“This project is very close to my heart as it ties in my profession and my Indian roots to create an immense impact in the daily lives of hundreds of people by preventing limb threatening foot infections,” said Women Who Win Co-founder Dr. Jhaveri, a podiatrist. “This project has also created a buzz in the podiatry community, particularly with Indian-American podiatrists as well as major footwear companies who have offered tremendous support for this initiative.”

7 Common Estate Planning Mistakes To Avoid
Hiring an estate planning attorney can be a pricey proposition, leading many to seek out online sources to create their own estate planning documents. Without the guidance of an attorney, though, many who go-it-alone make costly mistakes. Here are some estate planning mistakes to avoid:
1. Having an outdated estate plan and/or Will. - Life changes and your financial circumstances may change as a result. It is a good idea to review your estate plan at least once a year to keep it updated. Your estate plan should be modified accordingly if any of the following life changes occur.
-
Your parents have died and thus can no longer be beneficiaries.
-
Your children have gotten married and had children of their own.
-
You have divorced and remarried.
-
Your assets have grown (or diminished) significantly.
-
You no longer own a home, or you purchased additional property.

| RESPONSES/COMMENTS (CLINICAL) | |
From: Steven J Berlin, DPM
If the patient does not have active sickle cell disease, chances are she will not have vascular issues with any foot surgery. However, this data may be very scarce. If you have a major concern, you should consider the possibility of using EPIFLO as a preventive measure and to ensure more positive healing.
EPIFLO is a form of transdermal continuous oxygen therapy. It is the least expensive form of topical continuous oxygen but Medicare and the Blues do not cover this technology. Unfortunately, the insurance companies have not realized the potential savings with this device in prevention and wound healing.
Steven J Berlin, DPM, Medical Director, Ogenix (manufacturers of EPIFLO)

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 1 | |
From: Donald R Blum, DPM, JD
My dermatologist accepts Medicare assignment but does not accept other insurances. When you accept assignment, it is "ONLY" for services that you know are not excluded from coverage. For services that are excluded, you can accept payment at time of the service.
If you follow the CMS rules and guidelines strictly, I think you should do okay. All services provided to diabetic patients (whether on Medicare or not) are not necessarily a covered service if the patient does not meet the class findings (Q7, Q8, Q9).
One more item - remember for 2022, there may be a decrease in the Medicare reimbursements (due to fee reductions and sequestrations).
Donald R Blum, DPM, JD, Dallas, TX

|
| RESPONSES/COMMENTS (NON-CLINICAL) - PART 2 | |
From: Daniel Waldman, DPM
Over the past few years, I have also received more and more faxes from pharmacies (CVS accounts for probably over 90% of these faxes) stating that a patient expects to pick up the refill in a couple of days. When my staff calls the patient, they have no idea why this was sent as they have not requested the refill. I’ve called the pharmacists directly and they admit that the patient did not request a refill but the higher-ups at corporate management are instructing the refill request to be sent to physicians. When I asked the pharmacists why this is happening, they have flat out told me it’s all about corporate profits and they cannot do anything to change the system. The pharmacists seem genuinely interested in providing the best care but they are simply cogs in the machine of the corporations.
I wonder how many physicians simply sign off on these refill requests and patients are continuing to take medication that they no longer need. I have worked with a couple of law firms around the country about unsolicited/unauthorized faxes. A few years ago, I settled out of court with a company for a five-figure payout. This was not a pharmacy. Although class action lawsuits can be effective, they take many years and plaintiff awards are capped. The law firms make the majority of the payouts. Perhaps it’s time for congressional hearings?
Daniel Waldman, DPM, Asheville, NC

| RESPONSES/COMMENTS (SPECIAL COMMUNICATION) - PART 1A | |
From: Brian Kiel, DPM
Dr. Kornfeld, I think you are correct in that many podiatrists are "unhappy", but statistics show that a large percentage of physicians are equally unhappy and are quitting in record numbers. I do disagree with your inference that the public does not know who we are. I have been in practice for 48 years. Time and time again when people ask me what I do, I say "I am a podiatrist." They know exactly what I do.
In fact, years ago, I would say "foot doctor" and often they would say, "Oh, a podiatrist". Where we are going as a profession is questionable - too many foot and ankle surgeons and not enough podiatrists, but you can be sure the public knows who we are and appreciate our care.
Brian Kiel, DPM, Memphis, TN

| RESPONSES/COMMENTS (SPECIAL COMMUNICATION) - PART 1B | |
From: Steven Selby Blanken, DPM
I am one of many DPMs who are happy with the fact that the NBME declined our request (the "White Paper"). I have stated before, as I will state today, that in this day and age, transparency needs to be in the forefront of proposals like this. Many of us who are even very active in our state societies and APMA didn't even know about this when it was being thought of. I believe that some few DPMs in our profession intended that to be that way. Issues like this in the future should be announced nationally by APMA, be forwarded to all state societies and evaluated, discussed by its members, and actually be voted on by each and every individual state society.
This, in turn, would be formally submitted to all the delegates and leaders of APMA prior to any discussions outside of our profession. I hope we can learn from this. The efforts that started in California actually had a negative effect in other states dealing with physician bills. This, in turn, will hurt several state societies from achieving resolution from several items that they have fought for years. I will also commend the DPMs that want "equality" but we need to do this in the correct format that is best for most of us in our profession. If polled, I feel most podiatrists feel that they have enough parity and respect overall from most of the healthcare professionals.
Steven Selby Blanken, DPM, Silver Spring, MD
Editor's note: In the next PM News, we will poll our readers on the issue of how podiatrists feel about parity.

RE: The Passing of Dennis Norman Brown
Dennis Brown, founder of Northwest Podiatric Laboratory and Superfeet Worldwide, died on Dec. 31 at his home in Blaine, Washington, Superfeet confirmed. He was 96 years old. A WWII veteran in the British Army, Brown entered the world of podiatry when he started Northwest Podiatric Laboratory in 1964, where he led the development of industry-leading patented orthotic products (he had 61 patents to his name) and casting techniques.

Recognizing the cost of podiatric orthotics were too expensive for most athletes, Brown and his partner Christopher Smith, DPM, formed the Superfeet company to provide more affordable after-market insoles to the public.
