Did you know Efren Buff De La Rosa, DPM was a kickboxer in the 1980s? After training for the sport, he actually participated in a real bout and knocked his opponent out. Dr. De La Rosa graduated from the California College of Podiatric Medicine in San Francisco and served a one-year medical residency at Los Angeles County Hospital in Los Angeles and a second-year surgical residency at Delano Regional Medical Center in Delano, CA.

Incidence of VTE Following Hospitalization for COVID-19
Increasingly, we are evaluating patients returning to our offices with a history of prior hospitalization for COVID-19. This article highlights the need for our vigilance in considering post-discharge DVT in such patients. In this retrospective study, the authors report on rates of venous thromboembolism (VTE) and bleeding in hospitalized COVID-19 patients. Of 303 patients, 78% were on wards, and 22% required a high level of care (10% received invasive ventilation). Standard thromboprophylaxis or therapeutic anticoagulation was given in 97% of patients. Image-confirmed, symptomatic VTE occurred in 5.9% of patients during hospitalization and in 7.2% of patients at 90-day follow-up. Bleeding occurred in 4.3% of patients, with 1.3% experiencing major bleeding.
Among COVID-19 patients admitted to the hospital and receiving thromboprophylaxis or therapeutic anticoagulation, there were significant rates of symptomatic VTE and bleeding.

I appreciate the counterpoints of members of the community; none of them are news to anyone. The information I relate is the very information issued to patients at the time of their vaccinations in response to questions posed by them and addressed by attending staffs.
Is there evidence that a vaccine is effective for the new mutations? If the learned colleague can provide a citation I will gladly ingest this accordingly. Masks are a moot point with the majority of the public NOT wearing N95 quality devices, and if they do, where is the training provided to ensure the devices are correctly fitted? The virus itself is now known to be airborne and not confined to droplet transmission, which begs the question of just how useful the arbitrary six-foot social distancing is.
As important as it might be not to succumb to 'conspiracy' theory, it is equally important to maintain a questioning frame of mind, and not simply to repeat verbatim CNN news bulletins.

Dr. Levy makes a good point that DPMs are well trained to give IM injections of a vaccine and if there is a manpower shortage, we as a profession could solve the problem. Let’s be honest and take this one step further. A monkey could be taught to give an IM injection. The injections that podiatrists give all day long require knowledge and training. Hence, if the powers that be at the various states get their acts together, we could help solve this very serious problem and really make a difference for our society.

Dr. Levy suggests that podiatrists are adequately trained and equipped to administer the COVID-19 vaccination. He further suggests that the reluctance to permit the administration of such vaccinations is political, founded in a lack of understanding of the podiatric medical education.
Dr. Levy posits that a simple injection of epinephrine is all that is required for major adverse reaction to the vaccine; anaphylaxis is believed to occur in 1 per 100,000 doses. However, the occurrence of symptoms such as tachycardia, hypotension, cardiovascular collapse, laryngeal edema, nausea, vomiting requires more than epinephrine. Supportive equipment and care must be readily and immediately available.
I believe that he is wrong. A podiatrist is no more capable of screening for or the handling of emergencies than an orthopedic surgeon, urologist, dermatologist, or any office-based community healthcare provider. Clint Eastwood as Dirty Harry famously said, "a man needs to know his limitations." This is a matter of public safety, not a political issue. I suggest those who are considering administering the vaccination read this.

A year ago, we dealt with the uncertainty of this virus. Some thought it would be no different than the common flu, others considered it more potentially problematic. Now a year later, over 400,000 lives have been lost in the U.S. as a result of this virus. In my area of the country-LA- 95% of our inpatients are covid positive. Every day, I hear about a death from COVID. Our ICUs are flooded. Three or four patients seem to code very day.
The development of this mRNA vaccine is nothing short of a miracle. Many hospitals are making it mandatory for new employees. The PM News survey is very discouraging to me. I could think of no reason not to be...

RE: FPMA 2021 SAM Meeting

I had the pleasure to both attend and present at the Florida Podiatric Medical Association, Science and Management Symposium 2021, at Disney’s Coronado Springs Resort this past week. The theme for 2021 was “The Challenge of Change” and this challenge was met with an incredible hybrid conference with approximately 400 attendees present in Orlando, Florida and another 400 attendees online. I have been attending the SAM conference since 1990 and always look forward to this event and I am happy to report that the FPMA staff and Disney World did a fantastic job pulling off the impossible during a challenging COVID-19 pandemic.
I felt 100% comfortable attending this “live event” as the temperature screenings, hand sanitizing stations, strict adherence to mask wearing, room sanitizers, spaced out exhibit hall, and 6 to 10 foot social distancing were welcome safety measures. Hopefully, this is the first step towards regaining medical seminar normalcy and if any podiatric organization is considering hosting a live event, they should follow FPMA's lead in hosting an amazing educational event.
