VA Podiatric Joins Peninsula Foot & Ankle Specialists
 
Dr. Mirsada Hasanovic recently became an associate at Peninsula Foot & Ankle Specialists. She joins Drs. Arnold Beresh and Jeremiah Bushmaker. She said that she has long been interested in healthcare. She chose to specialize in podiatry care, Hasanovic said, treating many people with foot problems associated with diabetes—a growing problem in this country. She said she wants to help people protect their feet and not have to face amputation.

Hasanovic performs routine foot exams and minor surgical procedures in the office. She has surgery privileges at Riverside Regional Medical Center and Mary Immaculate Hospital in Newport News and at Sentara Careplex Hospital in Hampton, as well as at their affiliated ambulatory surgery centers. Hasanovic completed her three-year residency in podiatric forefoot and rearfoot reconstructive surgery in July at Heritage Valley Health Systems in Beaver, PA.

NY Doctor Has Long Commute to New Practice
 
There’s a new doctor in the village of Millbrook, and she probably makes the longest commuting trip by car every day to practice podiatry medicine and surgery with David Stamm, DPM. That’s because Fiona Connolly, DPM, has lived on Long Island since 1991 and has no immediate intention of moving to the Dutchess County area. “It’s not bad at all,” she said. “I’m always going against traffic both ways. The commuters are driving into the city in the morning and out of the city at night. I’m always going the other way.”

Dr. Connolly was born and brought up in Northern Ireland. After graduating from Queens University in Belfast, she began working in London and dated a man from Long Island. They married in Northern Ireland and moved to Long Island in 1991. She then attended the New York College of Podiatric Medicine, graduating as valedictorian in 1995. She is now dual board certified in podiatry medicine and surgery and was associate director of New York Hospital Queens.

Providers Balk as CMS Announces Penalties for 2013 EHR Issues
 
A CMS announcement that many doctors will see their 2015 Medicare payments cut by 1% for failing to meet federal electronic health-record incentive-payment program standards has provoked strong reactions from physician and hospital groups. “The American Medical Association is appalled by the news,” said AMA President-Elect Dr. Steven Stack, who noted that “more than 50% of eligible professionals will face penalties … a number that is even worse than we anticipated.”
 
The cuts will target more than a quarter million physicians and other eligible professionals. The penalties will apply next year to providers who failed to meet the program's Stage 1 meaningful-use criteria in 2013. Letters informing affected providers will be mailed in the coming weeks, a CMS official said. Instructions on how to appeal the CMS' penalty decision is available on the CMS website. Appeals must be filed by Feb. 28, 2015.

Query:  Effect of Non-Renewal of Staff Privileges

I just received a letter from a hospital and their affiliated surgical center. I have been on staff at the hospital/surgical center for over 10 years. According to the letter, the hospital bylaw change that occurred this year states the following: "A physician must have 20 patient contacts in a 24 month period and if that does not occur in that time period, then the physician will no longer have their privileges renewed at the next renewal cycle or they may volunteer to relinquish their privileges in writing prior to that renewal".

Since I am not in compliance with this hospital bylaw, then  I guess I must relinquish my surgical and admitting privileges either voluntarily or allow them to just expire? Since this is a hospital bylaw, I am certain there is no other recourse than to relinquish my privileges? Therefore, I am concerned what effect, if any, this will have on other hospital staff privileges that will be up for renewal in the future. I ask this because there is always the question that is asked on insurance, hospital, and malpractice renewal applications: "Have you (the physician) ever voluntarily elected to leave a hospital staff or not renew their privileges?"

Query: Suture Removal Coding
 
Is there a specific code for suture removal? This is not for post-op surgery, but for the patient who presents for removal of sutures after a laceration.

Response: CPT 15851 - removal of sutures under anesthesia (other than local), other surgeon - is the only CPT code available. You did not say if you placed the sutures yourself or if there was a need for general anesthesia to remove them. If you did suture the site, you might have very well performed one of the following: 
 
CPT 1200x - repair of simple wound(s); 
CPT 12041 - CPT 12047 - repair of intermediate wounds; or 
CPT 13131 - CPT 13133 - repair of complex wounds. 
 
All of these codes have a 10-day Medicare global. If you placed them yourself and are taking the sutures out within 10-days following their placement (on or before the 11th post-operative day), then the suture removal is part of the global surgical service. If someone else placed the sutures or it is beyond the global period. then you would need to bill the service as an E/M code following the E/M guidelines. If it were me and I placed the sutures, and they needed to come out beyond the global period, I would not charge the patient for just that simple service. From an ICD-9 coding standpoint, you would code V58.32 - encounter for removal of sutures.

V00.01XD: Pedestrian on foot injured in collision with roller-skater, subsequent encounter.
 
Is there a code for two roller skaters colliding?
 
First, are roller skates even still a thing anymore? I mean, other than how one knows spring has sprung in Central Park? Second, can you call a person on roller skates a pedestrian? Thirdly, if the answers to one and two are "yes," then these things should be outlawed, because they are obviously dangerous.
