Regular stretching can help resolve toe cramps, as well as physical therapy and Epsom salt soaking, says Bruce Pinker, DPM, board-certified podiatrist and foot and ankle surgeon at Progressive Foot Care NY.

It is also important to gradually adjust to new footwear and allow for a “break-in” period as some new shoes/sneakers can be stiff or rigid, “After being worn for a few hours, shoes can typically become more comfortable, and this can reduce toe cramping,” says Dr. Pinker. He adds that if toe cramps don’t resolve within three days, it is best to seek the advice of a physician.

An issue that arises is that of illegal kickbacks and fee-splitting legal rules. When concierge practices offer free services, this raises concerns under the federal antikickback statute, if Medicare is involved, or state antikickback laws. Enforcement authorities can view the free service as an illegal inducement for clinical services.

In my 42 years of practice, I saw patients daily whose neuropathic symptoms were spinal in origin (stenosis, L 4-5, S-1 disc)… it can oftentimes present initially as same, without the patient having current spinal symptoms. They also may have a history of same years ago.

Small fiber neuropathy (SFN) and large fiber neuropathy (LFN) represent two distinct categories of peripheral neuropathy, differentiated by the size of the nerve fibers they affect and the resultant symptoms and diagnostic approaches. SFN targets small myelinated Aδ fibers and unmyelinated C fibers, leading to symptoms like burning, tingling, and is diagnosed through hstory/clinical evaluation. LFN affects large myelinated fibers responsible for proprioception and vibration sensation, and with diagnosis typically achieved via physical exam and confirmed with nerve conduction studies and electromyography (EMG). The patient mentioned seems to have SFN. The causes of small fiber neuropathy (SFN) include:
 
Diabetes, autoimmune diseases (e.g., Sjögren’s syndrome, lupus, rheumatoid arthritis, celiac disease), infections (e.g., Lyme disease, HIV, hepatitis C), vitamin deficiencies (particularly vitamins B12, B6, and E), alcoholism, toxic exposures (e.g., chemotherapy drugs, heavy metals, industrial chemicals), hereditary disorders (e.g., hereditary sensory and autonomic neuropathies, Fabry disease), idiopathic (no identifiable cause), and finally metabolic disorders (e.g., thyroid dysfunction, amyloidosis)

In response to patients who had similar problems with GFS inserts, I would write on an Rx pad  "The devices (I never called them anything else) were not appropriate for the person's foot/ankle structure or their diagnosis. Please promptly provide a full refund. It always worked. Patients loved it, and they had me mold true Rx orthotics. Good for them and good for my practice.

My esteemed colleague asks how does Good Feet get away with charging $2,000 for an over-the-counter insert and some top covers. The answer is that they advertise and have salespeople work for them. Also, in general, the devices they sell work quite well. I am a big believer of supporting the arch maximally with an orthotic and the ones they sell do that. My patients LOVE the orthotics I make them and as I have said before are willing to pay a reasonable price for them... $850 for the first pair. I give a $200 discount on second pairs. I have a unique way I present orthotics to patients with which I have a 25% acceptance rate because they “get it”.

Why has all of our expertise and marketing for custom orthotics come to this? I have a Good Feet Store around the corner from my office, and I have heard the stories, seen the products sold to them, and it's sad. Some conclusions I have drawn rudimentarily from this are:
 
1. Podiatry needs to get aggressive with these companies and/or the public and declare the truth about these exorbitant (i.e. rip-off) OTC orthotics. This requires courage--and no one knows if it will work, but doing nothing is not an option.
2. We need to analyze why Americans will plop down $1,500 or $2,000 in a retail setting but will resist spending $100 for a pre-form that is great for them, their activities, and their footwear. I have four different styles of OTC insoles in my office. Although patients can get most of them dirt cheap from Amazon, they come to me (or should come to me) because I have a good idea which type they will be happy with, considering what they do and what they wear.
3. They overspend in the retail outlets because they are so well trained to do so. Basically, we believe we are going to get what we pay for. This does not happen, but 'buyer’s remorse' occurs much later in the process. Most importantly, people are not as comfortable accessing the healthcare system as they are 'going shopping'.
 
This can't be reversed, but it can be modified. It is time to do so.

I was surprised and dismayed to see the average net profits of DPMs this past year. Basically $103,000 for solo practitioners and $149,000 for partnership practitioners. I decided to search the Bureau of Labor Statistics website and saw that for 2022, the average podiatrist reported a salary/profit of $148,000, so I guess the PM Annual Survey is reasonably accurate. Using the same BLS website, RNs made an average of $81,000, pharmacists $132,000, cardiologists $339,000, family practice physicians $230,000, and orthopedic surgeons $526,000.  
 
Is it any wonder that we have a difficult time getting enough qualified students to apply to our schools? Knowing that statistics can be analyzed from different points of view, I find it hard to believe that DPM salaries are, in reality, that low. However, if these numbers really are true, then we as a profession need to take a long hard look at how we are functioning within the healthcare arena. I have opined in this forum in the past that we need to change our education/training model so that our graduates can become full-fledged physicians, and thus function completely as physician specialists, and not merely technicians. If these numbers don't open our eyes, I don't know what will.

As a classmate of Vince, 1981, PCPM, I want to add my condolences to Vince’s family and many friends. Vince always was upbeat as a student and I never saw him without a smile in the classroom and in the labs at our college of podiatric medicine. I remember him after his residency, seeing him at CME meetings and reunions, and he had not changed at all as a person or as a doctor!
 
I followed his career as he advanced as an educator. I am sure his many students are better as people and doctors after being educated by Vince. Sad time for our class and the DPM community.
