NM Podiatrist Discusses Causes and Treatments of Bunions
“In my experience, a good share of bunions are formed due to the way your foot functions. If you overpronate or have a bit of an unstable walk, your muscles overwork and cause the deformity,” said Dr. Derek Harper, podiatrist with The Memorial Hospital.

“If symptoms are not too severe, my first approach is suggesting wider, more comfortable shoes and fitting orthotics. If pain persists and interferes with your daily life, then surgery is likely needed to bring relief,” Harper said.

IL Court Limits Talk Between Docs In Malpractice Case
A woman suing her former foot doctor over complications from a surgery was partially successful in keeping the defendant from talking about the case with her current doctor, who is part of the same medical group, thanks to an Illinois appellate court ruling Wednesday. A Patient sued podiatrist Dr. B. and his affiliate in 2009 after having a left toe amputated because of an infection that supposedly arose from a foot surgery Dr. B. performed in 2003. Last year, Dr. B’s defense team put forward fellow podiatrist Dr. K. as an expert witness in the case. Dr. K. is a controlling member of the foot center and thus has a stake in the case’s outcome, but also happens to be the Patient’s current doctor.
A lower Illinois judge in September 2015 granted Dr. B. and the foot center’s motion to communicate with Dr. K. about the case but the Patient appealed, claiming the communication is barred by patient-doctor privilege. In a split decision Wednesday in what it called a case of first impression, Illinois’ First District Court of Appeals found that the communication was allowed, but placed certain limits on it. Because Dr. K. was put forward as an expert witness, the Patient will be granted the opportunity to take his deposition before he’s allowed to speak with the attorneys for Dr. B. and the foot center about the extent of the patient’s injuries, the appeals court ruled. “By barring ex parte communications until after the injury deposition, we afford plaintiff the opportunity to secure Dr. K’s testimony on damages without coaching by defense counsel, and to have her privacy interests adequately protected without unnecessarily impinging upon Dr. K’s right to assistance of counsel for the corporate entity on the liability and causation issue,” the majority wrote.

| PRACTICE MANAGEMENT TIP OF THE DAY | |
4 Branding Tips for Medical Practices - Part 2
Your brand isn't just what you say your practice is about. Your brand is about your relationship with your patients, and the emotional response they have to your practice. You can influence it through branding efforts, but the patient ultimately decides his perception based on his own experience. Here are four ideas that can help you meet your goals for branding, while minimizing wasted time, money, and effort.
1A. Prioritize consistency:
Consistency in your communications is a time- and money-saver. Develop a logo and color scheme that relates to the image you're aiming to convey, and use it in all your materials: letterhead, website, brochures, and signage. Repeating your logo and color scheme is the shortest path to memorability (and shorter means less effort and money). Make a strong visual impression that conveys your branding goals, and combine it with an experience that matches.

| RESPONSES/COMMENTS (CLINICAL) - PART 1 | |
From: Simon Young, DPM, Dennis Shavelson, DPM
What about psoriasis?
Simon Young, DPM, NY, NY
When Dr. Chaskin writes, “This probably is a biomechanical fault caused by ill-fitting footgear or something else,” he is sadly revealing the vestigial nature of biomechanics in podiatry. Is something else a loss of subtalar neutral in one's feet? Is something else a flexible flatfoot deformity? Is something else an inhibited peroneus longus muscle engine? Is something else a poor neural strategy for managing afferent proprioception?
Podiatrists who have actually read Root, follow the biomechanical literature, and dispense orthotics by testing for activation of the 1st ray rocker and conformity to optimal casting position are in inverse proportion to those claiming competency in Ilizerov. For the foot sufferers of humanity, that is sad.
Dennis Shavelson, DPM, NY, NY

| RESPONSES/COMMENTS (CLINICAL) - PART 2 | |
From: Don Peacock, DPM, MS
Dr. Fellner, you are exactly right. This procedure has not been subjected to scientific evaluation. A modification of the Wilson has been performed MIS and it is in the literature. This particular modification has not been.
In order for us to truly back up what we are saying about this procedure, we are going to need to publish it. I would not go as far as to calling it a sound-bite because we do have some case presentations that have been published in non-peer-reviewed journal articles. However, from a purely academic standpoint...

| RESPONSES/COMMENTS (CLINICAL) - PART 3 | |
From: Dieter J Fellner, DPM
On the question of the shortening of the first metatarsal S/P osteotomy: this is really not an issue, and might often be a desirable effect. With an osteotomy to decompress and reduce the mechanical forces on the joint, this results in the 'buckling' and displacement of the 1st MTPJ.
There are two important caveats:
First, the sesamoid apparatus must be positioned in the correct anatomical alignment in relation to the first metatarsal head. There is very little tolerance to this rule, if a surgeon wants to optimize on outcome, a flexible...
