PODIATRISTS IN THE COMMUNITY
IL Podiatrist Donates 3 Million Dollars to Library

Aurora Public Library will become The Richard and Gina Santori Public Library of Aurora when it opens its doors in spring, 2015. Aurora Public Library Board of Directors President John Savage announced today that the Richard and Gina Santori Charitable Foundation is making a donation of $3 million to the Aurora Public Library Foundation to establish a technology endowment.
Dr. Gina Santori

Dr. Santori said her late husband, Richard, was dedicated to lifelong learning. Richard Santori passed away in 2010. He was the owner of several auto dealerships both in and around Aurora, including Valley Honda, Valley Imports, and Valley Mazda Volkswagen. Gina Santori of Lisle, a physician and surgeon, who is on staff at Rush-Copley Medical Center in Aurora, now practices only on a pro bono basis caring for diabetic and renal patients.

PODIATRISTS AND SPORTS MEDICINE
OR Podiatrist Discusses Plantar Fasciitis

The term plantar fasciitis is actually a misnomer, as it implies that the plantar fascia (a band of tissue running from your heel to bones near the ball of your foot) is inflamed. Studies show there’s actually little inflammation present; rather, the fascia thickens dramatically (usually 6-10 millimeters compared to 2-4 mm in people without plantar fasciitis) as a result of being tugged on too intensely and frequently (this is called tendonoses).

Dr. Clifford Mah

“In addition to orthotics, calf/hamstring stretching, and lower extremity strengthening are the keys to addressing the root cause of PF,” says Dr. Clifford Mah, a Beaverton-based podiatrist. “If the mechanical loads aren’t lowered on the plantar fascia, symptoms will persist. Daily stretching is important, especially before and after activities.”

MEDICARE NEWS
HHS Adds 123 Medicare ACOs

HHS announced the latest round of accountable care contracts in the Medicare Shared Savings Program, adding 123 additional ACOs and reaching about 1.5 million more Medicare beneficiaries.

The new contracts, which bring the total number of Medicare ACOs to more than 360, come as there is still little known about how the earlier adopters have fared in the program, which was established under the Patient Protection and Affordable Care Act. The first cohort of organizations in the Shared Savings Program, numbering 27, entered the program in April 2012.

QUERIES (NON-CLINICAL)
Query: Listing of ACA Participating Physicians

Where can we find out if we are on the list of participating physicians for the ACA insurance program? We've been told that we should have gotten a letter, but I have yet to see one from any insurance companies I participate with.

RESPONSES/COMMENTS (CLINICAL)
From: Peter Bregman, DPM

When dealing with any RA patient, you have to think about the whole picture and make sure you explain all of the conservative and surgical options. Also, as medicine becomes more advanced, you have to consider the notion that the process may be halted or even reversed with the advent of new medical advances in the next 5-10 years. Doing anything destructive may not necessarily be advised. Depending on the patient's expectations, your treatment plan should be guided by that, and the medications the patient is already on need to be considered.

This is not only an RA foot, but it is a skew foot as well. So, if conservative therapy (orthotics, meds, shoe modification) is not on the table, and surgery is being considered, then I think this case lends itself to a combination of MIS and straight-forward osteotomies to correct deformities. I would attempt this before doing any joint destructive procedures. If Weil osteotomies are not optimal due to the distal destruction or lysis, then proximal osteotomies should be considered to correct in the plane(s) of deformity that is causing the most pain.

RE: Off-Line Benefit of PM News
From: George F. Jacobson, DPM

A benefit from PM News has been the individual e-mails between colleagues. I e-mailed Larry Kosova, DPM who helped me several years ago with Dragon. He was gracious enough to answer my question about upgrading while he was on vacation in Israel. There have been many other podiatrists with whom I have corresponded both from my posts and theirs. I just thought I'd mention another positive feature of PM News that goes unnoticed.

The government suggests that you have an outside specialist complete your security audit. An IT company that specializes in both cyber security and HIPAA and HITECH can ensure that your EHR is audit-proof.

YOU CAN'T MAKE THESE THINGS UP
RE: Walmart's Foot Scanner is Blind
From: Kevin A. Kirby, DPM

A 21 year old male patient of mine, whom I have been treating since the age of 9 for significant pes plano valgus deformity, came into the office yesterday to get a new pair of custom foot orthoses. He had outgrown his old pair of orthoses. Before he came to my office for a new pair of custom foot orthoses, however, he said that he decided that he first might try the foot orthoses that he saw being recommended by an automated "foot scanner" at his local Walmart store.

He said he took off his shoes and socks and stood on the Walmart foot scanner, realizing that his feet were so flat that his medial arches actually touched the scanner plate. What he next saw was the following message while standing barefoot on the Walmart "foot scanner": "Please take your shoes off before using the scanner."

He decided, after reading this message, that his money might be better spent seeing a podiatrist for his very flat feet, rather than getting his orthoses from Walmart. We both laughed about his story in the office yesterday, and I told him my colleagues around the country would probably also get a good chuckle out of this true story. Made my day!
