PODIATRISTS IN THE NEWS - PART 1
IL Podiatrist's Gait Analysis of Wrongfully Convicted Man Leads to Release

Michael Nirenberg, DPM assisted with the exoneration of a man who was wrongfully convicted. In 2013 Darryl Williams was convicted of kidnapping and robbery in Queens County, New York, but he maintained his innocence. Appellate Advocates, who represent indigent individuals took on the case and asked Dr. Nirenberg to compare the gait of the kidnapper to Williams.

Dr. Michael Nirenberg

Nirenberg’s report showed Williams’ gait differed from that of the criminal, and with this information and other evidence, Judge Michelle Johnson of Queens County Supreme Court threw out the conviction and dismissed the charges against Williams. Williams is now a free man.

PODIATRISTS IN THE NEWS - PART 2
Epsom Salts Not Recommended for Wounds, Infections, or Burns: NY Podiatrist

Magnesium and sulfate provide a soothing effect on the body, says Bruce Pinker, DPM, a board-certified foot and ankle surgeon at Progressive Foot Care in White Plains, New York, who often recommends epsom salt soaks to patients. Not only can it help reduce feelings of soreness, pain, and inflammation, but it may also help to reduce draining of superficial wounds, such as a paper cut and ingrown toenails, says Dr. Pinker, who notes epsom salt should not be used to treat deep, open wounds, infections, or burns.

Dr. Bruce Pinker

While Dr. Pinker notes it’s safe to soak superficial wounds that are not draining, it should be avoided if there is an open, deep, infected wound, skin infection, or severe burn. Speak to your healthcare provider to determine if epsom salt is safe to treat any wound before trying it yourself.

PRACTICE MANAGEMENT TIP OF THE DAY
Five Trends for Health Systems in 2023

Adaptability and flexibility underscore nearly every major trend facing the healthcare industry in 2023.

4) New technology - Fortunately, the news isn’t all bad. With new technologies comes the promise of better treatment options for many patients. MRI and CT technologies, for example, now offer faster speeds and higher image resolutions than previous equipment. Also, the imaging equipment is more widely available than ever before. When a healthcare facility purchases a new MRI or CT machine, it need not replace the older machine, whose image resolution might be suitable for diagnosing less complicated health issues. The newer machines can be used for more complex diagnoses, and this additional imaging equipment can help expedite evaluating more patients.

Most innovations are more expensive than their predecessors. Against the backdrop of the critical labor shortage, legacy healthcare institutions can help offset declines in their labor force by investing in new technologies.

RESPONSES/COMMENTS (NON-CLINICAL)
RE: APMA's Letter About Ongoing Discord Between Our Certifying Boards
Rober Scott Steinberg, DPM

In response to the APMA's December 21, 2022 letter, "Concerns About Discord Between Our Boards," to the membership:

Dear APMA,

This has been going on for nearly 44 years. We are a tiny profession, yet some continue to dig their heels in and proclaim how much better they are than the rest of us. What this has done is open the door to a more reasonable approach by more reasonable colleagues who have not let ego interfere with what is right, fair, and just.

RESPONSES/COMMENTS (RESIDENCY PERESPECTIVES)
RE: Residency is Hard Work
From: Michael Felley, DPM

After being in residency for over a year, I could sum up my time and hopefully help future residents who are hoping to match in a single word- WORK. There is no other way around it. Residency is hard work. After clinic, cases, and education, there still is work to be done and we sometimes have to put off our recreational time until later.

The most important thing to remember when working these long days is to not feel sorry for yourself. We only have three years to be trained into the best doctors and surgeons. Three years is not a long time! When we feel like it’s too much or we feel like we’re being pushed too hard and that we need a break, is the time when the rest of the group has to pick up your slack. We all have a role to play.

Just remember the quickest way to grow is to be pushed and forced to stretch our capabilities. That is when we learn what we’re made of. Sometimes a day’s work will last into the late evening and we still have to study for a surgery the next day, or we have early morning clinic. It’s difficult to stay focused on the real reason we are there. No matter the reason, stay focused on the prize. Don’t play the victim - work for your goals.

When you feel like you’re at the end of your rope and you’ve had to work late several nights or your attending tried to reinforce something you should have already learned and you're overwhelmed, remember this quote by Australian activist, Christine Caine: “… you think you have been buried, but actually you have been planted.”

Michael Felley, DPM, Wycoff Heights Medical Center, Brooklyn, NY

RESPONSES/COMMENTS (CODINGLINE CORNER)
From: Jeffrey Kass, DPM

A PM News subscriber wrote in informing readership that they were being denied on claims from Aetna Medicare advantage when billing CPT 11721 with 11055 (with appropriate modifiers). Lori Stack was kind enough to inform readership she appeals denials with “the APMA appeal letter” and gets paid. If Ms. Stack appealed to Aetna Medicare Advantage and they paid her, then they are aware of the problem and should have fixed it so that the problem does not repeat itself.

I think a better solution would be that the next person that gets denied for the same problem start a class action lawsuit. The solution is not for every individual practice to send in appeals letters separately every time they get an individual denial. The solution is to fix the problem once and for all and to put an end to abusive payment practices performed by insurance companies. Each time a practice takes the time to send the appeal letter in, it is costing you money to do so and in the process reducing the payment one receives.
