Consensus Guidelines Published for Off-Loading of DFUs
Relieving pressure on the foot through the use of "off-loading" improves healing outcomes and reduces complications in patients with diabetic foot ulcers (DFUs), according to consensus guidelines published today in the Journal of the American Podiatric Medical Association. "The Management of Diabetic Foot Ulcers Through Optimal Off-Loading – Building Consensus Guidelines and Practical Recommendations to Improve Outcomes," was authored by a multidisciplinary panel of nine experts who were supported by Derma Sciences, Inc. The document was developed to provide evidence-based consensus on the optimal use of off-loading as treatment for DFUs, a debilitating and costly condition that can lead to amputation of the foot.
|
|
Dr. Robert Snyder |
"Publishing this paper is intended to bridge the gap between evidence supporting the effectiveness of off-loading for diabetic foot ulcers and its use in clinical practice, and to guide the wound care decision-making process," said Robert J. Snyder, DPM, MSc, Professor and Director of Clinical Research at Barry University School of Podiatric Medicine and lead author of the consensus guidelines. "It is our hope that this manuscript will foster meaningful off-loading of DFUs by the specialist and generalist alike."

Ultra High Heels = Shoe-icide: NY Podiatrist
|

Dr. Hillary Brenner |
The doctors at WebMD are weighing in on what most women wear on their feet, ultra-high heels. "Heels are getting higher and higher," says Hillary Brenner, DPM. "We podiatrists like to call it shoe-icide." Dr. Brenner, a spokeswoman for the American Podiatric Medical Association, says ultra-high heels can lead to everything from ankle sprains to chronic pain.

MA Podiatrist Retires After 60 Years
Over the past 60 years, many Cape Ann fishermen have started and ended their careers, a couple of generations of patients have grown up and gotten older, and Gloucester has seen its fair share of businesses come and go, in its downtown and elsewhere. During those six decades, there has remained at least one constant, Dr. Silvester Smith, podiatrist. He has kept an office open downtown, and catered to the needs of patients since 1954.

In a couple of weeks, Smith — 89 years young — will fully retire. “I never thought it would end,” he said. What’s kept him going, Smith said, are his patients. “I’ve enjoyed every minute of it,” he said. “There wasn’t one bad situation that I can remember.”

FDA Issues Proposed Rule to Require Electronic Drug Prescribing Information
The Food and Drug Administration is issuing a proposed rule on drug and biological product labeling to require electronic distribution of the prescribing information intended for healthcare professionals, according to a notice to be published Dec. 18 in the Federal Register.
The FDA also said it is proposing that this prescribing information can no longer be distributed in paper form, except as provided by the regulation. The White House Office of Management and Budget has been reviewing the proposed rule since August 2013.

Query: Patients' Right to Paper Prescriptions
Do patients lose any rights under federal law to request paper prescriptions, so they can take them to different pharmacies to price compare?
Dan Chaskin,DPM, Ridgewood, NY
Editor's comment: PM News does not provide legal advice. Most prescriptions are covered by state law, which varies. Under most state laws, patients have the right to request paper prescriptions. Controlled substances are governed by federal law. Effective October 6th, 2014, the DEA stiffened the regulations regarding Schedule II drugs, which now require paper prescriptions. As noted in the pharmaceutical news article in today's issue, the FDA is seeking regulations to totally eliminate paper prescriptions.

Query: Retirement Questions
As of March 31, I will be retiring from my hospital position. I would like to keep my license active in case I want to pick up some work later on. Should I keep my DEA? Can I get malpractice insurance as needed. What about CAQH and NPI numbers? What about my DMERC license?

RE: MacPractice MD EMR
From: James Koon, DPM
Our office uses Mac computers and peripherals. We've been running MacPractice MD for three years now and remain very pleased with our purchase. MacPractice is the premier medical EMR/EHR for Macs. The program runs consistently and flawlessly, the peripherals work without glitches or hiccups, the support costs are so much lower than many others, the support is fast and efficient (though we never seem to need them except to answer user questions), and the forms are completely customizable.
We are beginning to use the (free) iPad ability as well. All updates are free. I very, very rarely need to call the IT people because I can do almost everything here myself. Since it's Mac, it just works. The program has an extensive library of educational videos and written explanations for every aspect of the program. They offer free weekly video conferences/lessons on different aspects of the program. The reports are detailed and customizable, the security is top notch and the visual appeal of the program is pure Mac.
We backup to a Drobo RAID tower nightly and weekly to a thumb drive kept securely off premises. It's cost-effective. The interfaces for the patient portal, the attachments ability (for patient photos and clinical photos as well as letters, forms, etc.), and the imaging ability are superior to anything else I've seen. The eRx is awesome.
Disclosure: I have no financial relationship to MacPractice MD.

RE: Seven Surgeonisms
1. All bleeding eventually stops.
2. A very bold surgeon is one who realizes that his patient takes all the risks.
3. It takes 5 years to know when to operate, and 20 years to learn when not to.
4. Don't look for things you don't want to find.
5. Surgery, like making love, should be done gently with adequate exposure.
6. The lesser the indication, the greater will be the complication.
7. It is better to be lucky than good.
