NY Governor Signs Order Allowing Podiatrists to Administer COVID Vaccine
Governor Cuomo signed Executive Order 202.82 on December 13 which modified certain laws related to our profession and will remain in place through January 12, 2021. This action by the Governor will allow podiatrists to administer vaccinations for influenza and COVID-19 at Points of Dispensing that are overseen or approved by the New York State Department of Health or local health departments and supervised by physicians or certified nurse practitioners.

Podiatrists who wish to participate must receive training in: 1.) Infection control. 2.) Use of personal protective equipment. 3.) Certification in basic cardiopulmonary resuscitation. Effect of Toe Exercises and Toe Grip Strength on the Treatment of Metatarsalgia
The relationship of metatarsalgia and toe function is poorly understood. The researchers investigated the efficacy of toe exercises for the treatment of metatarsalgia. Forty-one (56 feet) metatarsalgia patients (mean age ± SD: 63.4 ± 10.6) underwent toe strength measurement. Pre- and post-treatment VAS score, AOFAS score, marble pickup, single-leg standing time (SLST), were recorded and compared in two subgroups to evaluate impact of disease duration on treatment outcome.
Post-treatment, toe plantarflexion strength improved (all p < 0.01); VAS scores decreased (p < 0.01); AOFAS scores, marble pickup, and SLST improved (all p < 0.01). Patients symptomatic for > 1 year had significantly lower changes in VAS scores (p < 0.01). Multivariate analysis showed patients with longer disease duration, and larger body mass index had significantly lower improvement in VAS scores (p = 0.029 and p = 0.036, respectively). Device consistency assessed by ICC was excellent (0.89–0.97). Conclusion: Toe function and metatarsalgia are improved by toe exercises, suggesting that they are closely related.

2. Seek Feedback
To be sure the reality of your brand matches up with your intentions, you'll need honest feedback. The good news is that it's easier than ever to get. Online reviews are a good starting point. Many practices discount the feedback offered online. However, patient reviews can reveal genuine issues that negatively impact your brand. This gives you an opportunity to fix them. Fixing these potential issues will help you stay on track if you're trying to build a brand that represents excellent service and accessibility.
You can also solicit feedback from your patients with surveys. And don't forget that other physicians who work with your practice (and their staffs) may have valuable input on how your services match up with what your brand promises.

RE: Podiatric Medicine in Health Promotion - Not Podiatric Health Promotion

It is time for members of the podiatric medical profession to begin to change the way they think about public health (and the way the profession is perceived by other health professions and the public). We do not or at least should not treat feet. We treat people who may have problems affecting the pedal extremity. This is not a play on words but a shift in how we participate as part of the army of health professionals. Without getting into a long discourse about what I mean, let me provide an example of an area of healthcare as well as health promotion that demonstrates the importance of podiatric medical practitioners in public health.
This example is the patient who visits the primary care physician (e.g., family practice physician or internist) or the cardiologist and is told that they need to exercise and/or should walk 20 minutes daily. Almost never (if ever) do these practitioners ask if the patient has painful feet. Have we in podiatric medicine ever studied how many people do not engage in daily exercise because of podiatric problems? Has anyone ever determined the effect of the condition of the pedal extremities on cardiovascular health?
How much of cardiovascular morbidity (or even mortality) is affected by the condition of the feet and ankles? How many deaths from cardiovascular disease could be prevented if these victims had the benefit of podiatric medical care? Remember, cardiovascular disease is the single most common cause of death in the Western world. Let us begin to think about our comprehensive role in health not just foot health.

RE: Squaric Acid for Warts

In March 2011, I published an article in Podiatry Management titled "Squaric Acid for Recalcitrant Warts". I recently had two patients who have suffered through many of our current "wart treatments" only to have their warts return. So, I am using squaric acid again. I applied a small amount on the forearm to sensitize the patient to the squaric acid, and have the patient return in a couple of days. If the area turned reddish, showing a skin irritation, then I dispense a concentrated squaric acid solution for the patient to apply twice a day for two weeks, then once a day for the next two weeks.
In the past, the wart changed colors, sometimes whitish, sometimes blackish, and after about a month seemed to fall off. I have not used this for many years because of all the new methods of wart removal. I was wondering if anyone else has tried using squaric acid for warts?

I received my vaccine on Friday and actually was given the opportunity to sign up on the first day for vaccination with the “high risk” medical personnel like ICU, ER, etc. I’m not a “staff” podiatrist at my hospital but I do take ER call and I’m frequently consulted for inpatient services. I don’t believe podiatrists were excluded or I was given preference at my hospital because I’m way too young to be senior in anything.

If you're on active staff of a hospital, they will inform you of your tier turn after injections have been provided to ER workers and other staff who work closely with COVID patients. Be sure that you are on the hospital email list by checking with the medical staff office. Your hospital’s Covid injection supply will also play a role in the timing. Otherwise, I would check closely with your own family physician.

The individual states have to decide what doctors should get the COVID-19 vaccine first. A lot will depend upon the supply of the vaccine. It should not strike us as being unusual that an ophthalmologist was "first on line." Eyecare professionals have a strong disadvantage in this pandemic. Their faces are right up against the faces of their patients which renders them very likely to be infected. The same would apply to dentists, ENT doctors, and GI specialists who do upper endoscopies. Even though podiatrists tend to work close to six feet from patients' noses, make no mistake, we are all at risk from the moment we turn the key in our doors and we should not wait a day to be vaccinated when the offer is given to us.
