FL Podiatrist Discusses Proper Disposal of Opioids
 
According to Robert G. Smith, DPM, MSc, RPh, "Flushing or dumping opioids down a drain or toilet is not the best way to dispose of this type of medication. Sewage treatment systems cannot remove all the medications from the water released into lakes, rivers, or oceans. Pharmaceuticals have been detected in the environment and there is genuine concern that this could be causing impacts to human health or to aquatic organisms. A vast array of pharmaceuticals including antibiotics, anticonvulsants, mood stabilizers, and sex hormones have been found in the drinking water supplies of at least 41 million Americans."

"Patients should be empowered by prescribing providers with information centered on the preferred options to dispose of unwanted or unused opioid pills, medicated liquids, or other medications by utilizing a local take-back or mail-back program, or medication drop box at a police station, DEA-authorized collection site, or pharmacy if the pharmacy has a secure drop-box program," says Dr. Smith.

NC Podiatrists Volunteer at Homeless Shelter
 
Students from East Carolina University’s Brody School of Medicine and College of Allied Health Sciences recently spent an evening providing complimentary foot exams and physical therapy services to homeless residents of Greenville. The effort was part of the annual In Your Shoes foot clinic organized and staffed by the students at the Crossroads Community Center, Greenville’s homeless shelter. In addition to the students, area podiatrists provided complimentary evaluations and treatments for residents.

“A lot of times with the homeless population, we see calluses, because they’re on their feet for extended hours of the day and they walk long distances. Other conditions include long, painful toenails, foot fungus, and actually a lot of abrasions because they often don’t have the proper shoes to protect their feet when they’re walking on harsh terrain,” said Dr. Brian Jones, a podiatrist from Family Foot and Ankle Care of Greenville, who volunteered at the event.

ON THE LECTURE CIRCUIT
CA Podiatrist First to Keynote Innovations in Wound Healing Symposium
 
David G. Armstrong, Professor of Surgery and Director of the Limb Preservation Program at Keck School of Medicine of University of Southern California (USC) became the first podiatric surgeon to keynote the 8th Innovations in Wound Healing Symposium. Armstrong’s lecture, entitled “The Diabetic Foot: A Silent Sinister Syndrome- and what to do about it”, reviewed data from more than a generation of research supporting a team approach in limb preservation.

It also highlighted steps to take to build centers and programs and lessons learned along the way. The symposium, held in Duck Key, Florida, welcomed physicians, surgeons, scientists, and industry leaders from around the world.

RELEVANT RESEARCH
Block Test Appears to Be Most Useful in Clinical Assessment of LLD: Systemic Review
 
A variety of assessments to determine leg length discrepancy (LLD) is used in clinical practice. Evidence about validity and reliability may differ. The block test appears to be the most useful clinical assessment to measure LLD, followed by using devices such as the pelvimeter. All other clinical tests seem to be not useful and may therefore be avoided by the clinicians and physiotherapists. Full-length standing anteroposterior radiography may be used as a global reference standard to measure the anatomic LLD when comparing clinical methods and imaging diagnostics. 
 
The alternative measurements, PACS and MRI, need to be further investigated. The findings of the review may help clinicians to select the most valid, reliable, and accurate clinical assessment to determine LLD in their patients and to make a decision, whether a compensation for LLD is indicated or not.

PRACTICE MANAGEMENT TIP OF THE DAY
2022 Medical Practice Asset Protection
 
Asset protection for doctors goes far beyond protecting personal assets from malpractice claims and must include both passive risk management and active defensive measures. Here’s a short list that includes both traditional business and medical practice risks and some of the newer, growing risks specific to current events like social and economic pressures, covid and the politicization of healthcare.
 
2. Insure against all predictable business risks, not just malpractice. After leadership and compliance, insurance is your first and most predictable line of defense. We’ve previously covered the idea that insurance is a system of layers that protect you against specific risks, like employment law exposures, data breaches, RAC audits and managerial liability and that no single policy can adequately protect you from. Make sure you understand the details of your coverage, and that your coverage is to the right (high) limits, not just low limit “riders” added to your med-mal policy. If you own the property in which your practice operates, it’s time to re-visit your coverage. It should be adjusted to reflect current restoration and materials costs and timeframes, which are likely significantly greater than when you bought the policy.

RE: Preparing Podiatric Medicine for its Future Role in Healthcare: A Half Century is Much Too Long

More than 50 years ago, while I was serving as Dean and Vice President of Podiatric Medical and Curricular Affairs of the then California College of Podiatric Medicine, efforts were under way requesting that CCPM students be able to take Part I of the National Board of Medical Examiners (NBME). A formal request was made through a letter that I sent on May 26, 1971, to John P. Hubbard, MD, President and Director of the NBME.
 
Dr. Hubbard responded June 7, 1971, with a brief, perhaps dismissive letter which stated, “We appreciate your interest in the possibility of using the examinations of the National Board of Medical Examiners for your students. I must advise you, however, that admission to the examinations of the National Board of Medical Examiners is limited to students or graduates of approved..."

The "limited license scenario" always seems to follow DPMs like a kind of shadow. It’s a throwback to the very old days when MDs thought that we would compete with them "without adequate training." Yet, if you ask resident directors (MD/DOs), they will tell you that their DPM residents are "excellent." 
 
When I first arrived in California and served as a volunteer for our earthquake emergency program at our over-55 community, I was warmly welcomed. One doctor, Harvey Slater, MD, who grew to become a close friend, announced without my asking him: "I had DPM residents for years at our hospital (where he was director of the burn unit), and they knew more than my other residents." 
 
Dr. Slater always welcomed them into his home during the High Holy Days, along with the other residents. I miss Harvey. He loved podiatry and made no secret of it. I remember his funny comment after I gave a classical piano music concert at our community auditorium: "Mike listened to his mother when she told him to practice."
