NY/NJ Podiatrist Honored for Heroism

He's known around the Pascack Valley as "Bill the Superhero," but Montvale resident Dr. William Greco isn't one to boast his prowess. "It's embarrassing at times," he admits. On Aug. 11, Greco, a member of the Tri-Boro Volunteer Ambulance Corps rescued an intoxicated New York woman from being hit by a train that nearly killed them both.

The woman accidently lodged her 2005 Chrysler PT Cruiser onto the train tracks at the Montvale Train Station at around 3:30 p.m. that afternoon. Greco jumped over the fence of the station, pulling a hamstring. When he arrived to her aid, he "dragged" the woman by her belt and arm while losing his shoes in the process.

INTERNATIONAL PODIATRISTS IN THE NEWS

Barbados Podiatrist Introduces State-of-The-Art Biomechanics Lab

According to podiatrist Dr. Karen Padmore, a state-of-the-art laboratory for producing custom-made foot orthotics (CMO) is in the works for the Queen Elizabeth Hospital (QEH). The introduction of the foot scanning system at last Friday’s diabetic orthotics program at the QEH was only the beginning. Dr. Padmore revealed that the program also made provisions for the transfer of technology, with the setting up of such a laboratory at the hospital.

Dr. Karen Padmore

Dr. Padmore outlined that CMO made in a modern, technologically advanced laboratory were becoming more and more a fundamental treatment in the fight against diabetes. In addition, she noted that up-to-date biomedical foot assessment equipment, such as foot scanning, was essential since the custom-made devices can only be as good as the accuracy of the information collected.

HEALTHCARE and POLITICS

White House Calls for Healthcare Cuts, Permanent SGR Fix

The White House has called for a permanent—not temporary—fix to Medicare's sustainable growth-rate formula and about $400 billion in healthcare cuts, according to a source familiar with the fiscal-cliff negotiations. House Speaker John Boehner (R-OH), however, was generally dismissive of the president's new proposal, signaling that a permanent fix to the physician payment system is still not within reach.

The GOP leadership plans to introduce a bill this week that would extend current tax rates for Americans who make $1 million or less, but Boehner stopped short of saying whether that legislation would include a temporary fix to Medicare's physician payment formula.

PRACTICE MANAGEMENT TIP OF THE DAY

Become a Better Public Speaker - Join Toastmasters

Even though communication is a key to success in all areas of one’s professional life, a small percentage of us have ever received any formal training. If you want to increase your self-confidence, become a better speaker and leader, and communicate more effectively to increase your patient satisfaction and compliance, join Toastmasters (toastmasters.org). Membership in Toastmasters is one of the best investments you can make in yourself. At $36 every six months, it is also one of the most cost-effective skill-building tools available anywhere.

A Toastmasters meeting is a learn-by-doing workshop in which participants hone their speaking and leadership skills in a no-pressure atmosphere. There is no instructor in a Toastmasters meeting. Instead, members evaluate one another’s presentations. This feedback process is a key part of the program’s success. Meeting participants also give impromptu talks on assigned topics, conduct meetings, and develop skills related to timekeeping, grammar, and Parliamentary procedure.

QUERIES (CLINCAL)

Query: Footwear Modifictions S/P 4th and 5th Ray Resections

What would be the best footwear modification for a patient status post partial 4th and 5th ray resections to protect the remaining foot? (Footwear = orthotic, shoe modification and/or brace. Any combination can be included.) Are there any studies comparing the differences in terms of weight distribution comparing an orthotic vs. shoe modification vs. a brace?

QUERIES (NON-CLINCAL)

Query: Policy for Handguns in a Podiatric Office

In light of the recent tragedy in Connecticut, what, if anything, should I do regarding a policy of handguns being allowed in my office? What about officers who are required to carry handguns when in uniform?

RESPONSES / COMMENTS - (NON-CLINICAL) - PART 1B

RE: Efficacy of "Pain" Creams (Allen Jacobs, DPM)
From: Tip Sullivan, DPM

I appreciate the input that I have seen on the topic of “pain creams” but have an interesting observation. It seems as though pain cream (which has a very low SE profile) is being advocated for use when po meds (with significant SE profiles) are not effective. In my mind, that does not seem rational. I think that the low risk meds (topical) should be at least attempted before the higher risk meds (po meds). I do not see primary care, neurologists, or endocrinologists using pain creams as their first line of attack.

I wonder if it is because of their experience with ineffectiveness, the cost, or their training. Is there anyone using this as the first line treatment of choice? Again, my experience is that the effectiveness of these creams is unreliable regarding who gets symptom relief and who does not. Should I allow the compounding pharmacist to make that decision?

Is there any compendium or data that one might use to help guide choices? As Dr. Jacobs pointed out, there seems to be data available regarding specific in vitro findings, but I seem to be having some problem finding valid studies that are not financially-influenced. As the Mayo Clinic site says, “Opinions differ on the effectiveness of over-the-counter topical pain medications. While many people say these products help relieve their arthritis pain, scientific research reveals only modest benefits. Some products work no better than placebo in relieving arthritis pain.”
