PODIATRISTS IN THE NEWS
NJ Podiatrist Discusses Athlete's Foot

Athlete's foot is probably the best-known infection you can pick up at the gym. "It's an itchy rash that's usually red, inflamed, and scaly. It's highly contagious and can be caught anytime you're barefoot, in open-toed shoes, or wearing wet socks," says podiatrist, Velimir Petkov, DPM, owner of Premier Podiatry in Clifton, New Jersey.

Dr. Velimir Petkov

"To avoid getting athlete's foot, avoid going barefoot in common areas and keep your feet dry," Petkov says. The fungus thrives in warm, wet environments so he suggests washing and drying your feet regularly, changing your socks after exercise, wearing shoes in all common areas in the gym, allowing your training shoes time to dry before re-use, regularly cleaning your gym bag, and wearing flip-flops in the shower.

PODIATRISTS IN THE COMMUNITY - PART 2
3 Injured When Car Crahes into NJ Podiary Office

Three people were injured inside a doctor’s office on Route 70 Thursday morning, when a car crashed into the building, according to police. Helen A. Bach, 78, was attempting to park her 2007 Hyundai Sonata at the Medical Associates of Marlboro Internal Medicine, Cardiology and Podiatry. An investigation into the accident has determined that Bach’s foot slipped off the brake and onto the gas pedal, said Capt. Todd Malland of the Manchester Township Police Department.

The Hyundai was then propelled over a curb and through the brick and glass front of the building, injuring three patients in the waiting room, Malland said in the prepared statement. The force of the impact knocked a 51-year-old Brick woman and an 88-year-old Manchester woman out of their seats and onto the floor. Both were seated adjacent to the impact area, he said. Another patient, a 76-year-old Haledon resident, was also hurt in the crash, he said.

PODIATRISTS AND SPORTS
UT Podiatrist was Multi-sport All-Star

Sports have been a large part of Dr. Tara Harrington’s life. While participating in soccer, basketball, and track and field at Mazama High in Klamath Falls, she earned All-State honors in all three sports and Southern Oregon Conference Player of the Year in basketball and soccer. Besides being a 4.0 student, she was Oregon’s first high school All-American girls basketball player in 1993. After graduation, Harrington played basketball at Stanford University.

Dr. Tara Harrington

While coaching basketball at Cal State and Montana State and working at a consulting business, she received an opportunity to play professional basketball in Germany and Sweden. The experience changed her life when the emotional toll of her mother’s death caused Harrington to switch to podiatric sports medicine. She is now a Doctor of Podiatric Medicine specializing in foot and ankle surgery.

PRACTICE MANAGEMENT TIP OF THE DAY
Strategies for Better Billing and Collections - Part 5

Here are some resolutions that can improve your billing and collections in the new year.

5.  Resolve to review your contracts.

Take a little time at the first of the year to review your contracts with payers. Make sure you are aware of any changes from the prior year, especially if those changes require a change in workflow, process, or staff (re)training. Note all filing deadlines for the year ahead. This is also a good time to make a chart of when your contracts come due, so you can plan ahead for any re-negotiations..

RE: Arthrogram of the 2nd MPJ for Partial Plantar Tear
From Kenneth Meisler, DPM

In my office, we almost exclusively use ultrasound to diagnose 2nd MPJ plantar plate tears. We have done this many hundreds of times. In addition, I have seen many patients who were treated by other doctors for 2nd MPJ pain. Many of those patients had MRIs or ultrasounds performed to rule out a plantar plate tear. I have not seen a patient who had an arthrogram of the 2nd MPJ in many years.

I use two Hospital for Special Surgery fellowship-trained musculoskeletal radiologists for my MRIs and CT scans, each from a different radiology practice. They both prefer ultrasound to evaluate the 2nd MPJ for plantar plate tears. In spite of the lower payment they receive as compared to MRI, they feel that in most cases, it is better for evaluating plantar plate tears.

A podiatrist should be able to perform these ultrasounds in his/her office. However, it requires a very good ultrasound machine plus experience and/or training in evaluating for this injury. Every podiatrist will not be able to perform this because they will not have been trained to do it, have enough patients that require it regularly, or have an ultrasound machine that can evaluate for it. My suggestion is to get trained to diagnose plantar plate tears or refer the patient out to a radiologist, ideally fellowship-trained in musculoskeletal ultrasound, and ask them which works better in their hands.

RE: Reported Increase in Non-traumatic Amputations in People with Diabetes will Challenge Podiatric Medicine
From: Leonard A. Levy, DPM, MPH

An increasing challenge to podiatric medicine is occurring. Health News (December 12, 2018) reported that there are a growing number of people with diabetes in the U.S. losing toes and feet to the disease by middle age. A study suggests a reversal after years of progress against diabetes. From 2000 to 2009, the rate of non-traumatic lower extremity amputations fell by 43 percent, from 5.4 cases to 3.1 cases for every 1,000 adults in the U.S with diabetes.

But then amputations increased by 50 percent between 2009 and 2015, to 4.6 cases for every 1,000 adults with diabetes and was most pronounced in younger adults, ages 18 to 44, and in middle-aged adults, 45 to 64, researchers report in Diabetes Care. The senior author Edward Gregg of the Centers for Disease Control and Prevention (CDC) in Atlanta, stated, “This is the first time we have observed an increase in amputations.”

Man Develops Fungal Infection in Lungs from Sniffing Smelly Socks

In a cautionary tale for us all, a man in Fujian province was recently hospitalized due to his habit of sniffing his smelly socks every afternoon upon coming home from work.

After he arrived at a hospital in the city of Zhangzhou, doctors discovered that the ill man was suffering from a fungal infection in his lungs. A doctor told local reporters that this infection was apparently caused by the man’s unusual sock-sniffing obsession.
