TX Podiatrist Uses Acoustic Energy to Treat Plantar Fasciitis
Plantar fasciitis occurs when a patient suffers a micro-tear in the tissue that runs along the bottom of the foot, causing inflammation and heel pain. “If you can’t walk on your heels, then you can’t really walk," said Dr. Gabriel Maislos, a doctor of podiatric medicine. To treat the pain, Maislos offers a series of treatments using a device called E-PAT, which stands for extra-corporeal pulse activation treatment. It sends acoustic energy waves into the inflamed area to shock it back into shape, taking it from a chronic condition to an acute one.

“Basically, it reminds your body that you have this problem and you need to start the healing process, because when something’s chronic, it’s almost as if your body doesn’t realize that it needs to be healed," said Maislos. Each treatment takes about 15 minutes. Most patients require about three treatments to get them back on their feet without pain. Each E-PAT treatment costs $200, and it is not covered by insurance. Some patients may feel a little sore after the first treatment, but that usually goes away within a day or two.

Dr. Siesel, OCPM Alumnus, is the Interim Chair of the Biomechanics and Orthopedics Department. Dr. Siesel has been teaching at OCPM for almost 20 years! Dr. Siesel is also a diplomat on the American Board of Podiatric Orthopedics and Primary Podiatric Medicine. She is involved in many community service events that include treating runners (with OCPM students) at local and regional marathons.

Dr. Belovich is the Chair of the Basic Sciences Department and has been teaching at OCPM for 17 years. Dr. Belovich is a member of the Council on Podiatric Education. She has contributed to many publications, including the chapter "Fractures in the Carlston Annis Shell Mound, Skeletal Population", in the book Archaeology of the Middle Green River Region, Kentucky.

Barefoot Running Not for Everyone: MD Podiatrist
Barefoot running has produced a legion of followers, although there are no significant studies on the positive or negative effects of the trend. “You definitely run differently,” said Dr. James Christina, director of Scientific Affairs of the American Podiatric Medical Association, because running barefoot changes the strike zone to the ball of the foot, and flexes tendons and strengthens muscles in a new way.

Chistina adds that those who are unable to walk for long distances barefoot probably aren't meant to run barefoot. Shoes for barefoot running — such as the Vibram Five Fingers — are designed to provide no support, only to protect the bottom of the feet from foreign objects on the ground.

Revise Unfair e-prescription Policy, Doctors Say
The American Medical Association and more than 100 other state and specialty medical societies are urging the Dept. of Health and Human Services to revise a Medicare e-prescribing policy that slaps doctors with a financial penalty in 2012 if they don't meet specified e-prescribing criteria during the first six months of 2011.
The medical organizations are dismayed with a new regulation from the Centers for Medicare & Medicaid Services that uses e-prescribing activity during the first six months of 2011 as the basis for imposing penalties in 2012. In addition, the government would look at the entire 2011 calendar year to determine the 2013 penalties physicians would pay.

Query: Cost Savings With Digital X-Ray System?
Our office has not gone with digital x-rays as of yet. I can appreciate the ease of x-ray storage and modulation of the image. Pretty nifty. But on a dollars and cents issue, I cannot understand how digital films are going to pay for themselves. Other than convenience, how will it increase our income? Please explain.

In view of the history given by Dr. Agostinelli, the esophageal mass must be diagnosed. If carcinoma, consider the Howell-Evans syndrome, which would fit the pattern of lower extremity skin lesions depicted.

Consider Howell-Evans syndrome. This is a form of keratosis plantaris palmaris hereitarium (KPPH) that has keratosis of palms and especially soles, plus carcinoma of the esophagus. Referral to a gastro MD is indicated. Then, if diagnosis is confirmed, local care with large doses of vitamin A plus debridement and topicals like salicylic plasters and 40 percent ureas may be of help.

Although not seen every day, please consider cutaneous T-cell lymphoma (mycosis fungoides). Sometimes mistaken for T. corporis or psoriasis. Early lesions may even respond to topical steroids, further confusing the picture. Advanced disease may present with most of the findings mentioned, including the keratoderma. Hematology/oncology referral would be my choice.

The presence of oral thrush and peri-anal plaque makes me suspicious of undiagnosed HIV/AIDS. An HIV test or T-cell count will give you the answer you need. While I have not seen these particular lesions in advanced AIDS patients (with thrush), I have seen a variety of skin lesion that were outside of the norm.
Please educate all of us and keep us updated when you discover the answer.

Just for informational purposes, please note that the late Sol Balkin, DPM (who practiced in Glendale, CA) used silicon injectables for lesions and replacement of fat pads. His work dates back to the 1960s and he was authorized to use this material, but was unable to get approval for this off-label use as he did not perform before and after immunological testing. His success rate was excellent and he published many papers on the subject, as well as having lectured and demonstrated his technique.
