Ankle Replacement Can Change Lives: CA Podiatrist
Technological advances in ankle replacement are ushering in a new world of pain-free movement for people suffering from debilitating ankle arthritis. While once severely restricted in their activity, those who undergo ankle replacement can enjoy a complete turnaround and actively participate in life again.
|
Dr. Shannon Rush |
"With contemporary arthroplasty, ankle arthritis sufferers who thought their situation was hopeless can get a new lease on life," says Shannon Rush, DPM, FACFAS, a Fellow Member of the American College of Foot and Ankle Surgeons and surgeon at Palo Alto Medical Foundation, in Mountain View, CA. "For years they were told they'd always be in pain, but ankle replacement has totally changed their lives--they can now walk with ease, stop going to the pain clinic, and stop taking powerful opioid medications."

AZ Podiatrist Kicks Off Sock Tree Program
The Christmas tree in Dr. Stanton Cohen's office is decorated a little differently. Instead of only twinkling lights, ornaments and candy canes, an array of colorful socks hang on the tree branches. “They look like candle drippings,” Cohen noted. This could only mean one thing – the Yuma podiatrist’s annual collection of socks is in full swing.

For more than 20 years in December, Cohen asks the community to bring new socks to his office to help decorate his sock tree. Just before Christmas, the socks are donated to Crossroads Mission, which assists the homeless. “There’s a real need here. People donate clothes, shoes, used appliances,” but socks typically wear out and are thrown away, not donated, Cohen explained.

CA Podiatrist Discusses How Doctors Should Prepare for Obamacare
Healthcare reform will bring about sweeping changes for physicians in 2014, when provisions in the Affordable Care Act – from insuring all Americans to rewarding providers for better care – go into effect. How physicians manage these changes will determine whether they can continue to be successful in the field. U.S. News turned to experts to examine these changes and to help doctors prepare. Michael Nusimow, CEO and co–founder of drchrono.com, an iPad–based electronic health record system, and Dr. Bill Metaxas, a podiatrist who practices in San Francisco, offered insight and say doctors can expect three main changes:
|
Dr. Bill Metaxas |
A shift from private practice to medical networks, a full integration of electronic health records, and changes in the healthcare payment model. Prepare for a shift from private practice to medical networks. Prepare for a transformation to electronic medical records. Prepare to change the way you collect payments from patients

NHS Podiatry Contract to be Reviewed in UK
Patients with foot problems in the Stoke area are set to benefit from a review of the National Health Service (NHS) contract responsible for this treatment. Patients are worried about the length of time that it takes for them to see a specialist who can deal with their foot problems. According to the current contract, 95 per cent of patients should be seen by a podiatrist within three weeks of being referred by their GP. However, figures released in October showed that the NHS trust was only reaching 70 per cent of patients. Seventy-nine people had to wait over ten weeks while three were left without an appointment for around four months.
Podiatry services are an essential tool within the NHS, helping patients deal with everything from a common verruca to infected ingrown toenails to gout. Ensuring that people are seen as soon as possible means that the ailments can be diagnosed right away, and then treatment can begin after that.

QUERIES (CLINICAL) |
Query: Surgery for Patient with Possible RA Foot Deformity
A 38 year old female patient presented with moderate/severe pain with quickly advancing foot deformity. On initial exam, I suspected RA due to the severe foot deformity and erosions at the met head with deviation of toes in the lateral direction. The patient relates to a history of RA in her family. I have set her up with a rheumatology consult but that is months away. She wants both feet surgically fixed at the same time, which I think may be a challenging recovery for her.
|
Deformities in Female with Possible RA |
I have dealt with many older patients with the typical RA foot, and usually fuse the first MPJ and met head resections of the lesser MPJs. I have not dealt with a younger patient with this type of foot deformity and am looking for recommendations from other docs who may have had similar surgical experiences.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A |
From: Mark Pfeifer, DPM
I have been a member in good standing with the Walworth County Medical Society (which is a part of the Wisconsin State Medical Society) for well over 15 years now. I have been the acting treasurer for this group for the past two years and was asked to consider the presidency position, but the bylaws allow for only MD/DOs as being eligible for nomination.
Most members did not have a problem with a DPM filling the position, but it will take time to change others' minds, both at the county and state levels. I can remember a time when a DO was objected to in MD circles! All I can advise is take LITTLE STEPS, one at a time!

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B |
From: Stanton Cohen, DPM
The podiatrists in Yuma county AZ have been members of the county medical association for years. I have personally served in several elected offices, including VP. I could not be president because our president sits on the AZMA Board of Directors, which requires membership in AZMA and the AMA. They were not ready to accept DPMs in the state association, let alone the AMA.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2 |
From: Simon Young, DPM
Multiplan did you a favor. In my experience, Multiplan and Beech St. fee schedules for DPM services are extremely low, even meager. In my experience, Aetna uses Multiplan and Beech St.'s fee schedules rather than paying the out-of-network fees submitted by practitioners who participate in Multiplan or Beech St (another poor payer for podiatric services). This tactic allows them to utilize HMO fee schedules to discount the "out-of-network" fees you submit.
From my discussions with many insurance representatives, oftentimes the MDs are paid the STANDARD FEE schedule, and DPMs the AFFILIATE FEE schedule. It doesn't take a rocket scientist to know which is the "richer" fee schedule.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 3 |
From: Ron Raducanu, DPM
I firmly believe we are all interested in being the best doctors we can be. Please provide any clinical study data to support your position; otherwise, I'm not so sure we can strive to do what you think we should be doing to be "the best". I'm also not sure your recommendation of "open mindedness" can qualify in this situation as I'm not sure how I can incorporate what this article suggests and its benefits within the scope of clinical practice (in PA, at least). I do personally believe that by enhancing the foot's mechanics, we alter other mechanically-induced issues (back pain, sciatica, etc), but again, I'm not sure how to implement what this article suggests.

YOU CAN'T MAKE THESE THINGS UP |
RE: Here's Why AOFAS Claims They are Superior Foot & Ankle Specialists
From: Alex Tievsky, DPM
This video from AOFAS explains why an orthopedic foot and ankle surgeon is the "right" choice for patients. They don't directly say that they're superior to podiatrists, but they do poke at the fact that they have a medical degree and that they understand patients' conditions like diabetes and other medical problems better than the competition. This video seems to be grossly misinforming patients. The last time I checked, an orthopedic surgeon doesn't really spend time diagnosing neuropathy or PVD or anything outside of the muskuloskeletal system. I just wanted to see what other people thought about this and maybe how ACFAS can counter a video like this with a video that sheds some positive light on podiatric surgeons.
