NY Podiatrist Discusses Blisters
 
According to Jacqueline Sutera, DPM, a Manhattan podiatrist, there are two top causes of blisters on feet: friction and rubbing. “Blisters are fluid build-ups under the most superficial layers of skin and can form in areas of motion, such as joints,” she says. “They also typically form near bony prominences, which make contact with the shoe while running or walking. That friction causes the layers of the skin to pull apart and form a little bubble, which in turn, fills with fluid.”

Dr. Sutera says excessive moisture, like sweating, can also encourage new blisters to form. And if you’re wearing shoes that are new, too tight, or too big, you may experience blisters on your feet, too.

NY Podiatrist Holds 13th Annual Holiday Shoe Drive for the Homeless
 
Dr. Bruce Pinker of Progressive Foot Care conducted his 13th Annual Holiday Shoe Drive for the Homeless. To encourage people to donate footwear, and to help promote local businesses, he also held a raffle drawing with gift certificates from many local businesses.

Everyone who donated shoes was eligible for a raffle ticket. On December 15th, the winning tickets were drawn in White Plains. Over 1,000 pairs of footwear were collected this year.

4. Make Time to Regroup and Refresh
 
When you go through the process of introspection and investment required to commission and evaluate logos, website designs, and other marketing materials, it's natural to feel a sense of completion. But branding is never really done. Over time, the way you think about your brand will gradually evolve. Design trends will, too. Make a point of taking a fresh look at your brand and marketing at least every five years or so, to be sure your materials represent who you are today.

Mona Feigenbaum, DPM died December 23rd after a long courageous battle with breast cancer. Dr. Feigenbaum graduated in 1985 from the Temple University School of Podiatric Medicine and practiced in Plano, TX.

I have been using squaric acid topical immunotherapy for warts since it replaced dintrochlorobenzene (DNCB) years ago. It is the treatment of choice for a plantar verruca in my office since the mid '80s, with over 99% of my patients using that regimen. As you know, regardless of the method you use to treat verrucae, it can be frustrating. Most of my patients are young. They come into the office during the season of whatever sport they play. They show up because the lesion hurts. They want to treat it but also want to continue playing their sport. 
 
This is the perfect treatment under those conditions. I treat 3 to 4 of them daily. The local pediatricians and dermatologists refer me their verruca patients because they don't have a great way to treat it. It's too frustrating for them to see the patients until it resolves.

Sorry if I am not jumping on the bandwagon in order to be granted the privilege for administering the COVID-19 vaccine and you can refer to Dr. Kass' post regarding this as well as he had stated this more eloquently than I am about to. This in no way advances our profession, nor is there any financial incentive. Don't get me wrong, I am all for helping my fellow man, woman, and child as I have done that my entire career but enough is enough. Maybe it's the $70 capitated check I just received for a bunion surgery or the $128 for a pair of orthotics, combined with the 10% decrease in Medicare fees that is placing me in this holiday mood but show me the money, and the respect; both of which are sorely lacking. 
 
I don’t need to be considered a "real doctor" so I can administer an injection as this is absurd. Nurses, PAs, pharmacists can, but board certified podiatrists are unable to! How is this even possible? APMA, feel free to chime in at any time. I'll wait for what I know will be the usual politically correct response I have heard for the past 30 years. I'll remind you again when I am asked for a donation to the PAC.

I have to concur with Dr. Kass. It seems the Governor was, in making this provision, inadvertently provided with the training and qualification requirements of the British chiropodist as applicable in the mid-1960s. Will someone please provide the Governor the training requirements of a 21st Century podiatrist, USA. That should resolve the issue! Whatever next, perhaps the Governor would like to legislate that the podiatrist may treat the ingrowing toenail under supervision of the pedicurist.

I fully respect the opinions and belief systems of learned colleagues, and would like to foster an open dialogue. I wish to offer my opinion centered on Covid-19 vaccine hesitancy and wish to provide a mitigating and buffering response that may be used by podiatric physicians who wish to participate with administering the COVID-19 vaccine, and provide data to their patients as a public health initiative.
 
As I have viewed this vaccine hesitancy through the eyes of practicing two professions as well as an FDA and DEA advisor, the most widely recognized category of unavoidably unsafe products is prescription drugs. Some courts hold that all medications automatically qualify as unavoidably unsafe on the theory that...

Thank you Dr. Ribotsky for your observations and sharing (sadly) personal experience. There used to be a saying "refuse to fuse". Unfortunately, there may be a shift to do more fusions, so that both the patient and surgeon feel there is no more that can be done after. Therefore, both groups "settle". This may also be driven by marketing, in the desire to have a "new" technique. Doing a "Valenti" with no hardware has less reimbursement both to the provider and facility than an arthrodesis.
 
Another saying is don't do to a patient what you wouldn't have done to yourself. I know plenty of surgeons with neck fusions, rotator cuff and ACL repairs, total joint replacements, yet almost none with a 1st MPJ fusion or Lapidus. Certainly if "motion is lotion", fusion would not be a...

A PT block works very well for a gout attack. We must tell our patients that their foot and ankle is numb for a period of time depending on type of anesthesia used (lidocaine, bupivacaine, etc.) and they must essentially do no activities until there is full sensation. It sounds obvious but we still have to tell them.

I have long heard the unproven theorem that PT blocks are useful for the treatment of acute gout by inducing vasodilation and helping to "wash away" the uric acid. Funny how I believe the edema, hyperthermia, and erythema observed in acute gout represented hyperemia. The inflamed joint hardly appears to require additional vascular perfusion. PT blocks provide limited temporal effects. They are helpful for immediate pain relief. However, to suggest other theoretical effects is without a scientific basis and remains theoretical.
 
Speaking of theoretical, I utilize a focal anesthetic block with epinephrine to reduce the hyperemia and pain. My observation has been that epinephrine provides a more prolonged and profound analgesic effect. I want vasoconstriction. As I previously described, I perform a wide dorsal V block apex proximal. I do not encircle the 1st ray. Obviously epinephrine containing anesthetics are not utilized when inappropriate. PT block, dorsal block, no block. This is again the art of medicine, making reasonable efforts to reduce patient suffering.

The Shoe House, built in 1948 in Hellam, PA, was by far "Colonel" Mahlon N. Haines' most outlandish advertising gimmick. It is a wood frame structure covered with wire lath and coated with a cement stucco. It measures 48 ft. in length, 17 ft. in width at the widest part, and 25 ft. in height. The interior consists of five different levels and contains three bedrooms, two baths, a kitchen and a living room.
