AL Podiatrists Save Patient's Toe
When Alabama State University Director of University Relations Kenneth Mullinax was told that his big toe on his left foot may need to be amputated, he went straight to the University of Alabama at Birmingham to see Brent Haverstock, DPM, podiatry director of UAB Medicine’s Advanced Limb Preservation Program.

At the appointment, Haverstock admitted Mullinax to UAB Hospital, where he received IV antibiotics over the course of three days. Dr. Haverstock and Kenneth Unger, DPM, assistant professor in the UAB Division of Vascular Surgery and Endovascular Therapy, oversaw Mullinax’s treatment. Their efforts were successful, the infection cleared, and the big toe was saved.

Query: Covering Post-op Patients
I have inherited some post-op patients from a colleague who recently had to go on medical leave. His patients are still in the acute post-operative period and well within their global period from the procedures. How do you recommend I bill these visits?
1) As a new patient encounter using a -55 modifier? I am not sure if it matters, but the surgeon, from my knowledge, did not bill his surgical codes with a -54 modifier (surgical care only). 2) A new patient without a modifier and document reason for visit? 3) Just bill for post-op visit? Any advice would greatly be appreciated!

Response: 1) You should only bill using the -55 modifier if the surgeon billed the -54 modifier 2) The global period only applies to the surgeon or his/her group. If you are not affiliated with this other doctor, then the global period does not apply to you. It is a new patient to you and you should bill for the services you rendered. I would document that you are seeing this patient due to Dr. X’s illness.
Was there any formal agreement between the two of you? There is locum tenens and reciprocal billing options depending on the situation. I think that there are not that many patients to get into that arrangement.

RE: Where Science and Evidence-Based Medicine Don't Necessarily Hold True

A middle-aged female patient presented with heel pain. She has a long history of plantar fasciitis and she just moved to my geographic area. After all tests and examinations were done, I offered her an injection of cortisone. She said that the past doctor who treated her successfully only injected vitamin B12 and it helped. So, being a good doctor, I ordered blood tests and after I saw the results, assured the patient that her vitamin B12 levels were well within normal range. I gave her a series of two injections of dexamethasone and lidocaine. No relief. She once again insisted that I give her the vit B12. Begrudgingly, I did.
All of her symptoms abated. No pain on palpation. The patient is smiling. Was it a placebo effect or was it the alcohol in the cyanocobolamin injection? Go figure.

I am replying to another physician's question about which topical antifungal cream to use that won't be denied by insurance. I usually use clotrimazole cream or miconazole cream and it's not denied by any insurance. Miconazole cream usually seems to work better than clotrimazole.

RE: The Passing of Keith Kashuk, DPM
I also had the pleasure of completing my residency with Dr. Keith Kashuk. I was one of his last residents and took over his practice when he retired after 42 years. He was my mentor. Keith was the valedictorian of his class. He was truly the most skilled podiatric surgeon I have ever seen. It was said he had hands of silk. Dr. Kashuk expected 110 % every day and challenged you to perform at your best all the time. He expected you to be academically prepared and to give no excuses just solutions.
He has taught scores of residents over the years and made them excellent compassionate podiatric physicians and skilled surgeons. Our profession was greatly elevated with his energy, demeanor, and professionalism. I strive to continue his legacy of teaching young, promising residents and advancing our profession. He will be missed.
