| PRACTICE MANAGEMENT TIP OF THE DAY | |
5 Ways to Implement AI in Your Practice
Given the growing awareness of how powerful and useful artificial intelligence (AI) can be, healthcare organizations have good reason to try to leverage its vast capabilities. But figuring out how exactly to use this cutting-edge technology to address podiatric care-specific needs is no small feat. As your practice implements AI, there are five key steps to take in order to make sure that technology simplifies this transition.
Step 4: Get your clinicians on board (and onboarded) effectively.
To implement AI successfully, you’ll need to take concrete steps to maximize its adoption among your organization’s primary care providers. Keep in mind: Clinicians can be skeptical of new technologies, and some might fear being replaced by AI. In addition, some of them might have concerns about data security.
Training is also critical in maximizing the adoption of AI within your practice. For example, providing clinical evidence for any conclusions reached by the AI engine can help address clinicians’ skepticism about its accuracy, so it’s important to make sure they know how to access all relevant documentation within your AI-powered system. When making plans for training, make sure to consider how tech-savvy your providers are.

| QUERIES (CODING & BILLING) | |
Query: Coding Tendon Lengthening with Subtalar Arthrodesis
Can both CPT 27686 and CPT 28725 be coded together when the tendon work was for tightened tendons and needed for better talar mobilization. The talar head was not reducing and there was still severe tightness at the peroneal tendons. Both the peroneus brevis and the peroneus longus were Z-lengthenings for later repair (at the conclusion of the case) inferior to the fibula. Next, the calcaneocuboid joint was opened and mobilized placing a pin distractor across this joint to aid in this mobilization.
There was still significant tightness and very limited ability to reduce the talar head, and the patient had a severe tightness to his Achilles tendon left in about 30 degrees of equinus when the reduction was nearly completed. The decision at this point was made for an Achilles tendon lengthening and a triple Hoke-style lengthening was performed with two of the cuts being towards the lateral side of the tendon to aid in correction of the valgus.

| RESPONSES/COMMENTS (PUBLISHED PODIATRISTS) | |
From: Joel Lang, DPM
It is admirable that Dr. Meltzer took the time and effort to write a memoir. While attending a memoir writing class, I was also motivated to do the same. My intended audience was not so much the profession or my colleagues, but for my family only. I was born in a somewhat orthodox Jewish family. My elders were not anxious or interested is sharing their history and experiences. As a result, I know little about those who came before me. I felt it was important for my successors to know about some of the history of their family and to give them information about their heritage.
After my parents died, I purchased a number of handheld recorders and tapes and gave them to my surviving aunts and uncles asking them to tell some stories of their ...

| RESPONSES/COMMENTS (NON-CLINICAL) -PART 1A | |
From: Jon Purdy, DPM
I agree with Dr. Kesselman that people are far too undisciplined to manage their long-term care. The government is certainly not capable of that either, as can be seen with insolvency of Medicare and Social Security. The government is not a safety net, it is a burden on society. The government does not invest the monies, they spend it. And, at the end of life they keep YOUR money! In addition, instead of returning your money, they charge you more, with a 20% co-pay, annual deductible, increasing denials, and no free medications. Both of these programs can be taken from you should the government decide. How is that working for everyone?
What I was referring to has far too many details to go into on this forum, but suffice it to say, this would be one mandate I would be in favor of. The mandate to deduct and invest the monies until retirement, at which point a person would have millions. The other benefits of this are passed on as family wealth, a huge spike in the market and economy, as well as more money to spend as you see fit in retirement.

| RESPONSES/COMMEN7S (NON-CLINICAL) -PART 1B | |
From: Joesph Borreggine, DPM
The assassin who needlessly shot the UHC CEO did so for what reason I do not personally know since I have not read his manifesto, but I can only surmise that it was based on corporate greed and corruption. I have not delved deeply into this that much, but from what the media has reported, he committed this heinous crime because he opined that a family member did not receive the care that they needed due to the denial of services by UHC. I believe that was the primary concern and the rooted disgruntlement of the previously mentioned corporate greed and corruption.
This individual made a judgment to make an example of the CEO so that others in the same line of business would be on notice that things need to change. I am sure they do, but this was not the way to do it. I understand that many applaud what he did and are supporting him with defense fund support along with touting their opinions on social media. They are sorely mistaken as far as...

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 2 | |
From: James DiResta, DPM, MPH
If there is one lesson to be learned from these recent blog entries, whether overtly stated or implied, and I might add from some of our most esteemed colleagues, it is the stupidity of suggesting that we ought to "stay in your lane". If podiatrists of my generation stayed in our lane, we would be nowhere. We have come this far because we were willing to buck the system and work to improve our profession beyond the instruction we received. I anticipated that those coming along behind me would expand our scope further and not be satisfied with the status quo.
If we fail to move this profession forward and expand our scope to practicing more general medicine, we will be extinct in a very short time. The walls are closing in on us. Why are we committing ourselves to being stuck in our lane? It is unthinkable that our 4-4-3 model of education has limited us to treating only the local manifestation of...

| RESPONSES/COMMENTS (PODIATRIC EDUCATION) | |
From: Bret M. Ribotsky, DPM
Allen, you clearly missed my point. If the profession guaranteed financial success (like Ortho, Derm, Cardiology, etc.), then the applicant pool that is choosing to become nurse practitioners or physician associates would be giving more thought into our profession. If many DPMs are earning less than half of what other professionals are earning - this issue needs to be dealt with. While money is not the root of success, it does give a stable foundation that would lower malpractice claims (additional stacked procedures), would reduce insurance fraud, and give everybody a greater sense of well-being, respect, and success.
