APMWA Announces 35th Annual Student Writing Competition
The American Podiatric Medical Writers Association (APMWA) has announced its 35th Annual Student Writing Competition.
-
All papers MUST be non-technical in nature. Appropriate subjects include practice management, ethics, law, education, or any topic that would be suitable for a lay publication.

There is no word limitation. Papers will be graded for content, style, grammar, neatness, and overall impact.
-
First prize one thousand dollars ($1,000), Second prize seven hundred and fifty dollars ($750) Third prize five hundred dollars ($500) sponsored by an APMA Educational Foundation endowment from Dr. and Mrs. Steven Berlin, and recognition in the APMA NEWS and the APMWA Newsletter. Honorable Mention Certificates may also be awarded.

Specialization in Young Athletes Can Lead to Overuse Injuries: IL Podiatrist
According to Robert Weil, DPM, “Today’s epidemic of overuse injuries due to specialization affects both lower and upper extremities, across the board, in all sports at all ages. As the world of youth sports has grown dramatically, so have these injury problems. Overuse injuries cause a significant loss of time off the field, but more importantly, they threaten future sport participation which could inadvertently lead to increased obesity. These young athletes are at increased risk because growing bones are less resilient to stress and children’s awareness of symptoms as signs of injury are limited.”

“If specialization is the decision, then it is important to include off-sport conditioning that helps to develop the overall athlete and strengthen against these repetitive motion injuries. Concentrate on what Bob Gajda, famous sports therapist, called ‘strengthening the opposites and stabilizers.’ In other words, condition the muscles and tendons that help work against those repetitive motions. Athletic trainers, physical therapists, and personal trainers can help devise these programs. This kind of training is also helpful for multiple sport athletes.”

She completed her podiatric residency training in June 2011 at the Veteran’s Affairs/Stanford Hospital in Palo Alto, CA.

CA Podiatrist Publishes Book on Diabetes
Michael J. Marcus, DPM recently published his book entitled The Diabetic Foot: Preventing Loss and Amputation, A Pictorial Approach. Dr. Marcus wrote this book primarily for diabetic patients so that they would have a solid resource for their condition.

Through years of practice, Marcus found that patients often would come to him initially, and his practice would lose them due to insurance, or the patient would not remember the information from the visit, and often, when returning, the patient would have a more severe case, leading to amputation.

Improve Your Patient Experience With These Steps - Part 5
5. Commit to Timeliness
Be prompt in answering phone calls, don't keep patients on hold more than 30 seconds, and jockey them around from one person to the other. Also be diligent in returning phone calls and e-mails within a standard timeframe. Communicate the established timeframe to providers, staff, and patients. Again, this reinforces accountability.
Timeliness typically suffers the most when patients are kept waiting for their appointments. Check them in promptly and let them know how long they are likely to wait. Most practice management systems can track patients through the system beginning at check-in. It is important that staff understand this feature and use it consistently. It is a great tool and provides reports that measure performance in work flow and also helps identify any hiccups that keep patients waiting unnecessarily. It's an opportunity for everyone to improve patient service and patient flow.

Risk Factors for Melanoma by Anatomic Site
The authors analyzed data from 2617 first invasive melanomas (975 controls) from 2 population-based case–control studies in Australia and the UK. They found that the incidence of melanoma was highest on the trunk (35%) and lower limbs (34%) and was lower for the upper extremities (20%) and head and neck sites (11%). Men had a higher frequency of melanoma on the head and neck, whereas women more frequently had disease on the extremities. Melanomas occurred more often on the head and neck than on any other site in patients older than 70 years. Higher density of nevi was associated with higher odds for melanoma of all sites, but a stronger association was seen with many nevi and melanoma on the trunk and extremities. Very fair skin type was associated with melanoma on non-truncal locations.
This study sheds light on the factors that affect the risk of developing melanoma and how they vary based on the anatomic site of the melanoma. These data support the dual-pathway hypothesis, which holds that truncal melanomas are more strongly related to nevi and intermittent sun exposure, while head and neck melanomas are more related to chronic sun exposure.

Query: Paying for an Interpreter for a Hearing-Impaired Patient
A deaf patient called and requested an appointment. We made it, and then the patient requested a sign language interpreter be present. The interpreter charges $160 a visit. I assume I have to supply an interpreter. Is it okay for it to be a virtual video interpreter instead of an in-person one? Do Medicare and Medicaid pay for this service? If the patient misses the appointment, who is responsible for paying the interpreter?
Editor's comment: PM News does not provide legal advice. Under the American with Disabilities Act (ADA), a provider is responsible for the costs of a requested interpreter. This cost is not covered by Medicare or Medicaid and must be paid even if the patient is a no-show. You can use a virtual video interpreter as long as the patient consents to it.

I do not advocate substituting diagnostic ultrasound for x-rays, but it does offer a more expeditious adjunct study for the astute practitioner who has ultrasound in their office. I have used it for identifying plantar plate tears, flexor tenosynovitis, and stress reactions of the sesamoids. In recent communications with John Cozarelli, DPM, he has suggested a histogram to assess degenerative changes in the tendon and capsule apparatus quantitatively. If your existing unit does not have the software capabilities to assess and measure the degree of heterogeneity within these structures there are overlay programs that exist to measure changes in serial ultrasound images.

The first principle of medical ethics is that of patient autonomy. Given truthful and accurate presentation of facts without bias or coercion; a patient has the absolute and ultimate right to self-determine what diagnostic and therapeutic efforts will be pursued. The risks of “no treatment” should be discussed (and documented as understood).
As a healthcare provider, you are not obligated to administer care which you do not feel is in the best interest or even harmful for a patient. You do have an obligation to assure proper transfer of such patients for care elsewhere. As for the “lawyers having a field day”, I am not a lawyer; however, I suspect the realities are not so simplistic. The days of paternalistic medicine mandating unquestioned care are no more.
You are likely best-served not treating patients with whom you are not comfortable. You indeed have that option. However, to suggest that all such patients should be summarily discharged for fear of litigation and liability may be overly simplistic. Decision-making is a mutual effort between patient and doctor.

Dr. Sachs, Please allow me to clear up the misinformation that you have posted on this forum regarding Florida credits during the pandemic. Two weeks ago, along with the FPMA counsel, I attended the FL Board of Podiatric Medicine meeting and on behalf of the FPMA, its members, and non-members practicing in the State of Florida, I requested that the board drop the 8 credit limit on "at home" credits and allow all the full 40 credits to be taken live, virtual or online for this biennium. We discussed all the reasons that under these COVID-19 conditions, many podiatric physicians are still uncomfortable attending live meetings.
The board agreed, and is in the process of approving our request for this biennium. The FPMA is always fighting for the protection of our members and our profession and as part of our mission, we will always provide quality and approved credits to our members for every renewal period. I would like to personally invite you to become a member and join our group of caring practitioners. I promise, that as a member, you will always receive accurate and timely information from the Florida Podiatric Medical Association.
