FL Podiatrist Discusses Plantar Fibromatosis
 
According to Felipe Peterson, DPM, "The classic presentation of plantar fibromatosis is an insidious slightly tender nodule in the medial or central band of the fascia of the foot that ranges anywhere from 0.5 to 3 cm in size. Other factors that should be taken into consideration when evaluating a plantar fibroma are any adjacent bony prominences which can be detected with an x-ray, tendon insertions, hindfoot alignment, as well as the presence of an Achilles or gastrocnemius muscle contracture, which can exacerbate symptoms.

"Aside from clinical evaluation, a combination of ultrasound and MRI foot diagnosis can be used to exclude other potential causes including a lipoma, fibrosarcoma, or cyst. An ultrasound is useful to show the depth and size of the nodules, says Dr. Peterson.

IL Podiatrist is Worried About Medicare Advantage 
 
Patrick McEneaney, DPM, owner and CEO of Crystal Lake-based Northern Illinois Foot & Ankle Specialists, recently discussed why he is worried about Medicare Advantage in 2024. "The healthcare trend that I'm most worried about in 2024 is the increasing number of patients who are signing up for Medicare Advantage Plans. I have found that my most vulnerable patients are picking up these plans. These are patients who have multiple co-morbidities, are at high risk for exacerbations of their medical conditions, and/or those who have insecurities with their socioeconomic situations."

"These plans are touted by brokers promising extended benefits and significant cost savings. However, the savings are often only on the front end of the program at the time they sign up. Many patients don't realize that these plans may have co-pays, co-insurance, and deductibles, which often cost the patients more in the long run. I see patients skipping visits, treatments, or diagnostic testing in fear of the costs associated with these plans, which leads to poorer outcomes. The aggressive advertisements and monetization of these patients' lives to brokers' commissions have led to a significant amount of misunderstanding from patients," said Dr. McEneaney.

Query: Post-Operative Complications
 
I performed a hallux interphalangeal joint fusion on a patient 65 days ago. Against medical advice, he was walking in a regular shoe from day one. I documented it and put in the non-compliance ICD-10 in my note as well. This past Friday, he showed up in my office with the screw nearly poking through his skin. I plan to remove it this week; however, I wanted to bill an E/M for the encounter during the global with a -24 for this encounter. I would use the ICD-10 T84.84XA, pain due to internal orthopedic prosthetic devices, implants, and grafts, initial encounter. Would this be justified or is this a complication that is included in billing for the surgery?

Response: A patient had a hallux interphalangeal fusion 65 days ago. The appropriate CPT code to bill is CPT 28755 which is defined as the following: Arthrodesis, great toe; interphalangeal joint. CPT 28755 is classified as a major surgical procedure code with a post-operative global period of 90 days. The patient presented to the office with the screw that was used for the fusion almost coming through the skin due to non-compliance. The patient is going to have surgery to remove the screw shortly, but the physician wants to bill an E/M service for the encounter in the office. Can this be done?
 
In order to bill an E/M service during the post-operative global period set by the CPT code/procedure, it is important to determine first whether or not the E/M service is directly related to the procedure. If the E/M service is not directly related to the procedure, the appropriate level of E/M service would be appended by the -24 modifier. The -24 modifier is defined as the following: unrelated E/M service during post-op period. Use this modifier (only on an E/M code) when you perform an evaluation and management service during the follow-up period of an unrelated surgical procedure. You are entitled to bill for an E/M service performed during the follow-up period if that service is not related to the original surgical procedure. In this case, add the -24 modifier to the E/M service code. Make sure you reference this service code to the appropriate unrelated diagnosis on the billing claim.
 
Unfortunately, ICD-10-CM code T84.84XA which would be linked to the E/M service is indeed directly related to the procedure that set the 90-day post-operative global period, and therefore the E/M service is not reimbursable.

RE: Are We Failing our Diabetic Patients?

Healthcare equity has been defined as a condition in which all people have a fair and equal opportunity to attain their highest level of health despite racial, social, cultural, gender, or geographic differences. In my opinion, we do not and have not had healthcare equity in the United States. Increasingly, my patients are being denied insurance coverage for needed medications for the optimal management of diabetes, wound care therapies for ulceration care, fixation devices for the stabilization of Charcot’s joint disease, home nursing care, and therapy. The result is lack of equity associated amputation, decreased quality of life, financial burden, and mortality. 
 
Sadly, we have billions of dollars for war, support of massive immigration populations, and the protection of endangered species, but not for proper care of those afflicted by the diabetes pandemic. In my opinion, there is no reasonable excuse to explain the lack of funding access for proper healthcare in care of the diabetic patient. From at-risk preventative care to intramedullary nails and external fixation to advanced wound care products, private insurances, the federal government, and other "third-party carriers" are increasingly failing to provide necessary care for our patients with diabetes and diabetes-related foot pathology. It is difficult to watch. 
 
Perhaps patients with diabetes should be reclassified as an “endangered species”, or we should declare “war on diabetes”, or we should send our diabetic patients to Mexico to cross the border and return declaring they are “seeking asylum from poor medical care.” Maybe under these circumstances, we can do more to prevent ulceration and infection and reduce the risk of amputation and death.

I want to take issue with some of Dr. Gurnick's opinions because he didn't tell us how he arrived at them. Weekly, I see patients presenting with failed orthotic treatments. Nearly 100% have the same defects: Arch height 50% or less of what it should be; shallow heel cups of 2-4 mm; external rearfoot posts that do not extend to the medial and lateral edges of the heel cups. Rarely are any of these orthotics made from a plaster cast. 
 
Seeing what I see, "Plaster casting, 'a unique approach,' is often considered a lost art form." appears to be all too true. More than that, far too many DPMs are unwilling to take the time to cast. They do no more than the chiropractor down the street.
