IA Podiatrist Discusses Toe Cramps
Toe cramps are one of the most common foot-related complaints. They occur when the muscles in and around the foot and the associated tendons involuntarily contract but don’t immediately relax, explains Kelly John, DPM, MHA, a board-certified podiatrist and surgeon and assistant professor at Des Moines University’s College of Podiatric Medicine and Surgery.

Sometimes, toe cramps occur because of inactivity. For example, Dr. John says that if you don’t regularly use the muscles in your feet and then suddenly increase your activity level, cramping might occur. Similarly, if you exercise regularly but don’t cool down afterward, your muscles are more likely to tense up. If you plan on trying an electrolyte supplement, Dr. John encourages you to speak with your doctor first. Although rare, “a sudden shift in electrolytes can cause cardiac arrhythmias (irregular heartbeat),” she explains.

The newest medical school in central Florida, Orlando College of Osteopathic Medicine (OCOM), celebrated its Grand Opening March 10 and is accepting its inaugural class of students for Fall 2024. Hundreds of guests including the founders, VIPs, faculty, staff and students were in attendance. Chet Evans, DPM was the early consultant on the location for, and development of, OCOM and has a faculty appointment as clinical professor.

We did it! Thank you to the almost 1,500 podiatrists and students that requested to co-sponsor the One Board APMA Policy Proposition 2-24. This was a first in APMA history! The proposition passed the House of Delegates (HOD) with 63% of the vote. This proposition sets the table to bring the two CPME-recognized boards together into a single certifying board in Podiatric Medicine AND Surgery, just like our residency programs.
The details will be worked out and presented to the next HOD in 2025. I look forward to working with the ABFAS to make the result an inclusive organization of all those already certified, one that avoids division in the profession, and gives all new candidates an equal and fair opportunity to achieve the status of board certification in podiatric medicine and surgery, while promoting the high testing standard of residency training and fulfilling our obligations to protect the public. I pledge to keep you informed as this develops. We couldn’t have done this without your action!

While Dr. Spinner's insights, along with those of Drs. Tomczak, Oloff, Lombardi, and others, are undeniably respected due to their substantial contributions to podiatry, it's crucial to acknowledge that a profession's advancement can manifest in varied forms. There exist many individuals outside the well-known circle of seasoned podiatrists who have significantly propelled the field forward in perhaps unconventional or less visible ways. Their efforts, whether through innovation, academia, or other professional avenues, merit equal respect and consideration. Disregarding or minimizing these contributions because they diverge from one's perspectives echoes Mark Twain's humorous observation about limiting one's statements to the confines of facts.
As a founder and twenty-year director of a residency program and a podiatric sports medicine fellowship, as well as a member of medical/orthopedic executive committees at several major hospitals and surgery centers, where I have credentialed numerous podiatrists and MDs, and as a contributor to our journals over four decades, I value the importance of diverse contributions. The growth of our profession relies not only on the achievements of a select few but on the varied and unique inputs of many, each bringing distinct perspectives and innovations. Indeed, fresh ideas often hold remarkable potential. I’ll however be very clear that I have no political, or personal agendas. I would desire to have ABFAS diversify its certification into several inclusive components. In my opinion, they have proved themselves to be the podiatric certifying “Gold Standard”.

Dr. Gurnick's concerns about cases of poor foot care patients receive when they go to urgent care centers, and often at emergency rooms, is unfortunately correct. My own internist once lamented to me about the poor level of care patients with general medical conditions get at urgent care centers. We do, however, need to separate hospital emergency rooms from "street-corner" urgent care centers. If the hospital is large, one would expect access to numerous doctors in different fields including podiatry. The problem with hospital ERs is that there is often a long wait before being seen by a physician or even a triage nurse.
My personal experience with taking my ailing parents to emergency medical facilities is that urgent care clinics tend to be run by PAs and nurse practitioners. These medical professionals while good at triage and some treatments, cannot be expected to be experts in all facets of medicine and surgery. Hence they sometimes make mistakes or ideally will tell the patient to go to the local hospital if the condition looks serious. This is a problem that Dr. Gurnick cites from the perspective of being a doctor. Unfortunately, we will all have to deal with this problem at some stage when we or our family members become the patients.

Several years ago, while I was under a “different” malpractice insurance, I had an issue and needed some help. I ran into Ross at an APMA HOD meeting and told him my story. He listened and counseled and even gave me his cell number for follow-up. All free of charge, as I was not a PICA member. No sales pitch or pressure, just genuine concern and guidance. If that doesn’t tell you what Ross is “all about” I don’t know what does. Needless to say, I’ve been a loyal PICA patron ever since. Happy retirement Ross!

Once again, I am astonished and disheartened by the lack of preparedness on the part of providers to deal with a crisis involving their income. Did anyone not learn anything from the COVID shutdown in 2020? As a rule of thumb, each practice should have 3 months or more of income set aside in order to deal with any adversity that may befall a practice...such as a pandemic, or a cyberattack on their clearinghouse, or anything else that can be imagined. This is the same financial advice given to anyone who has a job they count on to pay their living expenses. I have 3 or more months of income in savings. I know I do, and I am not a doctor.
I know the running "joke" is doctors make the worst business owners, but this is ridiculous, in my opinion. If you own a practice, you better make sure you are a good business owner, or you don't have any business owning a practice. I have been reading in the news recently that various practices are going to have to close down/let staff go, because of the...
