TX Podiatrist Discusses Importance of Buying the Right Athletic Shoe
Ronald Lepow, DPM, assistant professor of orthopedic surgery at Baylor College of Medicine, specializes in podiatry and helping patients fix their feet. He’s also an avid tennis player and has trained for marathons in his free time. The most important purchase for any runner is the right athletic shoe, he said. “If you go play tennis, you don’t take a ping-pong paddle with you,” Dr. Lepow said. “You need the right equipment, and the main equipment you need to be concerned about with running are the shoes.”

Before buying a shoe, Lepow said it’s important to find a store that specializes in athletic apparel and shoes, and take advice from a knowledgeable salesperson. Athletic shoes should be a half size or full size larger than your regular shoe size. There should be a thumb nail’s distance between the end of your big toe and the end of the shoe, and you can determine that once you are standing with the shoes on and laced up. “Athletic shoes are broken down into two categories: running shoes and training flats,” Lepow said. “A running shoe is best on a track or running outdoors. A training flat is better inside a gym for lateral or side-to-side movements on machines.”

Proper Shoes Can Help Prevent Falls: Bahamas Podiatrist
According to Monique Mitchell, DPM. MPH, "Falls put seniors at risk of serious injury including hip fractures and head injury. Fortunately, most falls result in only minor scrapes and bruises, if that. But they can be frightening, and even if there’s little physical harm, people sometimes develop a strong fear of falling that can change their entire life and behavior. Between five percent and 10 percent of falls among older people do result in a major physical injury such as broken bones, serious cuts, and bad bangs to the head.

"Everything from slippery throw rugs, poor lighting, and side-effects from multiple medications has been identified as risk factors for falling. Foot problems and pain were also mentioned as risk factors for falls. Many people are not wearing the correct size or style of shoes which can contribute to falls. The Harvard researchers found no difference in the type of shoes worn and falls. They did however find that people who wore shoes indoors were less likely to suffer a serious injury from a fall than those who wore slippers, socks, or went barefoot. Shoes definitely helped with balance and support which will prevent falls," says Dr. Mitchell.

RE: The Passing of John Mark Enger, Jr. DPM
Dr. John M. Enger, 63, of Albert Lea, MN, passed away at St. Mary’s Hospital on December 31, 2021 after a courageous battle with COVID-19. He attended the Des Moines University College of Podiatric Medicine, graduating in 1987 with a Doctorate of Podiatric Medicine degree.

While in medical school, he was introduced to Elizabeth Farrell while she was attending nursing school in Des Moines and they married in 1986. The couple moved to San Francisco, CA, until Dr. Enger finished his residency. He and Liz, wanting to be close to family and friends, decided to move to Albert Lea, MN in 1988. He purchased the Community Foot Clinic from Dr. Loring Stead, then built a new clinic.

Query: Accepting Medicare Assignment
I have resisted accepting assignment with Medicare patients for 32 years BUT the competition now is much greater. Where I live there are 3 ortho foot and ankle physicians and 10 or so podiatrists. I find myself in a cash flow problem due to low patient load and am considering changing my practice to accept Medicare assignment.
I have heard points in favor of staying out like from derm and plastics as well as points in favor of accepting assignment. I thought I would get the opinion of some of my colleagues in both camps.

We had this exact problem here in Ohio and we checked with our patients to see if they initiated the refills. They too did not request the refills. We filed a complaint with the State of Ohio Pharmacy Board. The faxes quickly stopped.

Unfortunately, with the single sheet faxes of today, even when you mark unsubscribe, they still keep coming and it is hard to stop. Many years ago, I was getting 3-4 faxes a day from one company advertising office furniture. At the time, we had a fax that used a paper roll, and I attached 3 pages together (top of the 1st one to the bottom of the last one) that said UNSUBSCRIBE, called the number and left for the night. Surprise!! No more faxes from them. I always had a vision of them not being able to get in the office the next morning because of the mass of paper blocking the door.

I don’t disagree with Dr. Ressler as to ON shoes being the hot fashion shoe but to call it or to refer to it as a running shoe is completely wrong. New Balance, Brooks, ASICS, and Saucony are running/walking shoes. These companies are using technology to determine what needs to go into their products. ON uses eyes to determine theirs.
Our job is to guide our patients, and honestly my patients seem to appreciate and follow my recommendations as to the correct brand of shoes. ON shoes are fine to wear to the movies (whenever that is) but not as a replacement for proper athletic shoes.

RE: Preparing Podiatric Medicine for its Future Role in Healthcare: A Half Century is Much Too Long

As a 1980 graduate of NYCPM, I can say it has now been 41 years of the same old story - podiatry lamenting its role and position in healthcare. Podiatrists feeling "less than" and instead of calling themselves podiatrists, they introduce themselves as foot and ankle surgeons and say they went to medical school; 41 years of podiatry as a “second class” profession. To be honest, I have NOT had a good experience being part of this profession.
I now avoid interacting on a professional level with other podiatrists. However, I have had a wonderful and rewarding career and at 67 years of age, have no plans to retire. I love what I do and how I practice. But I have found the overall mentality of most podiatrists to be negative and disappointed in their career choice. Many “hate” podiatry. Most have a long list of grievances. It’s too late for me to...
