NY Podiatrist Discusses Buying Shoes for Diabetics
 
For people with particularly bad cases of neuropathy, healthcare providers may prescribe footwear specifically designed for people with diabetes, which can often be customized to meet an individual’s needs. According to Samantha Landau, DPM, a podiatrist at Touro University’s New York College of Podiatric Medicine, diabetes shoes can be beneficial for people with deformities or a history of ulcers. “However, many well-designed athletic or walking shoes can be equally appropriate if they meet biomechanical and protective requirements,” she said.

If you’re shopping around for your next pair of walking shoes, popular brands like Brooks and Hoka are a great place to start, but the most important thing is to find something that works for you. CA Podiatrist Receives ASMST Achievement Award
 
Amol Saxena, DPM, MPH was awarded the Achievement Award at the Second Annual American Society for Medical Shockwave Treatment (ASMST) Conference, in Boston, MA, March 13-15. Dr. Saxena was presented the award by fellow board member Jay Spector, DPM who spearheaded the ASMST Awards. The Educator Award went to Adam Tenforde, MD, who has published several papers with Saxena and presents at Complex Foot and Ankle Reconstruction conferences.

Dr. Saxena along with Dr. Gerdesmeyer conducted the FDA clinical trial that led to the approval of focused shockwave in the U.S. The two have collaborated on several papers with Drs. Tenforde and Liz Bondi, DPM. All three podiatrists are members of the 9-member multidisciplinary ASMST board. DPMs take the same certification test as MDs by the International Society for Medical Shockwave Therapy.

We start by knowing the signs and understanding what to look for. Any athlete with a BMI below 19 should be considered an immediate red flag, but numbers aside, patterns matter most. Spotting concerning behaviors early—and having open, honest conversations about an athlete’s motivations, goals, and long-term health—is crucial.
 
One of the most effective interventions we can offer is early referral to a mental health provider and a nutritionist. This team-based approach teaches athletes how to properly fuel and strengthen their bodies, while also helping them build the psychological tools needed to embrace healthier habits. It’s support that protects not just performance—but longevity in the sport.

Query:  Can an E/M Code be billed for a Fracture Complication?
 
The patient was seen in the office with a dislocated fractured toe. Closed reduction was done, which corrected it. Three days later, the patient came back to the office with loss of the correction. A discussion was had on the proper way to treat this which will be with a Kirshner wire. The discussion lasted 30 minutes. Is there a way to bill the E/M to review the closed reduction surgery or is that within the global period of the original closed reduction?

Response from Codinghelpline.comUnfortunately, an E&M cannot be billed unless the prior procedure caused new damage that was not previously identified. If the diagnosis remains a dislocated fractured toe, you would add the appropriate S code to indicate sequela. Remember, you performed a closed reduction procedure with a 90-day global period. That procedure failed, and the patient is now being taken back to the operating room. This represents a repeat procedure for a related condition.
 
For the new procedure, append Modifier -78 – Unplanned Return to the Operating/Procedure Room by the Same Physician Following the Initial Procedure for a Related Procedure During the Postoperative Period. Keep in mind that when Modifier -78 is used, a new post-operative period does not begin.

Duality and contradiction summarize the opinions expressed in PM News with regard to the future of podiatry. Like the opening sentence of Dickens A Tale of Two Cities, “it was the best of times, it was the worst of times, it was the age of wisdom, it was the age of foolishness, it was the epoch of belief, it was the epoch of incredulity, it was the season of light, it was the season of darkness, it was the spring of hope, it was the winter of despair.”
 
These are indeed the best of times to be a podiatric physician. Utilizing Medicare reported RVUs, the average podiatrist should earn a minimum of $269,900 annually. An eclectic array of the full integration of podiatry exists. Podiatrists as members of the IWGDF. Podiatrists as committee chairpersons in the ADA. I would tell the patient who is concerned who will actually do her colonoscopy procedure to look the doctor directly in their eyes and ask him or her. I doubt that many would lie. Based on their answer, she can decide whether to proceed with the procedure or not.

Dr. Caringi asks a simple question with a multitude of answers and/or reasons. I too graduated in the early 1980s (1982) from Scholl College and had excellent instructors teaching me biomechanics. What happened?
 
3-year residencies focused on surgery. There is a certain panache when called a “surgeon” and we all have egos. Schools changed their marketing strategies focusing on surgery. Follow the money, as always. In the ‘80s and into the ‘90s, surgery was fairly well reimbursed. Colleagues became lazy and had/have the medical assistant cast/scan and use Biofoam (what a joke) to build orthotics. Orthotics are not covered by insurance
 
The surgical portion of my practice was never more than 15% of my revenue. I loved the biomechanical aspect of my practice and it provided me with an above average...
