PA Podiatrist Discusses Diabetic Neuropathy
Brittany A. Portonova, DPM, a podiatrist who practices in Hazle Township, Pennsylvania, tells DiabetesMine, “For pain control, mild cases of diabetic neuropathy pain can be managed well with a complex mix of vitamins, most notably Vitamin B6 and B12, in addition to alpha-lipoic acid. Some vitamin stores or pharmacies will offer complex/combined vitamins that are geared specifically toward diabetic neuropathy. Moderate to severe neuropathy pain is usually well managed by a combination of over-the-counter or prescription topical pain creams and oral prescription medications with much success.”

Dr. Portonova continues, “Secondarily to pain management, we look to supplement with proper supportive shoes, orthotics, and diabetic shoes. It’s important in the presence of neuropathy to be fitted for these devices by either a podiatrist or credentialed pedorthist to make sure that you are getting a quality product that will not cause any unwanted blisters, sores, or infections in the setting of neuropathy. Lastly, therapy and exercise regimens are highly recommended to help with chronic pain, weakness, and gait disturbances that can affect those with neuropathy.”

After over 25 years with the VA healthcare system and over 38 years in practice, Eugene Goldman, DPM is retiring. Dr. Goldman will end his career at the Coatesville VA Medical Center, in Southeastern PA, as the first podiatrist to become an Associate Chief of Staff for Education, a position he has held for over 17 years.

3 Digital Payment Trends that Physicians Need to Know
Following are three digital payment trends that have already begun and will continue to accelerate in the coming years.
2. Mobile phones are the new payment terminals: Contactless mobile payment options, such as Apple Pay and Google Pay, continue to gain traction with consumers, with in-store mobile payment usage in the U.S. growing 29% last year, according to eMarketer. In 2020, 92.3 million U.S. consumers aged 14 and above used proximity-based mobile payments at least one time. Additionally, more than half of all smartphone users are expected to use contactless mobile payments by 2025.

Query: Coding Subtalar Joint Implants
I am performing surgery on one of our patients which will involve a gastrocnemius recession (CPT 27687) and a subtalar implant (arthroereisis) on the left foot. We are having trouble choosing a code for the arthroereisis procedure. It was suggested we use CPT 28899 and 0335T but I am not finding either of those codes on Medicare’s Fee Schedule and was told by another biller that they are new codes/fees. Do you feel these are the correct codes to use or know of any other code for the subtalar joint implant that we could use?

Response: There are only two choices for your procedure coding to consider:
1) CPT 0335T – Sinus Tarsi Implant Procedure
2) HCPCS S2117 Arthroereisis, Subtalar
You pick the code based on the description above. Please note that many payers consider this procedure as investigational and experimental, therefore not covered.

RE: Autofax Pharmacy Refills

My office often receives far too many faxes from pharmacies asking me to authorize prescription refills for my patients. This past year, before authorizing or declining the refill, I called or sent an e-mail to each patient, asking them if they wanted or needed the refill and if they had requested the refill. To my surprise, almost 100% of the time, the patient said they knew nothing about the fax, and had not initiated any request and they did not need the medication any longer.
My office is inundated with faxes on a daily basis from outsourced carriers asking for medical records, physical therapists who send cut and pasted medical records asking for my signature and a return fax to authorize continued care, and these unwanted faxes from pharmacies for prescription medication refills that were never requested by anyone. My office has to send back a reply fax to the pharmacy denying the request for the refill, or they will continue to send the faxes many times.
Any suggestions on how to curtail these endless unwanted faxes would be appreciated. I am aware that I could get rid of my fax machine but I also receive faxes (MRI reports, etc.) that are important such as insurance credit card payment vouchers and others. Can't something be done on a legislative level to stop the pharmacies from auto-faxing prescription refills when no one ever asked for the refill?

As podiatrists, we are always trying to recommend the best supporting athletic shoes to our patients. On several occasions it becomes very frustrating treating patients only to deal with their non-compliance due to "heavy or ugly" shoes. As an alternative, I have been researching "ON" running and athletic shoes quite extensively and have found several models to provide good support while checking the boxes of providing support and pleasing aesthetics.
My philosophy has always been to get patients to purchase shoes that they will wear as opposed to them continuing to wear their Keds and other brands just because they feel good. Let's face it, getting them to wear Brooks Beast or Ariel, New Balance 1540 and others is not always an easy sell. As most of you know, ON shoes are setting the fashion world on...

My premiums were going up significantly as well. My insurance company gave several options and I chose the one that does not increase my coverage by the cost of living. As a result, my premium was cut by 2/3. It is a chance to do this but I feel I will be able to cover any difference.

The ongoing USMLE discussion on PM News tends to embarrass me. Posts so far, giving thought to the past 50 years detailing buddy relationships with MDs, invitations to join professional MD/DO societies, faculty appointments in medical schools, task force generation, even teaching MD or DO students and residents, etc., are minimally interesting as historical and even present events. Disclosure: I also have enjoyed and appreciated much of the above in my four decades in the profession. However, these “achievements”, no matter how they stroke the ego, never did, and do not now, intimate anything close to recognition (by organized MD/DO medicine) of the education and training of Doctors of Podiatric Medicine as like or equal in any way to that of MD and DO degree holders.
It is also true that they do not intimate that the DPM degree is inferior to the MD or DO degree, just DIFFERENT. These things are true whether or not you are best friends with an orthopedic surgeon, or play golf with a neurosurgeon, or even if you operated on a member of their family. These “relationships” may indeed be advantageous to...
