Ribotsky, Lamb to Be Inducted into the PM Podiatry Hall of Fame
 
Podiatry Management Magazine has announced that Bret Ribotsky, DPM, and James Lamb have been elected to receive the 2016 PM Lifetime Achievement Awards, and will be inducted into the PM Podiatry Hall of Fame on Friday July 15th at the Hall of Fame Luncheon held in conjunction with the APMA Annual Scientific Meeting [The National] in Philadelphia, PA.

Dr. Ribotsky, the 33rd podiatrist to receive this honor, is a noted lecturer and host of Meet the Masters. James Lamb, the 10th non-podiatrist to be inducted into the PM Podiatry Hall of Fame, is the longtime executive director of the American Board of Podiatric Surgery (Now the American Board of Foot and Ankle Surgery). Full biographies of both honorees can be found in the March Issue of Podiatry Management.

MI Podiatrist Named President of MPMA
 
Dr. Crystal Holmes was named president of the Michigan Podiatric Medical Association, (MPMA). The announcement was made at the MPMA’s Great Lakes Conference. Holmes will serve as president of the MPMA through January 2018. She will guide the association’s management group, Kindsvatter, Dalling & Associates, and take the lead in pursuing on-going educational programs.

PODIATRIC PRODUCTS IN THE NEWS
AMNIOEXCEL® Amniotic Allograft Membrane Study Published in WOUNDS
 
Derma Sciences, Inc., a tissue regeneration company focused on advanced wound and burn care, has announced the publication of a peer-reviewed comparative clinical study. It compared AMNIOEXCEL® Amniotic Allograft Membrane plus standard-of-care (SOC) with SOC alone for the closure of hard-to-treat diabetic foot ulcers. The results are statistically significant and clinically meaningful, with the closure rate at six weeks in the AMNIOEXCEL® plus SOC treatment arm, far exceeding SOC alone treatment.
 
The study, “A Prospective, Randomized, Controlled, Multicenter Evaluation of the Use of Dehydrated Amniotic Membrane Allograft Compared to Standard of Care for the Closure of Chronic Diabetic Foot Ulcers,” was published in the peer-reviewed journal WOUNDS. The publication is expected in the March 2016 print issue of the journal. The study was funded by Derma Sciences and the paper was authored by Robert J. Snyder, DPM; Kenneth Shimozaki, DPM; Arthur Tallis, DPM; Michael Kerzner, DPM; Alexander Reyzelman, DPM; Dimitrios Lintzeris, DO; Desmond Bell, DPM; Randi Rutan; and Barry Rosenblum, DPM.

San Diego Doctors Win Long Campaign for Higher Medicare Pay
 
San Diego area doctors seldom complain to their patients about low Medicare pay. But for two long decades, many have complained bitterly to each other. That’s because an old and complex federal formula reimburses physicians here as if they were practicing in “rural” areas, and assumes their costs of rent and staff are low, resembling those in Fresno or El Centro rather than Anaheim or Los Angeles.
 
Now, after a tempestuous 14-year fight and a $5 billion lawsuit that got as far as the U.S. Supreme Court, a congressionally approved fix will gradually increase doctors pay in San Diego County and 14 other California counties that were inappropriately classified. Starting Jan. 1, the new rule, embedded in the Protecting Access to Medicare Act of 2014, will pay them higher rates, more like doctors practicing in Orange County and other higher cost urban areas of the state. Other categories of health providers who bill Medicare, such as nurse practitioners, podiatrists, and optometrists, may expect more money too.

Value-Based Milestones Signal Change in Medicare Reimbursement
 
Two aggressive milestones to migrate Medicare providers to value-based healthcare are on the horizon: 
  • In 2016, CMS expects 30 percent of Medicare fee-for-service (FFS) reimbursement to be tied to alternative payment models such as accountable care and bundled payments.
  • Also this year, the federal payor wants 85 percent of Medicare FFS payments to be based upon quality metrics.
"If you are a provider, or working with providers who accept Medicare beneficiaries, it's really important to know these changes are coming," advises Elena Tkachev, director of ACO analytics for Collaborative Health Systems (CHS). "It will be the responsibility of physicians to participate in these payments because no matter what happens, this change is coming."

