VA NEWS
GA VA Launches Home-Based Telemedicine Podiatry Pilot Program

The Atlanta VA is making it even easier for Veterans to get their feet checked. A new podiatry pilot program at the Atlanta VA Health Care System is the first in the country to meet Veterans where they are – at home. The Veterans most at risk for amputation sometimes can’t leave the house to see a podiatrist, in collaboration with the VA Southeast Network Clinical Resource Hub, the National VA Podiatry Program, and Atlanta Intermediate Care Technicians (ICT).

Dr. Nichol Salvo

“One Intermediate Care Technician will usually see 10-12 patients at the clinic. Because of travel and geography, the ICT will only see 2-3 patients a day,” said Nichol Salvo, DPM, Chief of Podiatry and Program Director of Limb Preservation at the Atlanta VA. “With that said, these are patients who have no ability to see podiatry otherwise and many are the most co-morbid and at greatest risk for amputation.”

PODIATRISTS AND DIABETES
Sensory Substitution Strategies Help Restore the Gift of Pain: CA Podiatrist

“I can’t think of anything that is more of an expression of humility than looking after someone’s feet,” says podiatric surgeon David Armstrong, DPM. A global leader in limb preservation for more than 30 years, Armstrong has helped transform the theory and practice of diabetic foot care. "We believe we can eliminate preventable amputations over this next generation,” Dr. Armstrong says. “But more important than keeping your limb is keeping your mobility and independence. That transcends limb preservation."

Dr. David Armstrong

“Folks with neuropathy don’t act like other patients,” Armstrong explains. “When you take away that painful feedback, then foot problems understandably become number 11 on everyone’s list of ten biggest priorities.” New sensory substitution strategies, from low-tech “foot selfies” to AI-driven circulation monitors, are emerging to refocus patients’ attention on foot health. “If we can just bump that up a little bit—if we can just make it ninth on the top ten list—then we can eliminate a lot of amputations.”

RESPONSES/COMMENTS (OBITUARIES) - PART 1
RE: The Passing of Thomas M. Pokabla, DPM

Dr. Pokabla was one of 60 attendings who brought cases to the very busy Austintown Surgery Center covered by the Youngstown PSR-12 residents when I was there 1988-89. He was always cheerful, funny, and often times turned his whole case over to the residents. The five residents schemed and fought to scrub his cases.

I also rounded with him at the Warrentown rehab hospital where he provided routine foot care and the staff and patients loved him. I regret that I never made the effort to thank Tom and all the other attendings that took time out of their busy lives to train residents.

RESPONSES/COMMENTS (OBITUARIES) - PART 2

I am sad to learn of Dr. Boxer’s passing. Although he was no longer on NYCPM faculty when I was a student, I had the privilege of being a student of his through his co-directing of my residency program. He had a profound influence on me with my visits to his office. I was first amazed at the efficiency of his practice with the small footprint of the physical space.

What he left me, more importantly, was his contrasting approach to a podiatry practice that I witnessed from other practices. From his consultation room that he used extensively with his new patients to his use of diagnostic testing that he performed without being used for superfluous billing. To the idea of thinking like a problem solver as any physician will do for our patients’ concerns. May his memory be a blessing.

RESPONSES/COMMENTS (MEDICAL MALPRACTICE)
RE: "Nuclear" Malpractice Awards
From: Allen M. Jacobs, DPM

So-called "nuclear" malpractice awards may be outliers, but statistical protection is not absolute protection. As podiatrists do more and more surgery, engage in more complex surgeries, treat more patients with diabetes, manage more ulcerations, and treat more patients who have multiple co-morbidities, the potential for poor outcomes and large malpractice judgment would be expected to increase. I was a defense expert in a recent case in which the plaintiff asked for $76,000,000. The jury came back with a $4,000,000 verdict.

According to AI-generated data, in 2023 there were 57 malpractice verdicts greater than $10,000,000. According to the international reassurance company TransRe, mega malpractice awards are increasing in number. According to author Alicia Gallegos, 2023 "blew away every record previously set among high malpractice verdicts." Although Florida, New York,...
