PODIATRIST IN THE NEWS |
IL Podiatrist Discusses Cause of Mummification of Feet
Details continue to emerge regarding the deaths of actor Gene Hackman, 95, and his wife, Betsy Arakawa, 65. According to an affidavit obtained by NBC News, Arakawa had "bloating in her face and mummification in both hands and feet. "Some conditions, such as dry gangrene, can cause an appearance similar to mummification, Melissa Lockwood, DPM, a podiatrist at Heartland Foot and Ankle, told Yahoo News. “It occurs when there is a significant reduction in blood supply to a body part,” Lockwood said. “We see it a lot in toes and feet because they are the farthest away from the heart, causing the tissue to die and dehydrate.”
|
Dr. Melissa Lockwood |
Dry gangrene often happens in people with uncontrolled diabetes or severe peripheral artery disease, a condition in which the arteries that carry blood to the arms and legs narrow or are blocked, Dr. Lockwood said. "When blood flow to an area is severely compromised, the tissue dies but remains relatively dry, leading to the appearance of mummification," she added. "In a living person, this can sometimes be incredibly painful and may necessitate surgical intervention such as amputation to prevent complications."

REESPONSES./COMMENTS (CODING & BILLING) |
From: Donald R Blum, DPM, JD
I think the issue might be that CPT S0395 is only for the casting of the foot, and that impression will then be sent to the actual manufacturer, who then builds the orthotics. I believe that when the physician then bills the L3000 code, the podiatrist is substituting themself for the manufacturer. In the case of CPMA and CPT S0395, Anthem might have agreed to pay for that service but not when the provider is charging the CPT L3000 in addition.
I think this would be similar to billing for a regional anesthetic (block) and then billing for a surgical procedure (not with a steroid). The local anesthetic is included in the surgical fee. Just because there is a CPT code does not mean that it will be payable.
Donald R Blum, DPM, JD, Dallas, TX

RESPONSES/COMMENTS (CLINICAL) - PART 1A |
From: PM News Subscriber, David P. Luongo, DPM
This is a picture of my patient with T-cell lymphoma. A biopsy of your patient may be a good idea.

Plaque-Like Calluses on Weight-Bearing Surfaces |

I've seen this before. For my patient, it was the onset of menopause where there was a sudden change in hormones. Have that checked out.

RESPONSES/COMMENTS (CLINICAL) - PART 1B |
From: Gary S Smit, DPM, Elliot Udell, DPM
I have seen this as a side-effect of medications. This is anecdotal, but I've seen it with anxiety medications like Zoloft and I saw it once as a side-effect from an artificial insulin product. This is very frustrating to treat. I think that oral antifungals is a good idea.
Gary S Smit, DPM, Kme, PA
I recommend doing the biopsy because if it turns out to be psoriasis or some other autoimmune disease, there are great new drugs available that can address these issues.
One patient of ours had psoriatic plaques all over his body. He went from dermatologist to dermatologist for years and still suffered. Finally, he was placed on Skyrizi and within a short period of time, he had resolution of all of his plaques and feels like a new person. Please let all of us know what the final diagnosis turns out to be.
Elliot Udell, DPM, Hicksville, NY

RESPONSES/COMMENTS (PM JURY VERDICT REPORTER) |
From: Joseph Borreggine, DPM
It’s quite unfortunate that the limits of liability no longer cover the cost of a malpractice case and it is highly important that podiatrists who perform surgery must consider protecting their assets...or your financial future will be in grave danger. The days are long gone now that your malpractice will be covering all monetary liabilities of the claim filed and possibly charged against you. The insurance companies probably will not be raising limits of liability any time in the near future since they’re in the business of making money.
The $1 million/$3 million claims made policies with limits of liability are from days long ago where we never thought anybody would sue for $1 million, but if you look at this article, you will see how much higher judgments and settlements are as of 2024 data. It’s all fine and good that everybody wants to be a board certified foot and surgeon in the podiatric field. However, everyone who is doing any foot or ankle surgery in this profession should think twice about...

RESPONSES/COMMENTS (PMNEWS QUICK POLLS) |
From: Rod Tomczak, DPM, MD, EdD
The results of the recently completed survey of students committed to a DPM degree or who already have a DPM degree and limited license piqued 689 professionals enough to respond to four questions. The most surprising outcome to me was the overall tenor of our colleagues who responded. Over at least the last 20 years, there has been a clamoring of DPMs for a degree change so that our profession could be granted an unrestricted license.
I was certain the pathway described leading to an unrestricted license and parity with the osteopathic and allopathic communities based on passing USMLE would generate thousands of participants. With the increase in DO granting institutions and the decrease in students applying to our DPM colleges, it appears we are quickly approaching an unbelievably important...

RE: Outrageous Foot Injury of the Day
A woman went to a St. Petersburg emergency room after a key got stuck in the bottom of her foot. Madison Martin said she was walking to the bathroom when she stepped on jeans that were on the floor.
|
Key impales woman's foot. |
Inside one of the pockets was a key; she said she ‘heard a crunch.’ Martin said the injured area was injected with a numbing substance and the medics "yanked it right out."
