Minimally Invasive Surgery - Wave of the Present: MI Podiatrist
According to Conway McLean, DPM, "Podiatry was an early adopter of minimally invasive procedures, although the complication rate was high since fluoroscopic technology wasn’t in regular use. With time, the benefits of MIS have become well-recognized in foot surgery and consequently a significant trend. Many foot procedures not deemed possible through small incisions are now everyday occurrences."
|
|
Dr. Conway McLean |
"A chronic callus on a toe, a loose ligament in a knee, a clogged artery in the leg, these and so many more are all problems well treated utilizing the principles of minimally invasive surgery. All these operations can be performed without a long incision and the associated damage to multiple nerves and arteries, fascia, and lymph vessels. Not all procedures can be performed with MIS methods, but when they can be, it just makes sense. And the list is growing all the time," says Dr. McLean.

NM Podiatrist Writes Memoir
This is a story of adventure, travel, unexpected professional disappointments, perseverance, and determination. A doctor's life is often romanticized as a life of wealth and status, free from the ordinary politics of the workplace. This memoir of this time reveals the true underbelly of life as a government employed doctor. You will also get an insider's view of what it is like to live through a medical malpractice trial and the subsequent effect on a doctor's professional life.

| PRACTICE MANAGEMENT TIP OF THE DAY | |
5 Ways to Implement AI in Your Practice
Given the growing awareness of how powerful and useful artificial intelligence (AI) can be, healthcare organizations have good reason to try to leverage its vast capabilities. But figuring out how exactly to use this cutting-edge technology to address podiatric care-specific needs is no small feat. As your practice implements AI, there are five key steps to take in order to make sure that technology simplifies this transition.
Step 2: Choose tech that is well-suited to meet your objective.
Because podiatric care has very different technological needs from other fields (and even from other areas of healthcare), it’s important to choose an AI-powered solution that meets those needs. Clinical tools built exclusively on generic large language models will not be able to address complex clinical use cases adequately. To get the required accuracy and precision for a clinical setting, podiatry practices need podiatric-specific AI tools, which can either be homegrown from the ground up or undergo significant fine-tuning.
When considering an AI solution, it’s also important to look into the vendor and make sure you’re a good match. Pay attention to the vendor’s track record, references, how long it will take to deploy the technology fully, and how disruptive its rollout will be to your practice.

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 1 | |
RE: Definition of Podiatry
From: Allen M. Jacobs, DPM
What is the definition of podiatry? A podiatrist is educated at the didactic and clinical level to diagnose and treat disorders of the foot, and in some states, the ankle and lower leg. The willful misinterpretation of the limited scope of practice of a podiatrist by some DPMs does not justify the practice of general medicine by a podiatrist. Example: a patient presents to you with tingling and burning paresthesia. You rule out local nerve entrapment. You order electrodiagnostic studies. The studies demonstrate a peripheral sensory neuropathy. Or you perform an epidermal nerve fiber density study. The test is consistent with small fiber neuropathy.
Now what? This is not pathology intrinsic to the foot. Whatever the etiology, as a podiatrist, you are not educated to proceed with further...
Editor's note: Dr. Jacobs' extended-length letter can be read here.

|
| RESPONSES/COMMENTS (NON-CLINICAL) - PART 2 | |
From: Steven Finer, DPM, H. David Gottlieb, DPM
Dr. Jacobs is completely correct. MDs making half a million work their tails off. The real money people are the CEOs of drug corporations making fortunes. It is not made through salaries but by stock options. Read the listed groups and you will see the money made when these people pay pennies per share for their options.
Steven Finer, DPM, Philadelphia, PA
Fellow podiatrists seem to like to bitch and complain about their lack. Lack of income, lack of perceived respect, whatever. The only person who can change both your situation and your happiness is ... YOU. I suggest changing your situation or finding a good therapist.
H. David Gottlieb, DPM, Baltimore, MD

| RESPONSES/COMMENTS (OBITUARIES) - PART 1A | |
I am totally saddened at the news of SuEllen's passing. I have been in this profession since the late 1970s. Few people have had such a positive and consistent impact on aspects of my practice as SuEllen and her husband, David. Yes, she was there to greet me when I came to so many conventions and quickly let me know if they had any new products to benefit my patients. Samples would be in the mail if I asked for a new product. It was that personal and professional relationship that made it so special.
A couple of months ago, I shed a tear when they announced they were retiring and closing Gordon Labs. They wanted to enjoy their lives doing other things. I had no idea that she was ill. It was a blessing to have known and worked with SuEllen and David for so many wonderful years. May SueEllen's memory be a blessing.

| RESPONSES/COMMENT (OBITUARIES) - PART 1B | |
RE: The Passing of SuEllen Dercher
I’m so sorry to hear of the passing of SuEllen Dercher. I first met her when I was a student in the mid-1980s. She was a wonderful representative of Pedinol and a kindhearted person who I always appreciated seeing when she exhibited at seminars; first with Pedinol and then after she and her husband took over Gordon Labs. She must have known thousands of DPMs on a first name basis. She was always absolutely pro-podiatry and upbeat and could easily make you smile. I’m happy to have been acquainted with her for all these years and will sincerely miss her sweet, helpful, knowledgeable, and loving attitude.
My warmest regards and prayers go out to her husband David and their family. She was extremely special and will be remembered fondly by all who were fortunate to have met her and were befriended by her over the years.

RE: The Passing of SuEllen Dercher
SuEllen was a dear friend of mine for over 42 years. We met when she visited my practice as a rep for Pedinol Labs. SuEllen had such a love for everyone and always went out of her way to achieve perfection. She was the nicest person I have ever met – kind, generous, and caring.
Over the years SuEllen did so much for podiatry - donating Gordon Lab products, donating first-year podiatry students white jackets and much more. I miss her terribly and my condolences to her husband, Dave and her son, Andrew.

RE: Outrageous Post-op Boot of the Day
This patient is 8 days post-op right foot bunionectomy with 1st metatarsal osteotomy with internal screw fixation. Here is what she said to me Friday, on her first post-op visit: "All I want for Christmas is this boot off."
