Grace Jurgiel, who has been interning at Segel Podiatry for the past five years is leaving to begin her medical school studies in podiatry. She returned to Martha's Vineyard when remote learning became the norm as a result of the coronavirus pandemic - back to Segel Podiatry and the mentorship of Jay Segel, DPM

Grace will be leaving soon to attend Temple University’s School of Podiatric Medicine in Philadelphia. Her interest is the study of rehabilitation and biomechanics, and Dr. Segel has given her a strong preparation, as that is his particular interest; he is a well-regarded expert in that area. Grace plans to return during the summers to continue her work with Dr. Segel and hopes they will practice together after her graduation in 2024.

NY Podiatrist Receives 4th Annual Ronald Soave Award
 
On December 3, 2020 at the New York State Podiatric Medicine (NYSPMA) Downstate Meeting, Lawrence Santi, DPM was presented the fourth Ronald L. Soave, DPM Integrity and Excellence in Podiatry Award. The inscription on the award reads as follows: “In recognition for your time and dedication to the podiatric profession and for your outstanding leadership in the APMA and NYSPMA.” The award was presented by the Award Founder Corrine E. Renne Landau, DPM and Dr. Soave.

4. Use the Remaining Money in You Flexible Spending Accounts (FSAs)
 
Most healthcare and  dependent care FSAs have a use-it-or-lose-it feature to them where any remaining balance in the account at year-end is forfeited back to the plan. Some employers offer a grace period into the spring of the following year or a $500 carry-over from one year to the next, but most do not. If you have unused funds in your healthcare FSA, consider scheduling end-of-year check-ups to try to empty the account. Leftover funds in a dependent care FSA could be used for after-school programs and camps.

RE: Pitted Keratolysis on the Foot in a Patient with Hyperhidrosis

A 22-year-old man presented to his primary care provider for follow-up of a laceration to the medial aspect of his right foot. On examination, he had multiple dark spots on the soles of both feet that didn’t bother him. His feet often got sweaty and smelled. The man was diagnosed as having pitted keratolysis (PK), characterized by multiple small pits on the soles of the feet which is often associated with hyperhidrosis and significant odor. The lesions usually have a punched-out appearance and are flesh-colored. The dark color of these lesions was due to the patient’s footwear.

PK is caused by bacterial overgrowth in the stratum corneum. Corynebacterium is the most common bacterial culprit, but Kytococcus, Actinomyces, and Dermatophilus have also been implicated. In addition to antibacterials including topical clindamycin, erythromycin, fusidic acid, and benzoyl peroxide, standard treatment for hyperhidrosis is advised. (J Fam Pract. 2020 December;69(10).

RE: Increased Malpractice Requirements by UnitedHealthcare

I recently found out that beginning in March 2021, UnitedHealthcare is raising the required minimum malpractice insurance coverage to 1 million/3 million in order to participate with their insurance, whether you are just applying to be a provider or to be re-credentialed. Until now, the requirement in Michigan was 100,000/300.000 to be a participating provider. This is the new rule for Michigan a 10 times increase! They are doing this in 13 states. Some of the states already have higher requirements but I think the highest 500,000/1.5 million.
 
So no matter which of the 13 states you are practicing in, you will have to increase your coverage from 2 to 10 fold. This is not a podiatry thing, but affects all practitioners. How can they do this? Why would they do this? We all know when a claim...

From a mechanical perspective, the decision on whether a joint should be fused, or not, is not that difficult. One simply needs to ask the question, “Will my patient benefit biomechanically more from sparing the joint or from fusing the joint during their daily weight-bearing activities?”
 
In order to arrive at a good biomechanical decision as to whether a joint fusion or a joint-sparing procedure is best for the patient, one must consider a number of factors. First of all, we must ask if fusing the joint is absolutely necessary in order to render the patient asymptomatic, to perform their daily weight-bearing and sports activities without pain, and to prevent progression of deformity over time. I was one of the small percentage of respondents who indicated I would not get the vaccine. But as with all surveys - there is little room for interpretation or explanation regarding why a response was affirmative or not.
 
In this particular circumstance - I will not get the vaccine because I had COVID-19. I now have natural immunity for at least a period of time based on my antibodies. The duration of this immunity may be up for debate but there is literature that suggests this may be for several months.
