Dementia Patients Fare Better With Home Care: ME Podiatrist
 
When Dr. Jeffrey Johnson started treating Alice Osborne, who has dementia, he thought she had little chance of surviving more than a few weeks. Her health had been steadily deteriorating while she lived in a nursing home, making it unlikely that she would heal from a simple foot wound without life-threatening complications, he said. Then her daughter and son-in-law, Cheryl and Larry Barkow, moved Osborne into their Farmington home nearly three years ago, became her full-time caregivers and saved her life, he said.

“It took a lot of effort and major sacrifices on their part, but they are the biggest reason that she is alive today,” Johnson said during a recent check-up of Osborne, now 93. Although the medical community knows dementia patients fare better at home than in nursing homes, many people are just overwhelmed by caring for someone with such a life-altering brain disease, he said. “Your average person can’t do it because it takes someone very special to do what they do,” he said.

Aussie Podiatrist Urges Cricket Players to Wear Specialized Shoes
 
A consultant for a leading sports shoe company has urged Cricket Australia to copy the AFL and order contracted players to wear specialized shoes after he discovered many wore modified cross-trainers. Simon Bartold, the international research consultant for Asics, discovered some players fussed more about a bat than shoes that could protect their feet and legs from injuries.

He blamed "old wives tales" about the footwear for their attitude - and possibly their injuries. "There is lots of folklore about what kind of shoe a cricketer should wear, particularly a fast bowler, and we're finding it's hard to shift those established wisdoms," said Bartold, who has a background in podiatry and biomechanics.

CMS Proposes Sunshine Rules on Industry Payments, Gifts to Doctors
 
Drug and device manufacturers would be required to disclose payments and gifts they give to physicians beginning in 2013, according to proposed federal rules released Dec. 14.
 
The Centers for Medicare & Medicaid Services published draft regulations outlining how companies would disclose financial relationships with physicians. The so-called sunshine rules, called for by the health system reform law, would ensure the transparency of transfers of money and gifts to physicians and discourage potential outside influence on clinical decision-making, said Peter Budetti, MD, CMS deputy administrator for program integrity.

Query: Creating Patient E-mail Addresses

I was told by an adviser at Practice Fusion that a good way to dispense clinical records was via email. Unfortunately, Many of our elderly patients don't have a computer. So Practice Fusion recommended creating e-mail accounts for them. However, I ran into a limitation with Yahoo, which only allows you to create a certain number of accounts. Does anyone have a way to circumvent this problem?

Can I report the PQRS measures to the Medicare Advantage/Medicare Plus Blue plans? Or are the guidelines for reporting just for regular Medicare?

Response: Nothing stops you from reporting them, but there is no reason to do so as you will not receive any credit or payment. I should, however, note that I have received some minimal payments from Medicare Advantage carriers for eRx measures.

The 1/8" felt acts as a plantarflexory strapping for the 1st MPJ. For a few days, it keeps the hallux from dorsiflexing. How the patient responds with both pain relief and activity inhibition will help direct a more permanent treatment plan.

RE: Compatible Billing Program or Billing Service with Practice Fusion (John Scholl, DPM)

Practice Fusion has their own billing component to their EMR software - called Kareo. From my review, it seems to be about 30% the cost of other "billing" software (150-300/mo). Practice Fusion is FREE (you must put up with some ads), but it has over 100,000 users now in all specialities. AthenaHealth has about 33,000.
 
I know of several pods using PF. I would like to hear of any experiences with this software and/or its billing component.

RE: MN Podiatrist Supports Statewide Single Payer Health Insurance System (Todd Rotwein, DPM)

Perhaps I need to clarify my prior statement. I see no need for any additional regulatory bodies. Dr. Rotwein mentions a model such as something similar to the operation of public utilities, but I see no reason for that. We have a system in place now that pays 97% of proceeds to providers and 3% for administrative costs, and that is Medicare.  The private insurers pay 75-80% of proceeds to providers and 20-25% for administrative costs, if you believe the proponents of a single payer system. My rhetorical question is that if the government can run on 3%, why can’t private insurers? Of course, we all know the answer to that. That difference of administrative cost adds up to about $400 billion yearly out of $2.5 trillion in total healthcare spending, again if you believe the people who advocate for single payer.
 
If you are fearful of government control of this segment of the economy, I think that has already occurred. I’ve been in private practice since 1989, and insurance reimbursements have always been established along the Medicare rates, at least in my area. Unless you accept no insurance coverage, including Medicare and Medicaid, you are not in control of pricing your services. The pricing has already been determined, that is the RBRVS. I do agree that the political corruption that exists in this country gives me pause, but I don’t know that the CEO’s of BC/BS, Aetna, Cigna, etc are any better. I also think that the private insurance industry contributes so much to the political corruption in Washington, and that we would be better off if they didn’t exist to the same degree that they do now. If you eliminate the reason for their PACs, which a single payer might do, the government might be more responsive to the electorate on health issues.
