NY Podiatrist Named Assistant Professor at Upstate Joslin Center
 
J. Kurt Concilla, DPM, a podiatrist, has been named an assistant professor of medicine for Upstate's Joslin Center for Diabetes. Concilla received his medical degree from California College of Podiatric Medicine and completed his surgical residency in Portland, OR. Prior to his appointment at Upstate, he worked as a board certified foot surgeon and podiatrist at Crouse Medical Practice. He resides in Cazenovia.

PODIATRISTS IN THE COMMUNITY
Retired Jewish OH Podiatrist Volunteers on Christmas Day
 
Members of B’nai B’rith, Mahoning Lodge 339, and the Jewish community at large put Operation Snowflake in motion on Christmas Day. Dr. Walter Malkin, a B’nai B’rith member, has volunteered about six years. “It’s a favor to our Christian friends,” he said of the project. “It’s something good to do, and you feel good doing it,” said the retired podiatrist. He helped staff the information desk at St. E’s, working the 10 a.m. shift. “It’s nice to see people in festive moods,” he said. “I enjoy being with people.”
 
Dr. Malkin, who was in practice for 48 years, said he is impressed by the crowds who visit the hospital on Christmas Day. “You see family and friends carrying in packages,” he said of the visitors remembering loved ones. “I wish this holiday mood would remain all year-round,” he said.

FDA Clears Solana Surgical Foot and Ankle Plating System 
 
Solana Surgical LLC has announced the FDA clearance of the CrossCHECK Plating System, the company’s new implant system for use by orthopedic and podiatric surgeons in foot and ankle procedures.
 
According to a company press release, the CrossCHECK System offers forefoot and mid-foot compression and stabilization features. The plates are manufactured in various sizes and configurations and have Type II anodization to enhance their fatigue strength and improve stabilization.

Other Drugmakers Likely to follow GSK in Halting Doc Payments
 
Partly in response to new physician payment disclosure rules, more drugmakers are likely to follow GlaxoSmithKline's decision to stop paying doctors for promoting its products at speaking engagements and for attending medical conferences. The company announced last week that it will stop making direct payments to healthcare providers for speaking engagements and attendance at medical conferences by the start of 2016. It also plans to roll out a new compensation system for every sales employee worldwide who works directly with doctors and other healthcare professionals who prescribe medications.
 
The moves come amid increasing scrutiny of the role of money in prescribing decisions. Under the Sunshine Act provision of the Patient Protection and Affordable Care Act effective in August this year, drugmakers and medical device companies are required to report payments and gifts valued at $10 or more to physicians and medical schools.

Query: Dispensing DME - Non-DME Provider 
 
I work for a hospital and we do not have a Medicare DME license. Because of this, I am told that we cannot dispense DME items to Medicare patients, even if the item is not covered by Medicare (e.g., OTC foot inserts). Is this correct?

Response: You can dispense, but the patient would be on the hook for the entire amount and you would have to have them sign an advance beneficiary notice (ABN). That ABN would need to specifically inform the patient that: 
1) You don't have a Medicare DME number; 
2) You can't submit the claim to Medicare; 
3) The patient could be reimbursed or the supply could be paid directly to another supplier only if that supplier had a Medicare DME number; and 
4) The patient would have to specifically state they understand these stipulations and was forfeiting their rights to reimbursement under Medicare. 
 
There is a form for patient self-submission to Medicare (CMS1490). The patient should understand that sending this claim form in will still not result in Medicare payment.

If this week's survey holds to its stats, so far a mere 30% of podiatrists are happy with their chosen profession, or have resigned themselves to the belief that they cannot change it. This is a shocking commentary on podiatry in the 21st century. Have we really grown as a profession? Have we matured in a way that has netted us the results we have all been looking for? Or have we become a profession of disappointed and unhappy people whose private lives will undoubtedly suffer from the fallout of professional dissatisfaction. This certainly speaks loudly for the failure of the APMA. Is anyone listening? Or are they just pushing forth their own agenda?  
 
We are no longer suffering from growing pains as a profession. We are suffering from victimization, complacency, and cooperation with a system that is going to completely destroy our ability to make a decent living.  
 
My opinion is that anyone who is dissatisfied with podiatry needs to develop skills that are unique and establish a niche practice. It creates value for your services and distinguishes you as different and better. It is a great way to feel satisfaction and increase your income. Being an expert is held in high regard. Being the same as every other DPM is not. Cut toenails a little too close and patients will leave your practice. Or, look outside the profession to connect to what you are passionate about and start to transition yourself out of podiatry. You are only stuck if you believe you're stuck. There are many ways to make a better living with a lot less stress.
