TN Podiatrist Discusses Challenges of Being a Woman in a Man's World
 
According to Gina Mendoza, DPM, "There were many challenges that I had to face being a woman in a 'man's' world. I am a surgeon and even in residency, there were certain residencies that were considered 'boy's clubs.' This meant that the males were provided with more opportunities to perform the challenging surgeries, and unfortunately, the female surgeons were almost discouraged from applying to these residencies," she said.

Turf toe is an injury to the 1st MPJ. Its etiology is not neuro-mechanical. Are you referring to another condition called turf toe/hallux limitus?

I presume Dr. Dananberg's treatment recommendations are referring to "functional" hallux limitus and not for a 1st MPJ with moderate or advanced arthritic changes? In my experience, these latter patients are often in more pain with MOVEMENT and DO benefit from carbon fiber plates...and sometimes surgery is indicated.
 
I'm curious what Dr. Dananberg recommends treatment-wise in these arthritic cases?

Dr. Dananberg and I have spent years proclaiming that the forefoot medial column and peroneus longus are the primary keys to functional lower extremity biomechanics and not the subtalar joint or rearfoot pronation. The problem has four components: a structural defect in the forefoot, a myofascial component allowing soft tissue flexibility, a peroneus muscle engine atrophy of disuse, and a neural strategy that has lost motor control of PL.
 
This requires structural optimization, muscle engine training, myofascial restoration, and neural strategy rebooting. Custom or customized corrective foot orthotics, stabilizing and compressing, kinesiology taping, foot centering pads, and compensatory foot centering training are all required.
 
All else including isolated foot orthotics, training, and surgical procedures are Band-aids or will biomechanically create new and possibly worse biomechanical sequelae in the feet and elsewhere over time.

RE: There Needs to be a Health Insurance Revolution

I have been telling all of my patients for the last few years that unless there is some sort of revolution or revolt against the so-called health insurance companies, both doctors and patients will continue to get abused financially. Every year, premiums go up, co-pays go up and doctors often get paid less for doing more. The insurance companies keep increasing their revenues. UnitedHealthcare made about $15 billion in revenue. I now refer to insurance companies as money transferring entities. They take money from people and then keep most of it, and then give a little bit to the healthcare providers who actually help people.
 
The reason they get away with it is because no one is making it a big enough public issue. This is likely because of lobbyists. If we were to have a revolution similar to...

This is a good opportunity to step back and look at the passing of the recent resolution on CPME 320. As I mentioned previously, there is not much difference to me whether we are DPMs or MDs. I understand the political and the billing consequences of same, but the reality is the educational directives need to be increased because parity should be based on the understanding and not just on a name change.
 
The podiatric profession has made tremendous leaps in education and changes in residency and related fellowships over the past 40 years. This is another step forward so that our understanding and the treatment of lower extremity pathology cannot be challenged and is the very best worldwide. We strive in that direction and I do believe this is a step to help move that forward. I applaud all the movers and shakers to help put this together and the house for voting as well.
