MI Podiatrist Discusses Congenital Clubfoot
 
According to Conway McLean, DPM, "Congenital clubfoot is a relatively common condition, occurring in one out of a thousand births. It can be associated with certain neurological diseases but generally is found as an isolated abnormality. Many of those born with a clubfoot deformity have had a conservative approach to care involving manipulation and serial casting known as the Ponseti method. This has become the treatment of choice, with surgery utilized for only the most severe cases."

"When fashioned correctly,  orthotics can improve biomechanical function and the complex second-to-second changes that are part of healthy gait. Those with a clubfoot usually benefit greatly from a custom, in-shoe support that decreases stress to those tissues producing pain. Orthotics require only that you put your shoes on. If you or a family member is suffering from the residual complications of a pediatric clubfoot deformity, don’t assume there is nothing that can be done. Life is better when you can do more, especially without pain!" said Dr. McLean.

DEA Proposes Rule For Telehealth Controlled Substance Prescriptions
 
The Drug Enforcement Administration released a long-awaited regulation on Wednesday that would create a process for prescribing controlled substances via telehealth, just days before President Joe Biden’s term ends. The proposed rule would create special registrations authorizing providers and telehealth companies to prescribe Schedule II to V drugs, like Adderall or Xanax, without first conducting an in-person evaluation. The agency also finalized a regulation that allows registered practitioners to virtually dispense buprenorphine, a medication for opioid use disorder.
 
Although telehealth proponents have urged regulators to enshrine pandemic-era virtual prescribing flexibilities, some industry groups said the proposed rule included restrictions that were too onerous for providers. “We implore President Trump to make it his urgent priority to withdraw this proposal immediately following his inauguration on January 20,” Kyle Zebley, senior vice president of public policy at the American Telemedicine Association, said in a statement.

"If your practice has 10 medical assistants but only two providers, you can compare that to similar clinics to see how far you vary from [the] standards. If a clinic is too highly staffed compared to their peers, it's a problem area and warrants further investigation. But if you are too efficient and have only two medical assistants [and are] understaffed compared to your peers, you need to hire another staff person. OBITUARIES
RE: The Passing of Richard Lee Skudlarick, DPM
 
It is with sorrow that we announce the passing of Dr. Richard Lee Skudlarick, who passed away peacefully on December 23 at the age of 84. He attended Officer Candidate School at Camp Pendleton. He served as a Second Lieutenant in Da Nang during the Vietnam War.

Dr. Richard Skudlarick practiced in Bellingham, Washington, until retirement in 2016. As a doctor, Dick was known for his diligence, compassion, and incredible sense of humor.

RE: Why We Need 3-Year Residencies

The reason is, quite frankly, simple. Money. Money, and Medicare. In all U.S. jurisdictions (except Maryland which has a Medicare exemption), residencies receive most of their financial support through Medicare. Medicare rules state that, essentially, they will provide full funding for the MINIMUM number of residency years required for board certification. If only 2 years of residency are required for certification, then Medicare would only fully fund 2 years of residency. This creates a financial burden on all 3-year programs. 
 
If you peruse the AACPM list of podiatry residency programs, you will see only about 10 do not have the Rearfoot/Reconstructive Ankle [RRA] suffix. If there was a single, initial certifying board, I believe that that ratio would eventually flip. Those who wish additional designation will be free to earn that. Also, the curriculum at podiatry colleges now has as their goal to prepare their students for residency, not practice. Who wants to go through residency to learn chip & clips?

How sad but true. Many of the matriculating students just do not have the hands or eye coordination to be a surgeon and thus this training is to some extent a waste. This also holds true of probably the lower one quarter of the graduating students at the podiatry schools that they are either not bright enough or motivated to be decent practitioners. 
 
I do not know the answer, but the low number of applicants seems to guarantee that less than adequate students are being pushed through the system and thus are doomed to failure or not providing the public a competent practitioner. We should try and get ALL medical school applicants to also apply to, or be made aware of, podiatry medical education. This could be done by the APMA putting up a website specifically directed to the DO and MD applicants.

Kudos to Dr. Neuhoff’s post. When I had the opportunity to address the APMA House of Delegates as president of ABPS, I argued against the homologation of our residency training model. We are not a surgical specialty…we are a diverse profession which has foot and ankle surgery as one of our sub-specialties. When I was done with my presentation, I was approached privately by, if I remember correctly, the executive director at that time. He confided that I presented a strong argument but the APMA had to protect the "grassroots" podiatrists, and the profession believed that all podiatrists, regardless of training, had a right to do foot surgery. I remember asking him if he had a daughter, and if he did, would he rather a doctor from my training model or his do her surgery. He leaned in closely and said, “well, yours of course, but I can’t tell anyone that.”
 
So there are 2 issues. The first is that, as Dr. Neuhoff said, that not all of our graduates either...

I echo Mark Smith’s phenol concern. About 3 years ago, one local compounding pharmacy said that in order for them to prepare a bottle of phenol, they would need the names of patients I have used it with or plan to use it with. I declined. I now purchase it from a national supplier. A pint bottle that used to cost a few dollars is now $125.
 
We have the same problem with monocloracetic acid. Local compounding pharmacies now want $150 for a few ounces.

I am heartbroken to hear of the passing of Ed "Eddie" Fazekas. As a longtime delegate to the APMA house of delegates, and as a representative of the FPMA, we became 20+year fast friends. Eddie would always greet me with, "there's the King" and we would laugh, pat each other, and share stories of our families. Ed was a true gentleman, kind soul, and just an all-around great dude. I will miss his smile, his greetings, and simply the person he was. Rest well my friend. This profession has lost a good one.
