Query: Denials for Chemical Neurolysis
I have recently been denied payment for chemical neurolysis for neuromas by Sisco insurance. They contend it is experimental/investigational. I appealed their decision and was given a peer to peer review with a general surgeon who admitted he has a contract with the insurance company and is paid by them. He states he used up-to-date evidence-based medicine to deny payment. In my letter of medical necessity to the insurance company, I cited many sources as to why this is a viable effective treatment. I also stated that other insurance companies like Medicare approve and cover this treatment. When I asked him why other companies paid and his didn’t he couldn’t tell me why.
What are other practitioners doing in these types of cases? Charging the patient out-of-pocket? Continuing to appeal?

Query: Amniotic Fluid Injections for Moderate to Severe Osteoarthrosis
I have a 57 year old healthy, female patient with right ankle trauma in 1987. Recent x-rays, MRI and CT resulted in the following conclusions: Mild posterior subtalar joint osteoarthrosis, severe talonavicular joint osteoarthrosis, mild calcaneocuboid joint osteoarthrosis, moderate navicular-intermediate cuneiform joint osteoarthrosis, moderate navicular-lateral cuneiform joint osteoarthrosis, and mild navicular-medial cuneiform joint osteoarthrosis. She is looking to avoid surgery, as her only real option is multiple mid-foot fusions. She has worn a Ritchie brace, undergone cortisone injections, and exhausted all traditional, conservative medical care.
My question to my colleagues who have incorporated amniotic fluid injections and utilized them for a severe case is what were the results? I understand that everybody will respond differently. The patient is very open to the idea and I further believe that amniotic fluid will be more beneficial for her than PRP. With the multiple involved joints involved, the plan is for at least 4cc of fluid into multiple joints. The cost is not an issue, but I am curious what your experiences have been with its use for what I would consider to be a more extreme scenario than just PF, mild OA, neuroma, etc.

Is this an April Fool's joke that's 9 months late or possibly 3 months early? Dr. Quezada states "Let nature take its course". That's good advice when growing weeds but hardly appropriate coming from a podiatrist ostensibly licensed in the State of Texas. Does D. Quezada let nature take its course with an ascending cellulitis? Nature's course with an acute compartment syndrome? Does nature resolve acute necrotizing fasciitis? If he's suffering from an acute case of vaccine hesitancy related to the new vaccines for COVID-19, one can understand that.
How about vaccinations that have eliminated diseases that were once a scourge such as polio, smallpox, diphtheria, and measles? If nature's way is his philosophy, does he simply lay his hands on all of his patients and pray with them for divine intervention? If yes, what's the code for that service? Asking for an agnostic friend.

The commentary of Dr. Ribotsky regarding the utilization of arthrodesis in foot and ankle surgery may represent his personal opinion, but is hardly consistent with the appropriate science and literature. Nor are his comments consistent with the manner in which surgical decision-making occurs. Arthrodesis of the 1st MPJ is not considered a primary option when reasonable alternatives are present. Arthrodesis is utilized when there is advanced degenerative disease, significant instability, neuromuscular disease, recurrent deformity, such that osteotomy is neither practical nor likely appropriate. It is an alternative to resection or implant arthroplasty of the 1st MPJ. Retrospective studies demonstrate a very high rate of patient satisfaction following 1st MPJ arthrodesis.
There is little functional loss subsequent to performance of a Lapidus procedure. The Lapidus procedure achieves a more durable and predictable maintenance of correction with a lower incidence of recurrent deformity. I know of no accomplished surgeon performing surgery for limitation of excessive and pathological pronation deformity (e.g. PTTD) who utilizes calcaneal cuboid arthrodesis in preference to an Evans procedure when indicated. Conversely, preservation of joint function has been increasingly advocated for example, TAR in deference to ankle arthrodesis, or double calcaneal osteotomy in preference to STJ arthrodesis.
Following clinical and radiographic evaluation of the joint and deformity, and patient specific factors, there are indications for joint preservation and there are indications for arthrodesis. Preservation of joint motion is always desirable, but not at the sacrifice of the high likelihood of procedure failure.

I have read various responses as to this question from well learned colleagues. So far, however, I have not read about some "classic treatment". First of all, a good history, regular radiographs, and physical evaluation of the great toe joint should lead you to believe you have an acute gouty attack.
A local or PT block, then an “aspiration” of the joint fluid is then accomplished. You should see either a gouty synovial fluid aspirate or in the rare case of a septic joint, the synovial fluid murky with purulence. After aspiration of the contents of the joint leaving the aspiration needle in the joint, swap out with another syringe for a...
