UK Podiatrist Discusses Platform Shoes
Platform shoes come in all forms: a high heel and a thick sole; or flatforms, where you walk on a flat platform. According to podiatrist Emma Supple, "You may feel more secure in these shoes and less likely to wobble after a cranberry and vodka, but you're actually just as unstable as in heels. Many people develop a sort of stomping walk in platforms, which puts pressure on the joints. This can lead to very sore feet, calluses, and arch pain.

"Some platforms are heavy and the extra weight is going to strain leg muscles. The solid nature of the sole means the foot is unable to bend properly so you may suffer immediate foot pain. Ensure the platform is made of a light material such as cork and that the shoe is strapped or buckled on for extra stability," says Supple.

Louis DeCaro: Something that many DPMs do not think about is eyesight (vision). Sometimes, children with focal point vision pathology sense that the ground is sloping. I have seen a bunch of children with eyesight pathology. Once this condition has been identified and dealt with, these kids do not rise up on their toes again. Thus, all DPMs should add vision testing of children to their work-up.

Penalties Rise for Failing to Sign Up for Health Insurance
Americans without health insurance will face stiffer tax penalties and other sanctions in 2016 for failing to sign up for coverage under the Patient Protection and Affordable Care Act, The Fiscal Times recently reported. Compared to this year’s average household penalty of $661 for not carrying health insurance despite enrollment eligibility, the average fine in 2016 is estimated to increase 47% to $969, according to the article, which cited information provided by the Kaiser Family Foundation.
Broken down by subsidy eligibility status, the average 2016 penalty for not signing up is estimated to run $738 for people eligible for marketplace subsidies and $1450 for people ineligible for subsidies.

How to Deal with Negative Online Reviews- Part 2
Online review websites can often determine whether or not a patient decides to use your practice. Reviews matter, and negative ones can derail your practice. Here are some tips on how to deal with negative reviews:
2. Move the conversation offline.
It’s best to move the conversation offline as soon as possible, before HIPAA-protected information is unwittingly exposed. “If you can identify a reviewer, reach out to them by phone,” said McClintock. Express your appreciation that they took the time to leave a review, even if it was negative.
If you can’t definitively identify the person who left the negative review, leave a comment on the review, encouraging them to contact the office. “You could say, ‘We’re sorry you had a bad experience with us. We'd like to hear more about it so we can improve our level of care and make things right. Would you call [specific person] at [direct line]?’” said Hamblin. This way, you’ve acknowledged the complaint publicly and others can see that you’re trying to rectify the issue.

Dr. Cohen's comment is well stated. First, in my experience Paul Kesselman is under-recognized in our profession as to how much effort and time he puts into addressing such issues. There are many others also who work for the benefit of all of us with little fanfare.
Secondly, he points out that the best ammunition Dr. Kesselman can have is information. Those who have such cases or related information that would be helpful should take the time to concisely share this with APMA. This will become increasingly more important as increasing attention is given to healthcare cost containment which can, at times, sacrifice quality of care. Vital services to help patients cannot continue without proper reimbursement, which includes not only the level of payment, but also the time and effort it takes to secure same.

Many years ago in a Medicare World far, far away, I billed CPT 20550 with the diagnosis bursitis. Then one day, the Empire no longer permitted it. There is no code in the Empire for steroid injection for bursitis. There was no rebellion. There are many codes that are no longer covered for certain diagnoses. CPT 20550 is used for fewer diagnoses today than when started in 1984.
I can't say that I disagree with Dr. Amarantos, but they can't be the ones to arbitrarily decide what is covered and what isn't covered without any input from the professions. Maybe they decide not to cover a fractured toe because...