You are right! You are a resident and there to learn. Much better that you learn from others' mistakes than your own! You should first ask questions of this “incompetent surgeon” who should be willing to discuss why he/she chose to do whatever it is you feel inappropriate. Many times out here in the real world, we “bend the rules” to accommodate individual needs of our patients. Prepare your questions well. Know the literature and be HUMBLE. If he/she blows you off, then go discuss it with your residency director. That is part of their job!

While in an externship during my 4th year of school over 30 years ago, the residency director sent me to observe the office practice of one of the Attendings.  His practice, and ethics, were appalling.  He showed up hung-over, did office procedures on people who didn't need them, and I actually heard him say, "I charge $1,500 for this procedure, but I'll do it for $1,200 cash, and $1,000 if you don't require a receipt"!  When I returned to the hospital, the director asked me how it went, and I told him what I had observed.  His reply was golden: "I send you there to see what NOT to become. Guys like him cause the rest of us to look bad. I believe that nobody is totally worthless, they can always be a bad example." You are in training to learn. Learn what to do, and what NOT to do. Don't be that guy!

I think most of us have been in a similar situation. The best way to go about it may be to learn on your own and share your knowledge with faculty in an open nonjudgmental way. Start with saying things such as, "I'd like to know what you're thinking. Can you explain what your thought process was? I'm not sure I completely understand, or I've read somewhere that this was one kind of approach. What do you think about it?" Be wise with your approach and the words that you use because you never want to come off as condescending, insubordinate, or closed-minded. You'll see that residency is about learning what to do but also what not to do; so if you follow through on the patients and realize the outcome was not optimal, realize that was an educational opportunity. What better way to learn than through someone else's mistake? However if patient harm occurs often, then that's an issue to take with your residency director.
 
The advice that I valued the most from my senior residents at the time was learn as much as you can, read as much as you can, and learn from a lot of people. That way, you'll have all the tools necessary to develop your own style when you begin to practice. In the meantime, try to enjoy the process, both the good and the bad.

I have been using Pediplast for about 25 years. It will work well in certain situations like the interdigital corns you mentioned. It stays in place better than OTC silicone pads so if those do not work or patients complain that they fall out, try the Pediplast. I charge $20 per device. The device will last about 6 months before it wears out. Insurance will not usually pay for them, so let your patient know this ahead of time. It is easy to use but there is a little learning curve.  
 
Take out the amount of putty you want. I don't measure things any more. Lay a bead of hardener across the top the length of the putty. If you don't use enough, it will never set up. If you use too much, it will set up too fast and get too hard. Work the hardener into the putty with your fingers. You have to work it in well. You have about 5 minutes before it sets up. You might want to wear gloves. The putty sticks to the gloves, but the hardener will turn your fingers red. Once the hardener is mixed into the putty, put the putty between the toes and squeeze the toes together to form the putty. Remove excess putty. This will make it more comfortable for the patient and save you having to make adjustments later.
 
You can grind it on a SaniGrinder, but you'd have to let it set for a day or so to do it, and you don't want to be doing this. Get it right the first time. I used to put a plastic baggy over the forefoot and have patients put on their shoes and walk around with it so it fits well. With experience, you won't have to do this, but at first, it's not a bad idea.

I note the recent posts in PM News from Dr. Lai and the response from Dr. Lullove. I can certainly understand why he is a board member of one of the organizations and would want to respond. In that regard, I’m not on the board of any of these organizations and have no vested interest. I join those which I believe best benefit me and podiatry. To that end, in wound care, I am a member of the Academy of Physicians in Wound Healing and no other organization in the field. 
 
The reason for this was pointed out in the original article by Dr. Lai. APWH is the only physician-specific membership organization in wound care providing physician-directed education and advocacy. The other groups mentioned by Dr. Lullove, as he clearly indicates, represent multiple fields practicing wound care. But nurses and others make up the vast majority of their...
