MI Podiatrist Appointed to Task Force on Physicians Assistants
 
Gov. Rick Snyder today announced the appointment of Vicki Anton Athens, DPM of Grosse Ile to the Michigan Task Force on Physician's Assistants. Dr. Athens is the president of Athens Podiatry Group, and has more than 30 years of experience in the practice of podiatric medicine and surgery. She previously served as chair of the division of podiatric medicine and surgery at Riverside Osteopathic Hospital, as well as podiatric residency trainer for the Oakwood Hospital System.

She has been the project director for pediatric and adult foot and ankle surgery for numerous mission trips. Athens holds a doctor of podiatric medicine degree from the William M. Scholl College of Podiatric Medicine. She will represent the Board of Podiatric Medicine and replaces Dr. Louis Geller.

NY Podiatrist Appointed Acting Chief of Surgery at VA Facility
 
Steven L Goldman, DPM, MBA has been appointed as the Acting Chief of the Surgery Department at the U.S. Department of Veterans Affairs – New York Harbor Healthcare System. He has been Chief of a Podiatry Service for over 16 years. Dr. Goldman will administer and manage a staff of over 100 full-time, part-time, and fee-basis employees in over 20 different surgical disciplines.

NY Podiatrist's Foundation Donates Supplies to Sandy Victims
 
Members of Armonk's Heavenly Productions Foundation recently visited schools in Far Rockaway that were affected by Hurricane Sandy to deliver 900 backpacks, toys, and, school supplies. Executive Board Members Lori Rocco, Dr. Kathy Reilly Fallon, and John Wallace joined adult volunteers from Bedford and Pleasantville and youth volunteers from Armonk, Bedford, and Pleasantville to visit Public Schools 106 and 197.

Rush for Podiatric Care Before Obamacare Kicks In
 
If you're having trouble getting an appointment with your doctor, you're not alone. Doctors' offices across central Florida are packed as patients try to get in before the new year. Foot doctors are running out of appointments. Dr. Gideon Lewis, a foot and ankle surgeon, has been in the operating room day and night.

He says several of his patients are getting their procedures done before the new year, because they are unsure how Obamacare will affect them. "I think right now everyone understands their insurance plans or at least knowing there's changes, they're getting more educated on what their plan offers; however, next year, there's such a big question mark, that they're trying to get as much done on a plan that they're familiar with," he said.

Query: Copying Non-Digital X-Rays
 
I have a patient who wants a copy of her x-rays. These are older x-rays taken prior to digitalizing our practice and we have no way to copy them nor can we find anyone in Memphis who can copy them. Does anyone know a source to have this done?

Query: Hospital Consult with Procedure
 
Can you bill CPT 99221 (or CPT 99222) - initial hospital care - with a "-25" modifier and a debridement procedure (e.g., CPT 11043-LT), and be paid on both the hospital consult and the procedure? Would we need an additional modifier on the CPT 11043 code?

Response: Coding for ulcer debridement is based upon aggregate size of all lesions debrided at that tissue level, regardless of where they are on the body. There is no "RT" or "LT" modifier usage anymore. So coding would be:  CPT 99221-25, CPT 11043 (no modifier).

Don't be in such a hurry to be this patient's Messiah. There isn't much in podiatry that is life-threatening and needs immediate surgery. Send her to an RA doctor to see if her disease is active. A rheumatologist can give her meds that you can't. If her feet really hurt her that much, you can give her shoes to accommodate the deformity. What are you going to tell her after you do a Hoffman-Clayton type procedure and her toes start drifting and her distal mets start showing a periosteal reaction?

AOFASs recent video for patients, “What is a Foot and Ankle Specialist,” is in response to a series of patient videos ACFAS posted on its websites this past summer that explain, in detail, the unique education, training, and certification of ACFAS members. The ACFAS videos may be found at two locations at ACFAS.org, plus on the ACFAS patient website, FootHealthFacts.org. On ACFAS.org, click on either “For Referring Physicians” on the home page, or at ACFAS.org/marketing. 
 
Interestingly, ACFAS created its series of videos in response to many AOFAS members who’ve posted grossly inaccurate videos about DPMs on YouTube or their own practice websites in the past. AOFAS also copied ACFAS several years ago when it created a patient website. If indeed “imitation is the sincerest form of flattery,” then we thank AOFAS for the compliments!

I really do not see this video as misinforming patients or disparaging podiatric surgeons. The fact is that an orthopedic foot and ankle surgeon is a medical doctor with at least 5 years of orthopedic residency; after that, a fellowship is accomplished in the foot and ankle. This video promotes this fact for their benefit. In the orthopedic surgical group in which I am presently practicing, we are now recruiting an orthopedic foot and ankle surgeon as well as another podiatric surgeon. We see these specialties as complementary.

I reviewed the AOFAS video, and it was pleasant enough for the 1:43 minutes.  I did not feel that it was derogatory towards DPMs. What I thought it reflected was that they are feeling a bit of "competitive heat" from well trained non-MDs. Otherwise, why would they go to the effort to even make the video? Also, I found it interesting that they chose a woman physician to be their spokesperson. Although medical schools are 50% female, orthopedic surgery, at least in my experience, is still a pretty male-dominated area. Just do what you do well and those videos are unimportant.

I purchased my diagnostic ultrasound machine from Biosound Esaote in 2005. The unit is an Aquila system type. I have been extremely pleased with this company. They offer a free instructional day to train you in the use of this modality. You can repeat this day as often as you feel necessary. I read with interest the article Dr. Kornfeld referenced concerning "Functional Medicine". There are many inflammatory states that are in fact sensitive to our diet. There is enough evidence that high sugar diets promote free radicals and premature cell death. In my opinion, everyone should be on anti-oxidants, either through the food that they consume or through supplements. In a perfect world, everyone would eat appropriately, exercise regularly, and obesity and metabolic syndrome would be a memory. Unfortunately, in the real world, this will never happen. 
 
Diabetes, hypertension, kidney disease, and all the other complications of metabolic syndrome are turning our population into an...
