Cosmetic Foot Surgery Should Be Realistic: NY Podiatrist
Dr. William Spielfogel said that while there are some acceptable reasons that women would opt to get cosmetic surgery on their feet, including painful bunions or deformities, "A lot of people want to fit a square peg into a round hole, and it just doesn't work. Sometimes you have to be somewhat realistic," said Spielfogel.

Spielfogel is the head of podiatry at Lenox Hill Hospital in New York and is a published author of several scientific papers and chapters in medical textbooks. He co-chairs the New York State Podiatric Medical Association's Clinical Conference in New York City.

WI Podiatrist Discusses Management of Diabetic Foot Complications
Diabetic complications in the feet can be very dangerous; in fact, diabetes is the leading cause of lower-limb amputations that aren’t the result of a traumatic injury. "Fortunately, most of these complications and amputations can be prevented," says podiatrist Dr. Amy Jaeger. "With proper foot care and guidance from a podiatrist, you can manage the effects of the disease on your feet. Comprehensive foot care programs can reduce amputation rates by 45 to 85 percent."

Diabetic ulcers are often one of the first signs of complications from diabetes in the lower leg, says Jaeger. "These ulcers can stem from a small wound or cut on the foot that is slow to heal. Remember, too, that people with diabetes have a decreased immune response, which can make common foot infections more prevalent. If left untreated, ulcers can become harder to treat and can lead to amputation."

OH Podiatrist Gets Ready For Capitol Hill
Dr. Brad Wenstrup, a Cincinnati podiatrist who also serves as a Lt. Colonel in the U.S. Army Reserve, spent much of the post-election weeks trying to get set up for his new job as congressman. Wenstrup, 54, who was deployed to Iraq in April 2005 and served as chief of surgery at the 344th Combat Support Hospital at Abu Ghraib, can’t fully maintain his private practice now that he’ll be a member of Congress.

Instead, he’ll keep his certification afloat by practicing at Walter Reed Army Medical Center, near Washington, D.C. It's a service for which he won’t get paid, but will get retirement points as a reservist. “It’s a good chance for me to continue to see patients," he said.

TX Podiatrist Sentenced in Healthcare Fraud
A San Antonio podiatrist and one of his employees were sentenced Friday to five months of probation and restitution of $83,754 for trying to defraud Medicare and Medicaid out of more than $300,000. Dr. Bhavesh J. Shah and Andrea Christina Iturrieta, a biller who lives with Shah, each pleaded guilty in September to two federal healthcare fraud charges. The pair's plea deals said Shah, 40, admitted to investigators that from 2008 through last year, his clinics billed for diabetic therapeutic shoes and inserts that weren't provided.

Rising Patient Deductibles Spell Trouble for Doctors
Health plan deductibles are rising, and patient self-pay of deductibles is typically difficult for doctors to collect, meaning that increasingly more doctors may lose money on patient deductibles. This is according to athenahealth, a Watertown, Massachusetts, company that provides cloud-based services for medical groups.
"Benefit plans are shifting to include higher deductibles," says Tricia Andriolo-Bull, Vice President of Payer Strategy at athenahealth. "There's more cost-shifting in co-pays and co-insurance. The higher deductibles are a good way for employers to keep premium rates down. But any increase in self-pay is bad for the physician because it's hard to collect."

I have used Aldara for several years as an alternative in treating verruca. In Canada, Aldara is supplied in blister packs. We have our patients apply it daily under occlusion for about 3 months. For this, you will require 12 blister packets. To avoid wastage, we have the balance of the blister pack placed in a baggy so it will not dry up. In addition, we recommend oral Vitamin A as an adjunct to boost the immune system response.

I have had success with Aldara cream with patients who range from children to young adults around 30 years of age. The off-label use that I subscribe to is nightly application and cover with Band-aid before bedtime. Normally, the patient will follow this regimen for 8 weeks. In some cases, the verucca will resolve during this time period. I have had a couple of cases that did not resolve in the 8 week period. After stopping the medication, the warts were gone after 1-2 months. I have not had success in older adults with this medication.

RE: The Future of EMR Software Companies

For those who are not already completing their meaningful use attestation period, as 2012 comes to an end, so does the opportunity to acquire the full amount of incentive funds from CMS. The many EMR software companies can no longer incentivize doctors to purchase their software by pointing out the high amount of available funds, and thus the potential profitability of the companies will decrease.
The big question is which of the many software companies will survive and which will be acquired by others. Amazing Charts has been recently purchased by Pri-Med. The future of Practice Fusion is probably the next big question. Can they continue to economically survive as a “free software”, or will they be sold or become an IPO, similar to the path of other high-tech business models? And with the acquisition of a software company, there is generally a change in the pricing structure. Changing from one software to another is not usually easy. Getting all your old medical records back as .pdf files would also not be easy to deal with.

I am not a computer guru, but in general (because I've been 'burned' with operating systems that have been sub-par), I would wait until a new operating system has been out for a while before I purchase a new computer (of course, after that timeframe, it's no longer 'newer'). I went on-line and purchased a new computer that had Windows 7. The point of this is to check with your EHR vendor before getting a new computer to ensure compatibility with the EHR system you have and to wait and see how the newer operating system is functioning.

I believe this discussion started because most of our hearts were broken a week ago Friday. Pistols are not the problem. Weapons of mass destruction are the problem. In mere seconds, 20 children and 6 adults were slaughtered. No, the pistol-packing-Petes, for the most part, don't worry me. It's those who just can't wait to go Rambo and be a "hero" who worry me. The NRA seems to be more a terrorist organization, suggesting more violence in the face of too much already.

I am not shocked and dismayed at the tone of Dr. Allen Jacobs' response to my inquiry as to the standards and practices for compounding pharmacists. After all, he has growled at me in this forum before! In no way did I portray the practice as a "free for all" and I certainly did not indict any such compounds as witches brews.
I never mentioned the FDA, and never compared any compounded product to that of the "products or therapies endorsed by Dr. Markinson in his lectures." That, in my opinion, was a very cheap shot. In fact, my query, if anything, elevated...
