NYCPM's Holiday Party Brings Joy to Hundreds of Harlem Children
On Wednesday, December 20, 2023, hundreds of children with their parents entered a Winter Wonderland of fun at the New York College of Podiatric Medicine (NYCPM). Kathy Reilly Fallon, DPM and her Charity-Heavenly Productions Foundation (HPF) donated 400 toys including Teddy bears, children’s books, pajamas, and toy cameras as well as candy canes, chocolates, Oreo cookie packs, and apple juice boxes.

The pediatric patients and children were greeted by NYCPM medical students and staff members Michael Trepal, DPM, Russell Volpe, DPM, Eunice Ramsey-Parker, DPM, and COO Mr. Joel Sturm. Children received candy canes, Oreo cookies, an apple juice box, a toy and a holiday treat bag from Stephanie Soliman, owner of Candie Cart while listening to holiday music. Children had photo opportunities with Frosty the Snowman, Reindeer, and Santa’s holiday helper. Children also participated in art projects with NYCPM medical students.

VA’s remote temperature monitoring program encourages Veterans to use the Podimetrics SmartMat in their podiatry care. The mat can detect diabetic foot ulcers or other foot disorders as early as five weeks before they appear.

“It’s much easier to treat foot ulcers in the early stages,” said Michael Pulla, DPM, chief of podiatry at Robley Rex VA in Louisville, Kentucky. “A late-stage foot ulcer or infection can lead to a prolonged healing time frame, possible hospitalization, increase in resources, and result in surgery or amputation. If we can identify a possible ulcer early, we can sometimes prevent these complications.”

Query: Tendon Resection
My question is regarding CPT coding for resection of exposed tendon. I have a patient with diabetes and a chronic ulceration. There is an exposed and devitalized anterior tibialis tendon that will need resection from its insertion and extensively within the expansive wound. I will be doing this in the operating room. What would be the appropriate CPT for this?

CPT 11043 debridement muscle, deep fascia, deep tissue layers, excluding bone.

RE: DME 2024 Fee Schedule

The DME 2024 fee schedule has been released. There is an approximate increase of 3% over the 2023 fee schedule for all DMEPOS categories. The DME fee schedule is not based on the same factors as the Medicare Physicians Fee Schedule and instead is based on the CPI and other economic indices that the Medicare Physicians Fee Schedule does not consider. CMS has offered their rationale for a smaller increase than last year because inflation is much less than last year and therefore a substantial increase is not warranted. Supply chain issues and other economic factors including raises to employees, however, mitigate most if not all of this increase. At least suppliers are also not facing the same potential fee schedule decrease as physicians. For more on the 2024 DME Fee Schedule Click Here.
Something new for 2024, with more information to follow is a new policy entitled the Lymphedema Act, which offers new coverage for the treatment of lymphedema. A whole new DMEPOS product category with many new HCPCS codes for lymphedema garments are effective January 1 2024. Look for more information on this during the first quarter of 2024.

Thank you, Dr. Tomczak, for reminding me of the mentorship of my classmate Dr. Allen Jacobs and my professor Dr. James Ganley. Both of whom, among others, contributed greatly to my practice of podiatric medicine and surgery. The medicine portion of my 47 year practice was as intellectually challenging for me as the surgical side and just as important to patient outcomes; actually, many times more important for the patient than a straight toe or a fused first TMJ. It was Jim Ganley who simply by example, demeanor, and intellect, taught us what it is to be a physician and surgeon. His emphasis on history and physical examination and the pearl that “diagnoses hide in plain sight” brings me to this discussion.

Three methods of taking impressions: plaster cast, foam boxes, e-scanning. I use a fourth, STS casting slipper socks. I got tired of waiting 5+ minutes for the plaster to set, so I switched to those things. They set in 60 seconds, you have to work fast but they are stronger than plaster and do the same job of capturing what you want.

During my education at ICPM, we were trained to cast and fabricate custom orthotics. I also was a lab technician at a custom orthotics lab, where we fabricated hundreds of orthotics a month. I carried this knowledge and ability into my private practice. But as my practice grew, I found it difficult to find the time necessary to continue being an orthotist and podiatrist. I then searched for a professional orthotic lab that catered only to podiatrists and fully followed my prescription requests. This satisfied my ability for more patient care time and treatment success.

This news bulletin from Becker's Payer Issues provides a very rationale reason for doctors to fear these replacement plans. Their advertising on TV and Radio during open enrollment should be even further restricted and is downright disgustingly misleading. The audience is told what they want to hear (how they can save money) but is never told at what cost.
The physicians and hospitals need to have the gumption to simply say NO! We are not going to take in anymore! If you have no respect for yourself, continue to take contracts which are 50% or less of Medicare. If not, then stand up and say no, you're not going to tolerate being abused and taken advantage of. If you don't stand up for yourself, who will?

| YOU CAN'T MAKE THESE THINGS UP | |
RE: Outrageous Case of the Day
A skinny little lady came into my office with her sister in early January 2023. She had a Jones fracture of her left 5th metatarsal. I put her into a CAM walker with instructions. Six months later, she returns with her sister, who states that her sister is non-compliant, and didn't wear the boot. The foot is swollen and painful and she wants to have it repaired. She also wanted me to put her into a regular cast post-op to stay on for as long as it takes to heal.
After I got this lady medically cleared and had her get psychiatric clearance for her bipolar disorder (so that I wouldn't get sued for lack of informed consent), surgery was scheduled. The surgery was uneventful and I put her into an old time cast with plaster and fiberglass, about 20 pounds worth. She was to come back to see me in 2 months. A couple of days before her return visit, her sister called to let us know her sister had died. Her sister told me she had died from multiple cancerous tumors, brain, lung, etc. She said, "She went peacefully and was cremated along with her cast." Case closed!
