MS Podiatrists Want ‘Ankle Privileges.’ Other Doctors May Stand in Their Way
Mississippi podiatrists are fighting for a new law to expand their scope of practice to include the ankle. They say the proposal would allow them to offer more comprehensive care for diabetes complications, preventing amputations, and to treat conditions such as Achilles tendonitis and ankle fractures, expanding patients’ access to care. The ankle itself is comprised of one foot bone – the talus – and two leg bones, the tibia and the fibula. And Mississippi is now just one of two states where podiatrists lack “ankle privileges,” as the American Podiatric Medical Association says.

“That’s the whole thing—they think you’re a foot specialist so you’re just supposed to stop at the talus?” said Charles Caplis, DPM, the owner of Foot Specialists of South Mississippi and vice president of the Mississippi Podiatric Medical Association, who came to the Capitol Tuesday morning to talk with lawmakers about the proposal. But they are facing opposition from the Mississippi Orthopaedic Society, whose members currently have an effective monopoly on ankle surgeries in the state.

Dr. Downey has spent nearly 25 years volunteering within the College. Dr. Downey’s service included two terms, 17 years apart, on the Annual Scientific Conference Committee, the chair of the Education and Scientific Affairs Committee, a member of the former CPG Management Council. He also currently serves as a reviewer of the Journal of Foot and Ankle Surgery, ACFAS’ peer-reviewed scientific journal.

FL Podiatrist Discusses Foot Health on the Farm
“There are a lot of common complications or foot problems we see, especially within the farming and agriculture industry,” says Tony Merendino, DPM, clinical assistant professor at the University of Florida Department of Orthopedic Surgery and Sports Medicine. “Fungal infections are common issues we see along with bacterial infections, bunions, and hammertoe deformities.” The most crucial fungal prevention step is keeping the feet dry and keeping socks and shoes clean and dry. Socks that wick away moisture to keep it away from the skin are helpful.

Although cotton socks are commonly worn, they are heavy, unbreathable, and uncomfortable when wet. Wool socks may be the best option when feet are likely to become wet during work activity. “Everybody wants cotton socks, but when you sweat, cotton will hold the moisture,” Dr. Merendino says. “We want material that the fluid moves through and evaporates away instead of staying next to your skin. If it doesn’t evaporate, it can cause fungal infection, leading to bacterial infection and many other problems. Wool socks absorb twice as much moisture and wick it away from your foot.”

Query: How to Get Medicare to Reject Bill for Orthotics?
I am having a coding problem. I have a patient with Medicare and Tricare whom I made orthotics for. Tricare will pay but needs a rejection from Medicare first. I billed Medicare trying to get them to reject the orthotic code but they just ignore it. I tried using the -GY modifier and had the patient sign an ABN. What is the correct way to bill Medicare so they reject it in a way that would allow Tricare to pay?

I've been following the stream of posts that was originated by Dr. Jacobs regarding PSSD and the diagnosis of peripheral neuropathy including the related double crush syndrome.
This device has been around for many years, and the basic principles behind the current application are still the same. Dr. Jacobs' original comments were well stated indicating the controversy that continues with the treatment and that it has not been accepted into mainstream medicine.
That said, Dr. Roth has interesting comments and there's no reason why a practicing physician cannot apply new techniques to their patients as long as...
