Sedentary Lifestyle Leads to Weakened Bones: Indian Podiatrist
|
|
Dr. Chaitanya Shah |
Studies have time and again found that leading a life of low-activity and not exercising daily can prove to be extremely harmful. Podiatrist Dr. Chaitanya Shah states, "The sedentary lifestyles that most of us lead these days lead to maximum harm to the bones. Along with the fact that we live in air-conditioned homes, travel in air-conditioned cars, and shop at air-conditioned malls, we are hardly ever exposed to the sunlight, and this worsens our case because we do not get vitamin D at all."

CT Podiatrist Discusses Children and Running
Robert F. Weiss, DPM says, "In the absence of long-term studies on children’s running, many debates have taken place. My thinking has always been that running is a sport that everyone can enjoy, if done at the individual’s own level of fitness and ability. There are, of course, overuse problems in children just as there are in adults. It is also important to remember that a child’s thermo-regulatory system is not as well formed or effective as an adult’s. Yet, they seem to have a greater psychological tolerance for heat and have shown to tolerate cold poorly. Therefore, care should be taken when running in extreme weather conditions."
|
|
Dr. Robert Weiss |
"The parent of a young runner should do what he or she can to keep running fun for the child. Try not to push. Let running be something the child chooses to do. Keep it in moderation, and allow the child a way out if he or she so chooses. The danger comes when the pressure to run is placed on him or her by enthusiastic parents, coaches, or peers. That is when the child will run even though it hurts," says Dr. Weiss.

Three Last Minute Tax Tips for 2014 - Part 2
While last minute holiday shopping is probably the main thing on your mind, it’s important that you don’t let your tax strategy fall to the wayside. According to Patrice C. Washington, the Money Maven of the Steve Harvey Morning Show, waiting too late could affect your ability to benefit from some time-sensitive tax advantages. Here are three tax tips you can tackle at any income bracket.
2. Move Up Deductions: If your income will be high this year, move up payment on as many deductible expenses as you can. Pay bills that are due in January in December. Just like with charitable deductions, if the proof of payment is in December, you can take advantage of this write-off for your 2014 tax bill. Ms. Washington explains that this strategy is popular with mortgage bills and prepaying estimated taxes before the end of the year. “There is, of course, a word of caution regarding this strategy,” says Ms. Washington. “If you think you will actually earn more money in 2015, and therefore be in a higher tax bracket, you might want to hold on to these tax deductions for the 2015 tax bill.”

Query: DMERC Questions
1) We called DMERC and were told to post the hours that our front desk staff is in office, not the hours that the doctor is in. Does that make sense?
2) What is the procedure to change the information in your DMERC file? Does this information have to match your Medicare information - i.e., hours of operation, suite number, etc.?

RE: Who Gets Our Money?
From: Jeffrey Kass, DPM
Who, if anyone, gets monies deducted from their income from sequestration? Do politicians get monies deducted from their salary? Are we having money deducted from our income because these bafoons in Congress cannot come to a compromise regarding the deficit? So, in other words, they are not doing their job and we suffer financially? Why is it that I am constantly reading about members of Congress getting raises? Why aren't their salaries frozen like ours are with the Medicare SGR?
Why is it that members of the medical community are allowing sequestration cuts, penalties for this and that...what other profession gets hit with monetary deductions in pay like this? The podiatric community should seriously consider hiring a social activist to help us, as we are too busy getting pre-authorizations to find the time to complain about these issues. New Yorkers don't like hearing or seeing Rev. Al Sharpton too much, but the bottom line is he is a voice that is listened to because he represents a minority people. The medical landscape is changing and we need a "Rev. Al Sharpton" type of activist to help support our causes.

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 2 | |
From: Stephen Peslar, BSc, DCh
I wholeheartedly agree with Elliot Udell, DPM. His comments and insights should be very seriously considered by podiatrists, wherever they are in practice. I read an interesting article about the chiropractic profession. From this site, I copied and pasted the following: "The podiatric medical profession focused on a particular set of problems for which allopathic medicine had little interest and a limited ability to deal with effectively, i.e., common foot disorders. A key occurrence in the development of the podiatric profession was when the AMA determined that medical physicians should not get involved with "minor" foot problems. This opened the door for podiatrists to flourish in their chosen area of specialty, and retain complete control of their scope of practice without fear of intrusion by organized medicine." [6]
6. Skipper JK, Hughes JE:Podiatry: A medical care specialty in quest of full professional status and recognition. Soc Sci Med 1982,17(20):1541-1548.
My question is: Have too many podiatrists decided that they only do bone surgery and biomechanics, and has this therefore opened the door for nurses to treat "minor" foot problems and for physiotherapists to treat DFUs?

| RESPONSES/COMMENTS (CODINGLINE CORNER) | |
From: Todd Lamster, DPM
These lesions, the small punctate epidermal hyperkeratoses, exist within the epidermis, and should be coded as nothing more than 700 or 701.1, in my opinion. If one looks up 757.39 in any ICD-9 book, or on CMS.gov, the definition is "other specified anomaly in the skin", under the section of CONGENITAL ANOMALIES OF THE INTEGUMENT. These lesions, which we see so often, are not congenital, so I have stopped using that code as I feel it is a misrepresentation of the pathology.
There is an article on this subject by Drs. Bakotic and Shavelson from Podiatry Management. It did not review how to code the lesions we see daily in the office. I would like our experts in podiatric dermatology to weigh in.
Todd Lamster, DPM, Phoenix, AZ

This just doesn't make sense. Much of the problem is due to the cutting of payments leading to reduced staffing. How does cutting payments even help more?
