Digital Tenotomy Can Prevent Many Diabetic Infections: MI Podiatrist
According to Conway McLean, DPM, "Lower extremity amputations are a tremendous source of illness, disability, and death for Americans living with diabetes. One of the most common sites for these problems to develop is at the tips of the toes. The digits are susceptible to changes in their positioning over the years, becoming angled up or down, even over adjacent toes."

"A surgical procedure can be used to straighten the toes. This can be a risky venture, taxing the individual’s blood supply, which is often inadequate. Many diabetics are leery of having this kind of correction performed, entailing increased risks of problems with healing (more common in diabetics), risks of infection, and prolonged immobility. Under-utilized is the simple release of a toe tendon.This can be achieved without an incision, a simple procedure, performed in the treatment room, with a needle stick. This can provide enough straightening of the digit to ease the pressure. Not all hammertoes can be corrected this way. But this simple one-minute procedure can sufficiently alter the inexorable process of skin death, infection, hospitalization, and amputation," says Dr. McLean.

IN Podiatrist Collects Over 500 Pairs of Winter Boots for Ukrainians
Boots on the Ground was created at the end of October by Valparaiso podiatrist Paul Sommer, DPM. After collaborating with businesses across Northwest Indiana, the organization successfully collected over 500 pairs of gently used and new winter boots that will be shipped to Ukraine.

“It was surprising. I didn’t expect this much of a response,” said Dr. Sommer, who has been a podiatrist for 30 years. “I was amazed that people went out and bought boots.”

Family Foot & Leg Center Welcomes FL Podiatrist
Karan Malani, DPM recently joined Family Foot & Leg Center (FFLC), and will be seeing patients at the FFLC Naples office. Dr. Malani attended the Temple University School of Podiatric Medicine where he graduated with honors. He later went on to complete his podiatric residency at the Harvard Cambridge Health Alliance.

Five Trends for Health Systems in 2023
Adaptability and flexibility underscore nearly every major trend facing the healthcare industry in 2023.
1) Staffing challenges - According to one recent poll of healthcare organization employees, staffing was the top challenge for 58% of respondents, even ahead of expenses and revenues. Around the country, human resources departments are working overtime to retain the staff that remained through the two-year COVID-19 pandemic period in which 20% of healthcare employees reportedly left the workforce. Healthcare isn’t alone facing staff shortages, but the 24/7/365 need for medical care only allows for so much flexibility. Losing workers to competing jobs with the ability to work remotely for higher pay presents healthcare employers with a real quandary.
Emphasizing flexibility wherever possible is not a catch-all solution, though it will be critical to minimizing the outflow of workers. Many healthcare jobs can be tailored for remote work, along with avoiding night and weekend shifts week after week. Allowing employees to work from home reduces their gasoline expenses, saves them time spent in traffic, and in some cases might allow them to save on childcare. The more flexible a practice can be in providing remote work, the greater their success.

Query: Refund of Co-Pays
As patient co-pays increase and physician fee reimbursements decrease, are we obligated to refund a patient copay if the physician fee reimbursed is lower than the co-pay? For example: the patient co-pay on the insurance card is $75 and the approved physician fee is $45 for the office visit. Must we reimburse the patient the $30 difference?

Response: Absolutely! If the patient pays more than the insurance says they owe, then you have to refund the balance. It doesn’t matter why they overpaid, you must refund the overpayment. By contract, you can only bill the patient the balance the insurance says they owe.

Query: MIPS Exception for 2022
It looks like CMS is offering a COVID-19 MIPS Exception for 2022 as they have done the two previous years. This will need to be filed by December 31, 2022. Can anyone provide a good resource with specific directions for filing the exception?

I don’t know Dr. Kilfoil and I never clicked on any of his videos that he uploads here until today. I’m not sure why he posts them here as they seem to be directed to the layman vs. the professional but nonetheless I enjoyed watching it.
I would like to compliment the doctor on his enthusiasm for which he loves his work and takes care of his patient. I did not like the fact that he referred to himself as “just a dumb podiatrist” as I think that is a disservice to himself and the profession.
I once obtained an autograph from Henry Winkler otherwise known as “the Fonz” and on it he wrote “self-respect is joy”. Dr. Kilfoil, you appear to be an excellent podiatrist, far from a “dumb podiatrist.”
