AT THE COLLEGES
OH Podiatrist Appointed Emeritus Professor at KSUCPM

After nearly 32 years of service, it was difficult to imagine the Kent State University College of Podiatric Medicine (KSUCPM) without Vincent J. Hetherington, DPM, MS following his retirement in September 2020. However, that wasn’t a reality that the community had to imagine for long. Effective January 2021, Dr. Hetherington has assumed a new title of Emeritus Faculty that will allow KSUCPM to benefit from his continuing identification with the College he has served so long.

Dr. Vincent Hetherington

In his new role, Dr. Hetherington looks forward to continuing his support of CPM students, academic programs, alumni outreach initiatives, student research project development, as well as bridging international opportunities. Emeritus status rightfully acknowledges Dr. Hetherington’s lasting contributions and legacy as a valued faculty member.

PODIATRIC PRODUCTS AND SERVICES IN THE NEWS
Tayco External Brace Provides Improved Patient Compliance: IN Podiatrist

Patrick DeHeer, DPM, board-certified podiatrist, pediatric specialist, and inventor of the Equinus Brace, uses the TayCo External Ankle Brace for many ankle and hindfoot ailments. This device is versatile because it can immobilize the ankle, then convert to free motion, allowing for dorsiflexion and plantar flexion. “The TayCo Brace has become a very intricate part of my practice because traditional bracing that goes inside of a shoe is fraught with compliance issues. Patient compliance is much better than traditional AFOs,” said Dr. DeHeer.

Dr. Patrick DeHeer

The TayCo Brace fits outside the patient’s shoe and can be combined with an orthotic. “You get the best of both worlds. The patient gets support and stability from the brace, plus biomechanical and gait correction from the orthotics.” Dr. DeHeer incorporates the TayCo Brace into treatment of posterior tibial tendon dysfunction (PTTD) plus many other ankle and hindfoot maladies.

PRACTICE MANAGEMENT TIP OF THE DAY
2021 Remote Patient Monitoring Changes:  Key Takeaways - Part 8

In December, the Centers for Medicare & Medicaid Services (CMS) released its 2021 Medicare physician fee schedule final rule. Within the final rule were some significant changes concerning remote patient/physiological monitoring (RPM) and the provision of other remote services. Here are the most significant takeaways from the final rule.

8. Re-establishing 16 Measurement-days to Bill

CMS confirmed that it will maintain the existing requirement that 16 days of data for each 30- day period must be collected and transmitted to bill CPT 99453 and 99454. The 16-day requirement was confirmed despite an industry push to lower the figure, with some hoping it would drop to as few as six days.

CODINGLINE CORNER
Query: Referring Versus Ordering Physician

When billing Medicare for in-office services (dispensing durable medical equipment or performing x-rays), are we considered the referring physician or are we the ordering physician? This is in reference to the billing claim form, box 17.

Thomas Rehm, DPM, Asheville, NC

Response: The best discussion of the differences between these two types of providers comes from the National Uniform Claims Committee which provides a simple explanation:

Referring Provider - The referring provider is the individual who directed the patient for care to the provider rendering the services being reported. Examples include, but are not limited to, primary care provider referring to a specialist; orthodontist referring to an oral and maxillofacial surgeon; physician referring to a physical therapist; provider referring to a home health agency.

Ordering Provider - The ordering provider is the individual who requested the services or items being reported on this service line. Examples include, but are not limited to, provider ordering diagnostic tests and medical equipment or supplies.

To make it even simpler: In most cases, if the patient needs (must have) a prescription to have the service provided, you are the ordering provider. Yes, physical therapy is an exception. My advice: You are the ordering provider in the majority of cases.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: CPME 320 and Wound Care Rotations
From: Bryan J. Roth, DPM

The Council on Podiatric Medical Education (CPME) 320 Document, Standards and Requirements for Approval of Podiatric Medicine and Surgery Residencies, is currently under revision. There has been much discussion within our profession regarding the direction of the 320 Document, which impacts residency training and thus, the future of our profession. What exactly does our profession desire? Parity and privileging are frequently discussed in podiatry; however, instead of discussion, I believe one must practice it and not simply discuss it.

In my opinion, the 320 document affects everyone in our profession regardless of their involvement in residency education. The document guides the future of...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
From: Matthew B. Richins, DPM, Steven E Tager, DPM

Cast prone. I do not know why we were taught to do it with the patient sitting supine in a chair; perhaps a colleague can tell me. However, I doubt it will change my mind as I get superior results when the patients are flat on their bellies.

Matthew B. Richins, DPM, Joplin, MO

What I consider pearls are more likely than not a way of practice life. Somewhere along the road a bright light came on and a few things became more apparent than ever. More “pearls” if you will:

• Rearfoot to lower leg alignment is an absolute must if FF control is to be established. Hence the need for a RF post most of the time.
• With the FF following the RF (for most of us bipeds), it’s the bandleader. The FF (the band) takes its cue and does whatever it's told to do. RF malfunction simply means FF malfunction i.e., functional adaptation. Neuromas, bunions, HTs TB, HR, HL. etc. results.
• Neutral position closed kinetic chain suspension impressions technique is an...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 3A
From: Robert Kornfeld, DPM

I wholeheartedly disagree that podiatry needs to remain a medical specialty with a plenary license and DPM designation. I have been a podiatrist for 40 years and have listened to this conversation ad infinitum. Unless and until we are MDs or DOs, we will never have parity. I think it's ludicrous to believe that given the choice, no one will opt to become podiatrists. That is simply an empty argument. Students will fill all specialties as per the available number of residencies over time.

I am at the tail end of my career and it will never matter to me one way or the other, but my opinion is that podiatry as a profession will die if it is not absorbed into allopathic or osteopathic medical school training programs.

Robert Kornfeld, DPM, NY, NY

RESPONSES/COMMENTS (NON-CLINICAL) - PART 3B
From: Matthew J, Connolly, DPM

As an older practitioner, I can tell you that the profession has indeed made strides toward equality. However, I haven't seen my situation addressed, and in that spirit I'll put in my two cents. I'm an employed podiatrist in a large orthopaedic group. The group has allowed me to practice my specialty -- they have been very specific in empowering me to practice as I wish (even when orthopaedic and podiatric training differs) if I can back up my actions with good researched data. It's a good group. The problem comes with our after-hours clinic, our walk-in availability, our presence on sports event sidelines, and more. In each case, my limited license does not allow me to be a representative of our practice. Our nurse practitioners and physicians assistants can perform each of those duties.

I respect the training and abilities of our NPs and our PAs. My training is certainly good enough to match and outpace theirs. If our orthopaedists are presented with an eye injury at a high school football game, I...
