should aspire to. He will be missed.

Amol’s response is a bull’s-eye of what is needed. I’ve been saying this for over 15 years and I know Barry Block has been saying for over 40 years that the only solution for our profession is to amalgamate into mainstream medicine as our own specialty. The schools should convert to giving an MD degree and we push for our own specialization in the American Board of Medical Specialties. This would facilitate physician extenders easily incorporated into our practices to handle the palliative and routine aspect of our profession, with oversight directly. All issues with credentialing, licensing, and fee disparity would be eliminated.  
 
I join with Amol willing to volunteer and help this transition into the future.

It has always [since the late '80s] been my opinion that the profession of podiatry should START with one certification board credential [Board Certified Podiatrist] and then have sub-specialties. This is analogous to the MD/DO pathways. This allows all competent practitioners the ability to be credentialed. Some will have one certification, some will opt for two, and others will opt for three. I see this as a win-win for all.

I am following the discussions about board certification which seems to surface every ten years. Remember, the primary purpose of certification is to provide confidence to the public that the practitioners holding these documents are competent in their field of expertise. Public safety should be the overwhelming goal as our profession seeks to satisfy those podiatrists looking for parity among their peers. I do not see how one certification board can accomplish our responsibility to the public to ensure quality, safe, and responsible care in all the facets of podiatric medicine. 
 
Surely we can appreciate the need to recognize those individuals who have been trained, educated, and tested to render skills above the basic podiatric medical therapies to the public. Pathways for these skill sets are available now, unlike many years ago.
 
Along with the general public, hospitals and surgical centers need objective criteria to adjudicate applicants requesting privileges. These are indeed privileges, not “rights” of practitioners in all branches of medicine, and we are no different. I commend our leaders as they devote much time and energy in this endeavor. Remember, public safety must be the driving force behind these discussions!

I concur with all of the kudos for Ross Taubman. He has given generously of his time and heart for this profession. I am grateful and proud to call him a friend. But we would be remiss if we failed to acknowledge the deep commitment of Betsy Herman Taubman, who in the late 1980s and 1990s was known as the "Mother of APMSA," the student organization.   
 
I was fortunate to be the representative of my DMU Class of 1991. Betsy, a APMSA director, herded us like the unruly cats we were. She taught us how to serve, not how to take advantage of our positions. I think back to my small APMSA group. We ended up with two podiatric deans, a university provost, several APMA presidents, and many successful practitioners who relieved pain and suffering of thousands of patients. Thank you, Betsy, for all you contributed to your "kids" and the profession. To Ross and your son as well. You trained us well! Enjoy this well-deserved time together!
