NJ Podiatrist Discusses Best Ways to Prevent Blisters
As podiatrist Velimir Petkov, DPM tells Allure, blisters are small pockets full of body fluid and when it comes to those that occur as a result of physical activity, they're most often caused by friction. "Wearing shoes that are too tight or the wrong socks can also cause them," Petkov says. "A lot of runners get them especially when running in hot and humid conditions." While a bad blister can certainly ruin a good workout, they're actually our bodies' way of cushioning and healing damaged skin, Dr. Petkov explains.

Petkov's number-one suggestion for avoiding blisters (specifically the super common type that pop up on feet) is to focus on footwear — and that includes socks. He suggests buying some moisture-wicking socks for exercise. If you do end up with a blister, Petkov says blister pads, padded bandages, and moleskin are best to not only prevent more blisters but protect the ones you may already have as well.

Skin and Eye Protection Against UVC from UV Germicidal Irradiation Devices
As the COVID-19 pandemic stretches on and personal protective equipment continues to be depleted, ultraviolet C (UVC) germicidal radiation continues to be a frequently utilized method of decontamination of N-95 masks to allow reuse. This leads to increasing exposure to UVC by healthcare workers performing disinfection. The authors exposed three sunscreens, a pair of UV goggles, and a clear face shield to UVC irradiation five separate times, then averaged the transmission for each sunscreen/device. The baseline irradiance was 22.72 mW/cm2, and this was reduced to 0.01 to 0.02 mW/cm2 by all sunscreens, goggles, and face shields.
Sunscreen and goggles or face shields should be recommended for anyone performing UVC decontamination procedures to reduce the risk of erythema, photoaging, skin cancer, and photokeratitis or conjunctivitis associated with repeated exposure.

An emergency fund should, in general, have 3 months of necessary expenses saved in cash if you are a dual income household. That number increases to 6 months, to even a year, if you are a single income household or one person’s income is relied upon to provide for your family’s standard of living. It is always a good idea to review your budget to see if any changes to your necessary expenses occurred. If your expenses increased or if you had to dip into your emergency fund for the year, now is a good time to focus on replenishing it.

When we see a patient, we perform an H&P. We then know their health status. But, what do the patients know about our health status? What is their expectations of our health? It should go without saying that they do not want to catch a disease from us. If they are immunocompromised and pregnant, or a child who is immunocompromised, and we pass measles to them, how would we live with ourselves? What about polio, flu, Haemophilus influenzae type b meningitis, pertussis, rubella, etc.?
I hope you have been covering yourself with PPE from head to toe, for the sake of your patients. What extraordinary measures have you been taking to keep your patients safe? Now that you have made your position on vaccines public, what is your increased liability? Have you informed your hospital of your personal decision not to vaccinate, as well as your employees and or partners?

RE: Proposed CPME Residency Requirements

I was recently told about a proposed change in residency requirements (by CPME) for logging “biomechanics cases”. Apparently, the number of cases needed was 75 and the proposal was to change it to 50. This seems like a very small number to me. Most surgical cases are born out of faulty biomechanics. This being the case, doesn’t 75 cases over a three-year period seem low as well? I always thought one of the things that differentiated podiatry was that we were rooted (no pun intended) in biomechanics.
I would think both ABPM and ABFAS would feel biomechanics to be important. I realize the American Board of Podiatric Orthopedics got swallowed some years back, but the tenets of the profession are based on biomechanics; are they not?

I just checked and Gill Podiatry has the product for sale. You can also contact a compounding pharmacy in your area to make this for you. One caveat: remember under OSHA rules that once you open the bottle, you have thirty days to use the product at which point you must discard the contents. The bottle must be clearly labeled as to the date it was opened.

I can best distill Dr. Ribotsky’s query by stating: “Why would we want to surgically create a hallux rigidus as a primary treatment when FHL, hallux limitus, and hallux rigidus are root causes of biomechanical foot and postural pathology in civilized mankind?
I can only answer Bret by supplying disruptive terminology and practice habits into the mix: #1 If a higher rearfoot or forefoot SERM-PERM interval exists, that disruptive, superfluous motion must be reduced or eliminated using some combination of props, surgery, and joint and muscle engine training or therapy.
#2 If a lower rearfoot or forefoot SERM-PERM interval exists, then additional motion must be supplied using props, surgery, or joint and muscle engine training or therapy. This can only be accomplished by consultants with enough education, experience, dexterity, and passion in their respected area(s) of specialty to eliminate bias and economic drive.

The commentary of Dr. Saxena regarding arthrodesis in athletes is contrary to retrospective studies. For example, Da Cunha EF, et al. (Foot Ankle Int. 2019) demonstrated that arthrodesis in young athletes was reasonable, effective, and associated with a high rate of satisfaction and resumption of athletics for a variety of sports. Similarly, McMahon, et al. demonstrated the effectiveness of the Lapidus procedure in athletes (Foot Ankle Int 2016).
Yes, athletes like all people require some adjustment following 1st ray arthrodesis surgery. And yes, other joint-sparing procedures in anyone, including athletes, is generally preferable to arthrodesis when indicated. However, to suggest that a well performed arthrodesis when indicated is a “a death knell” for athletes is simply not consistent with published studies on the subject.

Allen Jacobs, DPM, claims that the use of a posterior tibial block in acute gout is an unproven theory. There is nothing theoretical about the use of sympathetic nerve blocks (e.g., posterior tibial nerve block) to cause vasodilatation in acute gouty attacks. Not only does the block typically result in immediate relief of pain, but also vasodilatation created brings more blood to the innervated areas. This also results in a significant rise in temperature.
The chemistry of saturated solutions is clear, based on long known concepts, not merely a theory. That is, by increasing temperature and fluid volume, the sharp crystals of the supersaturated solution of sodium urate dissolve and, in addition, Ph increases. A low Ph (i.e., acid) results in the increase of crystals of sodium urate. The reason a PT block is not popular in other medical specialties is likely that the average physician does not know how to administer it, including rheumatologists. No one can administer a PT block better than a podiatric physician.
