Don't Play Through the Pain of an Ankle Sprain: TN Podiatrist 
 
Starting a new fitness routine as part of your New Year’s resolutions? Protecting your feet is critical. “‘No pain, no gain’ may be a motto for most workouts, but ‘too much, too soon’ can lead to foot and ankle injuries, which can sabotage fitness goals,” says Jeffrey Loveland, DPM, a foot and ankle surgeon and Fellow Member of the American College of Foot and Ankle Surgeons.

Playing through an ankle sprain can cause further damage to tendons, which may take much longer to heal or possibly require surgery. What’s more, you may be overlooking a more serious injury. “Seek treatment for potentially-sprained ankles right away,” says Dr. Loveland. “Untreated or repeated ankle sprains may lead to chronic ankle instability, a condition that causes persistent pain and a ‘giving way’ of the ankle.”

On Day 25, Deshawn Radden had a hell of a time making fire. He was the first to get flame, nursing something small but strong. Then he began to self-sabotage, as his nervousness got the best of him, and he smothered his fire. But at the end, when it counted, he breathed life back into the flame, surging ahead to a close victory.

And while much of the Barry University podiatric medical student’s journey mirrors that up-and-down experience, unfortunately, it did not end in the same outcome. After an incredibly eventful time on the island, he ultimately fell short, still finishing in a respectable second place.

PRACTICE MANAGEMENT TIP OF THE DAY
2022 Medical Practice Asset Protection
 
Asset protection for doctors goes far beyond protecting personal assets from malpractice claims and must include both passive risk management and active defensive measures. Here’s a short list that includes both traditional business and medical practice risks and some of the newer, growing risks specific to current events like social and economic pressures, covid, and the politicization of healthcare.
 
1. Manage employment law risks. We’ve covered many aspects of this issue before, with discussions ranging from why you need employment manuals and specialty insurance to how to throw a safe office holiday party. Employment law conflicts are the leading cause of risk for all businesses, including medical practices, and current social, economic, and political pressures, along with social media, have magnified these risks. Covid is not over, and many practices will need to continue to manage ongoing issues of vaccine mandates, whether self-imposed or otherwise, mask usage by staff and patients, quarantine rules, and other related issues. This means having known and uniformly enforced policies in place on all these issues and professional guidance from employment law experts.

RE: Bilateral Plantar Pseudo-Kaposi Sarcoma Treated with Propranolol

Pseudo-Kaposi sarcoma (PKS), also known as acroangiodermatitis, is a benign cutaneous reactive angiomatosis with clinical and histopathologic characteristics resembling Kaposi sarcoma (KS). There are 2 variants, one the Mali type, associated with venous hypertension, and the Stewart-Bluefarb type, associated with an arteriovenous malformation or an acquired iatrogenic arteriovenous fistula in patients with chronic renal failure. A patient is described with bilateral plantar PKS treated with propranolol, a non-selective β-adrenergic blocker which improves vascular diseases, such as proliferating infantile hemangioma and other vascular diseases, owing to its vasoconstrictive and angiogenesis-inhibiting effects.
 
Treatment of PKS tends to be conservative. PKS is a rare disease among cutaneous reactive angiomatoses and is thought to be characterized by a vascular abnormality followed by histiocyte recruitment and endotheliocyte and pericyte hyperplasia. PKS typically presents with violaceous papules and plaques, which are usually located on the lower limbs. Source:  Takaaki, Mine, et al., JAAD Case Rep. 2021 Dec; 18: 74–78.

Before I retired from the military as a podiatrist over 30 years ago, I remember several military podiatrists using vitamin B12, a steroid, and a local anesthetic when injecting a neuroma in the fore foot. In my experience, I seemed to have better outcomes with these injections than those podiatrists who did not use B12 in their neuroma injections. When I was in training, many times I was reminded that practicing medicine is both an art and a science. I cannot see that vitamin B12 injected in the foot as being contraindicated unless the patient is allergic to it. Could be something for a podiatry student or resident to explore by using an accepted experimental design and approach.

First, early in my podiatric career, I wrote to the Florida Board of Podiatric Medicine and requested a declaratory statement centered on giving tetanus vaccines and boosters. I received a positive clarification validating my belief. My hope is that the readership will understand that the nationwide push for pharmacists to be allowed to vaccinate patients was orchestrated by both state and national pharmacy associations and endorsed by large chain pharmacy companies. Florida Medicare pays the pharmacy, not the individual pharmacist, $66.43 for the medication as well as $16.59 administration fee for a total of $83.02.
 
Prior to going to podiatric medical school as a pharmacist, I worked with the Florida state pharmacy board and state associations (FPA, FHSP) and at the time, U of Fla (my employer) to enhance the pharmacist’s midlevel role. I helped usher in...

RE: The Passing of Keith Kashuk, DPM

I was very sorry to hear about Keith passing away. We spent many days working together in the earlier years. We enjoyed each other’s company and learned from each other about things in podiatry that were needed - in particular, the education of residents. I will miss not ever seeing or speaking with him. He left many friends in the profession who will always be grateful to him. I also just want to echo words from Ira Baum and others in Florida who had the opportunity to know and work with him. My deepest condolences to his wife and family.

I only learned today of Keith's passing. I am so saddened. I have a lump in my throat writing this. I find myself recalling all the incredible moments I and many others shared with this great man for over 40 years.
