SC Podiatrist Makes Christmas Story Come Alive for Children
 
Inside the Columbia Foot Clinic, there's more than the daily flow of patients. There's a man who loves Christmas, and some say he even resembles it. "He's like our personal Santa Claus and as you can tell, he even looks a lot like Santa,” Audrey Calhoun, lead receptionist at the Columbia Foot Clinic said. Dr. James Suber has been seeing patients for the last 47 years and he says he doesn't plan to retire anytime soon. "I'll only go if my mind goes or my body goes, but as long as I am okay mentally and physically, I'll try to be here,” Dr. Suber said.

Now in his eighties, Dr. Suber is trying to leave his footprint somewhere else, in the world of children's books. The 82-year-old doctor says he wrote a poem 20 years ago and hoped to turn it into a book. "I had to find someone to illustrate it and that took years,” Suber said. A patient of his, Vic Carrabotta, helped make that happen. Dr. Suber's dream has made its way into many homes already. It's available at Barnes and Noble and at the Columbia Foot Clinic. "We did donate some of them to Epworth Children's Home and of course they can distribute them or keep them there in their library and let their children read them,” Suber said.

TX Podiatrists Host Holiday Coat Drive 
 
The Podiatry Group of South Texas is holding a coat drive this month to help support the homeless in San Antonio, TX. Throughout December, The Podiatry Group will be taking coat donations at each of their 16 locations through December 31.

Once the coats have been collected, they will then be delivered to SAMMinistries, an interfaith ministry which helps the homeless. SAMMinistries was established in 1983 and offers shelter, housing and services to the homeless and their families in San Antonio, TX. They also offer volunteer opportunities for families, groups, and individuals. Through tailored programs, SAMMinistries strives to overcome homelessness.

Medicare Cuts Payments to 721 Hospitals With Highest Rates of Infections, Injuries
 
In its toughest crackdown yet on medical errors, the federal government is cutting payments to 721 hospitals for having high rates of infections and other patient injuries, records released Thursday show. One out of every seven hospitals in the nation will have their Medicare payments lowered by 1 percent over the fiscal year that began October 1 and continues through September 2015. The health law mandates the reductions for the quarter of hospitals that Medicare assessed as having the highest rates of "hospital-acquired conditions," or HACs. These conditions include infections from catheters, blood clots, bed sores, and other complications that are considered avoidable.
 
The penalties, which are estimated to total $373 million, are falling particularly hard on academic medical centers: Roughly half of them will be punished, according to a Kaiser Health News analysis. The new penalties are harsher than any prior government effort to reduce patient harm. Since 2008, Medicare has refused to pay hospitals for the cost of treating patients who suffer avoidable complications.

STARK UPDATE
CMS Sends Message That Stark Liability Is Limited to 4 Years With Self-Disclosure
 
“Stark liability does not exist forever. If you are outside the four-year reopening period, you probably have no liability to CMS,” says Bob Wade, who is with Krieg DeVault in Mishawaka, IN. Wade got the good news when CMS responded to several of his hospital-clients’ applications to the self-referral disclosure protocol (SRDP). CMS turned them down, but only because the hospitals didn’t have liability under the Stark law because the claims were deemed too old.
 
In letters to Wade, CMS explained that it “uses the time frame established under the re-opening regulations at 42 C.F.R. 405.980(b) as a guide to determine the time frame of the SRDP. As such, the time frame of the SRDP is limited to four years from the date that the disclosing party submits the disclosure to the SRDP, unless reliable evidence of fraud or similar fault exists….Because all of the non-compliance disclosed by [reporting entity] occurred outside of the four-year time frame under the SRDP, CMS is removing [reporting entity] from the SRDP.”

Three Last Minute Tax Tips for 2014 - Part 1
 
While last minute holiday shopping is probably the main thing on your mind, it’s important that you don’t let your tax strategy fall to the wayside. According to Patrice C. Washington, the Money Maven of the Steve Harvey Morning Show, waiting too late could affect your ability to benefit from some time-sensitive tax advantages. Here are three tax tips you can tackle at any income bracket.
 
1. Make More Donations: According to the IRS, you may deduct contributions in the year they are made. Even if it is December 31st, charge your charitable gift to a credit card or get your envelope postmarked. While you may not pay the credit card bill until 2015 or the check doesn’t clear until then, you are still covered. “Don’t forget that if your gift is over $250, you must obtain a written receipt or record of the transaction from a qualified charity,” says Ms. Washington. “If you plan on donating clothing or household goods, look forward to deducting the fair market value as long as everything is in good condition.”

Thanks, Dr.Costantino, for once again shining a light on who may be providing foot care to the next generation of patients. Many of our esteemed colleagues in higher  academia may believe that podiatry has a lock and key on foot care and we can treat the patient community the way we see fit. If we as a profession want to restrict future podiatric services to "complex foot surgery", many podiatrists believe that the public will simply have to suck it up because "we own foot care." Unfortunately, the opposite is true.
 
Podiatry evolved to serve the needs of the public and not the other way around. If we are foolish enough to not teach the next generation of podiatrists podiatric medicine, biomechanics, and all other facets of non-surgical podiatry, we as a profession will lose the privilege of serving the public, and as Dr. Costantino pointed out, there will always be another allied healthcare profession in the wings, waiting to take over.

In response to Dr Costantino's observation about foot care nurses and we as a profession relinquishing basic foot care, I can speak to this in a forward looking fashion. Where I practice, there are no podiatrists for 150 miles except me. I see less than 2% routine foot care for better or worse. Most people are very sheepish about asking me to cut their toenails, as I am seen as a specialist who presumably would not provide the service. Don't get me wrong, I'm not above it and I welcome the duty, as frankly it is relaxing and enjoyable, although menial.
 
The bulk of the routine care is taken up by chiropodists and much more by foot care nurses. If I were not to have such a wide territory and so many primary care referrals and patients, I am sure that I would be upset that I was not getting the routine care. The downside is that the nurses are requesting through training, the abilities to do wound care, biomechanics, etc. They are well funded and organized, well educated and have been in the system and know how to get things done. The outcome can be predicted as we see we lost our corner on orthotics.

Repetitive microtrauma (RMT) of shoes against toenails along with abnormal biomechanics that cause an increased dorsiflectory stiffness at the distal end of the distal hallux phalanx (that move or press against the nail causing onycholysis, fragmentation, surface scuffing and deformations) are undeniably the number one cause of primary onychodystrophy and secondary invasion, infection, and re-infection of opportunistic organisms like fungus and yeast in shoe-wearing societies. These classify as Type II and Type IV in The Shavelson-Bakotic Classification System of Onychodystrophy: Bakotic B, Shavelson D: The Pathogenesis of Nail Unit “Dystrophy”; Podiatry Management; Aug 2006: pp.149-155.
 
Generous shoe toe-boxes and foot orthotics that address flexible forefoot foot type pathology, along with maintenance topicals that are FDA-indicated for brittle or traumatized nails (such as Nuvail) must replace the “red herring” that primary onychomycosis is not the root cause of most ugly toenails. Lasers may not be the panacea.
 
Dr. Hartley is correct when she states that trauma is the root cause of toenail onychodystrophy and not onychomycosis, and that distal onycholysis precedes hyperkeritinization within the nail unit.
