MA Podiatrist Re-elected to State Podiatry Board
 
Dr. Raymond S. Murano, Chief of Podiatric Surgery for Hallmark Healthcare was recently re-appointed to the Commonwealth of Massachusetts Board of Registration for Podiatry by Governor Deval Patrick. He was originally appointed by Governor Cellucci in 1999, and then re-appointed by Governor Romney. He currently serves as the Board’s Chair.

VA Podiatrists Team Up with Salvation Army for Winter Shoe Drive 
 
As temperatures in central Virginia drop, the importance of having good shoes only increases. Local podiatrists in Charlottesville want to make sure everyone has a decent pair of shoes, so they're collecting gently used shoes for the rest of the month. For about 25 years, podiatrists in Albemarle County have partnered with the Salvation Army for the Fill Their Shoes Drive, giving away as many pairs of shoes as possible each winter. This year, they say the goal is about 10,000 pairs of shoes.

The shoes will be given away next month. "People need help more than ever with the changing shape of the economy every year. People need a hand these days - or a shoe!" said Dr. Bryan Snyder. Since this campaign began years ago, the groups have collected and donated nearly 100,000 pairs of shoes.

IEFS Presents Ilizarov Award at 9th Annual Meeting
 
Alexandru V. Georgescu, MD, PhD, was awarded the Ilizarov Award of Excellence at the 9th Annual International External Fixation Symposium (IEFS) sponsored by the University of Texas Health Science Center in December 2014 in San Antonio, Texas. Dr. Georgescu practices in Cluj-Napoca, Romania and is a Professor and Head of the Department of Plastic and Reconstructive Surgery at the Iuliu Hatieganu University of Medicine and Pharmacy in Cluj-Napoca, Romania.

Dr. Georgescu's many years of service with expertise in limb salvage surgery with plastic and reconstructive techniques, numerous publications and book chapters, research and teaching have distinguished him as honorary recipient of this year’s award. EHR Stage 3 Proposals Closer to Approval
 
Proposed rules for Stage 3 of the meaningful-use program, scheduled to begin in 2017, and rules concerning 2015-certified electronic health-record products, are a step closer to approval. HHS and the Office of the National Coordinator for Health Information Technology have submitted them to the Office of Information and Regulatory Affairs in the federal Office of Management and Budget.
 
The proposed rule for the third stage of the meaningful-use program, for example, says that there will be “changes to the reporting period, timelines, and structure of the program. These changes will provide a flexible, yet, clearer framework to ensure future sustainability of the EHR program and to reduce confusion stemming from multiple stage requirements.”

RE: Insurance Companies are Ruining Medicine

"More than 80 percent of families on the new health insurance exchanges have a deductible of at least $6,000, according to data from the U.S. Department of Health and Human Services. Practices, therefore, must ensure their readiness to collect the bulk of their revenue directly from patients through the first half of the coming year instead of from insurance companies."
 
This was a quote from an ACFAS bulletin. Most of us have routinely collected deductibles and copays as a standard business practice over the last two decades plus. It is obvious to me that what is happening is that, in the near future, private insurance coverages will become "catastrophic coverages only"  for everyone. You can bet your bottom dollar that the premiums will not go down either.
 
I was around in the early years of Medicare, and it was the worst insurance as far as allowed payments. Soon, it will be the highest payor and most reliable for payment. I am at the end of my run in a great profession, but I am happy that I am not facing the challenges ahead for all practitioners, not just podiatrists.

Why has there been no comment on having the "rules" reflect what is needed to treat these elderly patients with severe nail deformities? When the Medicare fees schedules were released, I compared myself to my colleagues and even checked out what my old professors' profiles were like. I suggest that you do the same. You will discover that even those who claim to be "surgeons" provide the (basic non-surgical) care that is needed to their patients. There is no data on how many patients are cash-paying because they don't "qualify." So one should not assume that these Medicare statistics represent a fact that a doctor is prancing in the gray zone. It also assumes that you always know the subjective information provided by that patient.  
 
Patients are not the best historians and their perspectives may be different. I have completed Data Trace's home study for the PICA discount for years. They frequently have cases where they give the patients' and doctors' perspectives on what transpired precipitating the lawsuit, and the accounts don't match up! Don't presume you know what subjective information transpired between a patient and doctor unless you were there. In 30 years, I have heard all sorts of perspectives about my local colleagues which I know do not represent them. Be mindful of accusations as there are laws about that too (slander and libel).

I am saddened by the passing of Dr. Gina Carlo. She was one of my first group of students at PCPM class of 1988. Gina was a top student and one of the nicest people you would ever want to meet. Her enthusiasm was infectious. She was also a student leader, but in a kind and gentle way. Dr. Carlo performed well at her residency at the then Suburban General Hospital in Norristown, PA. Being true to where she grew up, she returned to Carbondale Pennsylvania in the heart of Pennsylvania's anthracite coal region. Like many of upstate Pennsylvania's coal towns, Carbondale was in an area of depressed economics, yet Dr. Carlo served her community well, caring for its residents who remained the heart of the town.
 
Like all of us, Dr. Carlo faced the ups and downs of life, including her last battle with illness, but she never waivered in her devotion to her practice and her patients. Gina, you will be missed by all, especially by me, and you will live in my memory as one of the great students who came from the days of the PCPM. Rest in peace.
