AAWP Holds Strategic Planning Meeting
 
The American Association of Women Podiatrists (AAWP) gathered in New Jersey for a strategic planning meeting from January 10-12, 2025, where exciting plans were unveiled for the year ahead. The organization is thrilled to announce a rebranding effort and is focused on expanding its membership while offering more educational and in-person opportunities for members in the coming months.

NY Podiatrist Discusses the Right Way to Cut Your Toenails
 
According to Ammar Al Rubaiay, DPM, "the way you trim your toenails can be a potential cause of your ingrown nails. Specifically, trimming your toenails too short or rounding the edges can increase your risk of an ingrown nail. So, when cutting your toenails, you don’t want to cut them on a curve or cut the corners. Instead, cut them straight across the top, leaving about a millimeter or two of nail at the top. This should be long enough that the corners of each toenail sit loosely against the skin at the sides. And always make sure you’re using a sharp, clean nail trimmer when it’s time to cut your nails."

I"n addition, make sure you’re addressing other possible causes of ingrown toenails. Wear shoes that fit properly and avoid high heels or pointy-toed shoes, which squeeze your toes together and can increase your chances of ingrown toenails. Look for breathable footwear and use antiperspirant or foot powder if you have extra sweaty feet. All that moisture softens the skin of your toes, which can make it easier for the nail to cause a small cut. And don’t pick or tear your toenails, which also increases your odds of developing an ingrown nail," says Dr. Al Rubaiay.

INTERNATIONAL PODIATRISTS OVERCOMING ADVERSITY
UK Podiatrist with Rare Brain Condition Completes 268-Mile Marathon
 
A brave podiatrist couldn't walk in a straight line after being diagnosed with a rare brain condition just 18 months ago… now she has completed a 268-mile ultramarathon in less than a week. Inspirational Mel Sykes, 42, was diagnosed with a Chiari malformation after losing her balance and slurring her speech in the summer of 2023. The condition sees the lower part of the brain push down into the spinal canal and brain stem, often causing double vision and balance problems.

Her consultant neurosurgeon at Leeds Teaching Hospitals, said, “Mel is a really inspirational patient and has shown huge determination to recover and return to ultrarunning. She had severe symptoms when she came to us and needed surgery urgently. It’s fantastic to see how well she recovered – completing this race is a truly remarkable achievement.”

lerEXPO Hosts inaugural APMA Surgical Complications Virtual Seminar 
 
The inaugural APMA Surgical Complications Virtual Seminar hosted on the lerEXPO platform took place this past weekend and was a resounding success rating 4.92 out of 5.0 stars on the value of the education with 75% of the surveys returned. This initial collaboration between APMA and lerEXPO brought together 16 experts from around the country to present on surgical complications related to forefoot, rearfoot, trauma, and diabetes.

Marie Keplinger, Raquel Sugino, Zeeshan Husain, Tom Chang, Mark Prissel, Jeff McAlister, Christy King, Karl Dunn, Bryon McKenna, Randy Clements, Kelsey Millonig, Haywan Chiu, John Steinberg, and Jake Wynes. Surgical complications are inevitable, and each of the four sessions finished up with a roundtable Q&A that stimulated dozens of questions from the audience.

RESPONSES/COMMENTS (INSURANCE ISSUES)
RE: "Covered California Obamacare"

"Covered California" is the State of California's "Obamacare" exchange for individuals to purchase government subsidized health insurance. Television commercials are running almost incessantly espousing and touting plans with as low as $10 monthly premiums and guaranteed coverage for pre-existing conditions to entice the uninsured public to purchase taxpayer subsidized health insurance. Shame on these same people who by design omit in the advertising that these low-cost plans have huge deductibles, huge office visit co-pays (sometimes as high as $105 per visit), and if a bronze plan is purchased, only a 60% insurance coverage after the huge deductible is met. 
 
The government advertises these "low-cost" plans, but it continues to put the doctor at an increased risk of not being able to collect full payment for services. It also harms those patients who need medical care and think they have insurance coverage to help pay the cost of the care, but then learn they have very poor coverage and will be left with high out-of-pocket costs that some simply can't afford and will never pay.

I think it’s time for a recalibration of the professionals we are graduating. We are wrong to label ourselves as a surgical profession. We are general practitioners who can choose any or all of the medical arts disciplines as they relate to the foot.
 
In my 50 years of practice, I prided myself in being able to perform dermatological, medical, orthopedic, and surgical treatment to my patients... solely dependent on their need. I tried to instill that in my residents and the preceptors who observed our offices.
 
We need to wake up and rethink our current education. Not everything can be solved with the knife.

Dr. Kass makes some excellent points about the medicine part of podiatric medicine. It's too bad that CPME, APMA, and the colleges don't bring podiatric medical education into this century. The curriculum must be expanded to include more medicine. The Illinois Association of Podiatric Physicians and Surgeons (ILAPPS) (formally IL Podiatric Medical Association) failed to convince Scholl College to switch from using candles to using light bulbs.
 
As someone who joined 209 out of 218 students at the Illinois College of Podiatric Medicine in the early 1970s who went on strike for two months against the college, I can tell you, as it was true back then, it is true now: Students are being cheated.

Dr. Chaskin brings up an interesting point about the unfairness of the New York podiatry scope. I find it absurd that in order for a podiatrist in New York to treat skin above the ankle, the doctor must be board certified in forefoot and rearfoot surgery. What other medical profession has a board certification requirement by law to treat patients? This law is so absurd. I have younger colleagues who finished three-year surgical residencies and are adept at treating ankle conditions and performing ankle surgery, yet are forced to take their patients to New Jersey to treat their patients because the process in New York takes time to complete. 
 
I agree with Dr. Chaskin that there are numerous lawsuits that can be brought to change the current status quo. Aside from what I mentioned above, there is the fact that “interstate commerce” is being affected. If a doctor is competent to perform an ankle surgery in New Jersey, then they are competent to perform it in New York. Every day I read how various professions are increasing their scope of practice....nurses, NPs, PAs, etc. I wonder when podiatry is going to jump on the bandwagon.

I stopped using phenol years ago when it became a hazardous chemical to ship...it was still relatively inexpensive to purchase, but it was three times the price to ship it! I switched to 10% NaOH and have never looked back. It works every bit as well.
 
A local compounder charges us about $30 for a 30cc bottle which lasts hundreds of procedures.
