Does anyone have experience using topical Aldara (imiquimod) for the treatment of plantar verrucae? It is a drug classified as an immune response modifier. Its application dosage is three times per week for up to 16 weeks.

RE: The Passing of Susan Gudeon
I want to thank everyone for your kind thoughts and prayers on the loss of my brave wife Susan, who finally lost her 28 month battle with pancreatic cancer yesterday. She's resting and at peace, always having appreciated the love and support you all gave to her. For those of you who have asked, here are the details of the arrangements:

RE: Use of Terbinafine With History of Chronic Hepatitis C (Edmond F. Mertzenich, DPM)

Pulse dosing terbinafine will not raise liver enzyme levels. Two tabs daily for one week and repeat for 3 consecutive months, then re-evaluate at 6 months to determine if additional dosing is necessary. I've used this method for the past 10 years without any side-effects or liver enzyme issues. If the patient goes to Walmart, he can get the medication (28 pills) for $4. Just make sure you're dealing with a dermatophyte, not spores or yeast.

As the primary doctors of evaluating human motion & biomechanics, we need to evaluate our patients accordingly, and with quantitative data. There are affordable systems available to use in our practices to quickly evaluate our treatments in the cases Dr. Kass is talking about. These systems can evaluate our patients barefoot, in shoes, in shoes with orthoses or braces - all in under 5 minutes. Evaluate, intervene, and re-evaluate.

RE: Displaced MBA Implant (Louis W. Nordeen, DPM)

As the MBA (or any other arthroereisis implant) doesn’t occupy or involve the STJ, why would you do an arthroscopic procedure there? What indication is there to violate a joint without radiographic indications of osteoarthritis or pathology? Did you mean to ‘scope’ the sinus tarsi? If so, wouldn’t that disrupt the very fibrotic tissue and remodeling of the area which would make the need for the implant moot? I’ve also taken out my fair share of these, done nothing else other than some physical therapy, if indicated, and they do fine. I’d have to see some stats on this to be convinced it is unnecessary.

RE: AOS Incompatibility problem with Windows 8 (Elliot Udell, DPM)

It's an AOS problem, period. They had months with the new Windows 8 (W8) code to update their program. That's their job. Microsoft does not have to modify Windows for every third-party program on the market. I would really push AOS, and I mean really push. Contact your state society, and post on other listservs.
Don't let up on AOS, and if they refuse to update their program, they may be in violation of the agreement. And, it was AOS's responsibility to check if AOS worked with W8 as soon as it was released to developers. AOS just decided to abandon you. It was reasonable to expect computers to die, and that AOS customers WOULD be replacing computers with ones with W8.

RE: Definition of Diabetic (Charles Morelli, DPM)

ADA guidelines for diagnosing:
Diabetes: fasting glucose of >126 or HgA1C of >6.5%
Prediabetes: fasting glucose of 100-125 or HgA1C of 5.7% to 6.4%
Normal: fasting glucose <99 or HgA1C of about 5%
Unfortunately, many patients are not told specific numbers by their doctor or do not want to hear that information when told. It is amazing how many patients deny it even though I can see the diagnosis listed in their medical record. There are always those patients who carry the diagnosis but do not consider themselves to have the disease, so but they do not take pills or insulin. For those patients, I explain that the “medicine” for them is better diet, weight loss, and exercise. This is when they usually state that their doctor has been telling them to do just that for years!
Can one become non-diabetic? Perhaps not, due to genetic predisposition. Certainly they can be well-controlled or in remission such as after gastric bypass.

RE: Efficacy of Compounded Drugs (Bryan C. Markinson, DPM)

In response to the query of Dr. Markinson, compounding is not a "free for all" in which arena any practitioner might conjure up a witches brew of their liking. Compounding pharmacies with which I am familiar (I have used products from 4 different compounding pharmacies) are all very much supervised by pharmacists and Pharm.Ds, with the requisite knowledge to understand what agents may be combined with what agents, what agents may not be combined, and so forth. I suggest that those responsible have a great deal more knowledge of potential interactions and other potential issues than Dr. Markinson.
Individual compounded products do not undergo an FDA approval process anymore than...
