NV Podiatrist Stresses Need for Foot Exams for Diabetics 
 
Although American Diabetes Month was in November, podiatrist Dr. Nicole Dalessandro continues to stress the importance of diabetic foot screenings. According to Dalessandro, diabetes is increasing in the country’s population, especially as obesity rates increase. “I’ve seen statistics that say between 3 to 5 percent of the population has diabetes,” Dalessandro said. “That’s a significant amount, and all of them should be getting foot screenings.”

During her examinations, Dalessandro looks for poor circulation or reflexes, loss of sensation, skin infections, calluses, and more. Early detection of these symptoms can prevent ulcers or fungal infections. “Like you go to the dentist twice a year, diabetic patients should be seeing a podiatrist,” Dalessandro said. “We want to practice preventative medicine and catch the problems before they occur.”

PA Podiatrist Marks 399th Publication
 
Dr. Arthur Helfand has had two segments published in the Third Edition of The Encyclopedia of Elder Care which has just been published by Springer Publishing Company. The segments include one titled "Foot Problems" and a second segment titled "Podiatric Medicine."

This marks the 399th publication by Helfand, with number 400 due in 2014. Helfand is Professor Emeritus at Temple University and is a past president of the American Podiatric Medical Association.

Query: Audit-Proofing Measure 15
 
Now that incentive monies have been paid for the past two years, the post-payment audits have started. Many of our EMRs can "confirm" that we have met many of the meaningful use core measures. Measure 15 (security audit) can be a difficult one to complete and have audit- proof later, especially if you are a solo practice. Does anyone have any tips or references to help?

When will Medicare carriers come out with their guidelines for appropriate covered codes and linkages?

Response: The actual LCDs with ICD-10 should be out sometime in April 2014. Prior to that, the various MACs will be testing the use of ICD-10 by providers at various dates. Each MAC (both local and regional DME) have posted their specific testing dates on their websites.

The days of expert-based medicine (EBM) where highly respected clinicians (who may or not be financially compromised) recount clinical anecdotes, good results from a series of cases and narrative reviews to determine treatment efficacy and diagnostic accuracy are waining in mainstream medicine. 
 
This is being replaced by evidence-based medicine (EBM) where decisions about patient care use valid and reliable data obtained from patient outcomes. We live in an increasingly data-driven environment.
 
The rate of change is occurring faster in medicine than in surgical subspecialties like podiatry. Given the cost of healthcare, there will be an increasing demand by society for accountability in clinical practice. Which EBM the profession embraces will determine its success in the future.

When my son is home from medical school, he often spends the day in my office or with me at the wound care center, and upwards of 90% of patients ask him if he is going to specialize in podiatry.

Neanderthal Toe Points to History of Incest
 
DNA extracted from the fossilised toe of a 50,000 year-old Siberian Neanderthal woman has revealed that she was the child of two closely related parents. The findings, published in the scientific journal Nature, also indicate incest was not uncommon among Neanderthals, modern humans' nearest extinct relatives.

After running the information through a series of simulated inbreeding scenarios, the researchers established her parents were either half-siblings who shared the same mother or double first cousins.
